Bearing the Weight of End-of-Life Work: A Qualitative Study of Emotional Experience and Coping Among Palliative Care Professionals in Northern Italy.

Background: Palliative care is a clinically, relationally and emotionally demanding field. Continuous exposure to suffering and death exposes professionals to compassion fatigue, vicarious trauma and grief, and burnout, while also offering opportunities for compassion satisfaction and meaning-making. How professionals cope is central to sustaining the supporting role in diseases with a poor prognosis. Objectives: To explore the emotional and experiential dimensions of palliative care work and the coping strategies, individual and team-based, that professionals deploy when accompanying patients

From Control to Connection: Reflections on Implementing Safe Steps in Acute Mental Health Units.

This paper presents a reflective account of the implementation of the Safe Steps for De-escalation practice model over a 12-month period across three acute mental health units. It describes the implementation process of the model, designed to reduce restrictive practices in acute mental health settings. The Safe Steps model is a mental health nurse-designed and led approach for person-centred, structured and relational de-escalation. It positions therapeutic engagement, emotional attunement, and psychological safety as central components of effective responding, particularly during high-stress

Enhanced Recovery After Surgery (ERAS) Protocols in Pancreatic Cancer Resections: Their Impact on Postoperative Morbidity.

The sole treatment for pancreatic cancer, which is a hazardous condition, is surgery, which has a high postoperative morbidity rate. Traditional perioperative care often makes the healing process more difficult and increases the risk of problems. The Enhanced Recovery After Surgery (ERAS) protocols that were created to address colorectal operations are intended to alleviate surgical trauma and promote recovery based on evidence-based, multimodal interventions. The current review will focus on the ongoing problem of the high rate of pancreatic resection morbidity and consider the effectiveness

[Chyle leak in a hemodialysis tunneled catheter without associated chylothorax: An exceptional complication].

The presence of chyle in a hemodialysis catheter is a rare complication, usually observed in case of chylothorax secondary to central vein stenosis, particularly of the superior vena cava and the left brachiocephalic vein. We report the exceptional case of a 55-year-old woman in maintenance hemodialysis presenting with chyle in her left subclavian tunneled cuffed catheter during each interdialytic period, without any recent history of trauma or tumoral disease. The chest CT scan was unremarkable. The milky fluid was systematically observed by the nurses at the beginning of each hemodialysis se

Association of Childhood Abuse With Incident Inflammatory Bowel Disease.

INTRODUCTION: A link between inflammatory bowel disease (IBD), stressful life events, and psychological factors has previously been reported. Our objective was to examine the relationship between emotional, physical, and sexual abuse of childhood and risk of IBD using a large cohort of female health professionals. METHODS: We included participants in the Nurses' Health Study II who completed the Physical and Emotional Abuse Subscale of the Childhood Trauma Questionnaire and the Sexual Maltreatment Scale of the Parent-Child Conflict Tactics Scale in 2001. Diagnosis of IBD was determined by sel

War is the Enemy of Health. Pulmonary, Critical Care, and Sleep Medicine in War-Torn Syria.

The Syrian crisis, now in its fifth year, has created an unprecedented strain on health services and systems due to the protracted nature of the warfare, the targeting of medics and health care infrastructure, the exodus of physicians and nurses, the shortage of medical supplies and medications, and the disruption of medical education and training. Within a few short years, the life expectancy of resident Syrians has declined by 20 years. Over the first 4 years of the conflict, more than 75,000 civilians died from injuries incurred in the violence. More than twice as many civilians, including

Closed traumatic rupture of the thenar muscles from the origin: a case report and review of the literature.

Closed traumatic rupture of the thenar muscles is an unusual and rare injury. Traumatic musculotendinous injuries in the hand and wrist occur primarily from penetrating trauma. Only 2 such cases were identified in medical literature. We report a case of closed traumatic rupture of the thenar muscles in an otherwise healthy 33-year-old female nurse who sustained a hyperabduction injury of her right thumb and wrist during a daily occupational routine, resulting in complete avulsion of the right abductor pollicis brevis and opponens pollicis from their origins. After declining initial surgical ma

Barriers to implementing intermittent catheterisation in spinal cord injury patients in Northwest Regional Spinal Injuries Centre, Southport, U.K.

Intermittent catheterisation is the preferred method of managing the neurogenic bladder in patients with spinal cord injury. However, spinal cord physicians experienced problems when trying to implement an intermittent catheterisation regime in some spinal cord injury patients in the northwest of England. We present illustrative cases to describe practical difficulties encountered by patients while trying to adopt an intermittent catheterisation regime. Barriers to intermittent catheterisation are (1) caregivers or nurses are not available to carry out five or six catheterisations a day; (2) l

Dermatological conditions presenting at an emergency department in Singapore.

INTRODUCTION: In Singapore, a significant proportion of patients receive specialist dermatological services via referrals from points of primary care, such as polyclinics and emergency departments (ED). The study hospital, Tan Tock Seng Hospital, is an acute care general hospital with a large catchment area, and has the busiest ED in Singapore. The aim of this paper was to describe the types of dermatological conditions presented at the ED in the year 2007. This information is useful for the future education of junior doctors working in the department, as well as for the allocation of future r

Hourly neurologic assessments for traumatic brain injury in the ICU.

OBJECTIVES: Hourly neurologic assessments for traumatic brain injury (TBI) in the critical care setting are common practice but prolonged use may actually be harming patients through sleep deprivation. We reviewed practice patterns at our institution in order to gain insight into the role of frequent neurological assessments. METHODS: A 6-month retrospective review was performed for patients who were admitted to an intensive care unit (ICU) with the diagnosis of TBI. Electronic medical records were reviewed based on billing codes. Variables collected included but were not limited to patient d

Association of Posttraumatic Stress Disorder With Accelerated Cognitive Decline in Middle-aged Women.

IMPORTANCE: Posttraumatic stress disorder (PTSD) has been hypothesized to lead to impaired cognitive function. However, no large-scale studies have assessed whether PTSD is prospectively associated with cognitive decline in middle-aged adults. OBJECTIVE: To assess the association between PTSD and decline in cognitive function over time. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included participants from the Nurses' Health Study II, an ongoing longitudinal cohort study involving community-dwelling middle-aged female nurses residing in the US who had at least a 2-year nursing degre

Team Situation Awareness Within the Medical Field Context: A Scoping Review.

BACKGROUND: In the medical field, the lack of team situation awareness is a substantial cause of communication failures, decision-making delays and treatment errors, particularly in emergency scenarios such as resuscitation and trauma management. Although this concept has been extensively studied in aviation and military fields, its application and efficacy in complex medical environments remain in a phase of rapid development and faces challenges. AIMS: To conduct a scoping review of team situation awareness research in the medical field. STUDY DESIGN: This scoping review followed Arksey an
