BACKGROUND: Yaws is a chronic, highly contagious skin and bone infection affecting children living in impoverished, remote communities and caused by Treponema pallidum subspecies pertenue. The Philippines was thought to be free of yaws following the 1950s eradication campaign but it has been reported in the Liguasan Marsh area, Central Mindanao. Neurosyphilis results from infection of the nervous system by Treponema pallidum. The diagnosis of neurosyphilis is overlooked because of its rarity. It can manifest as early or late symptoms after primary infection with syphilis, with symptoms lying on the spectrum of asymptomatic early neurosyphilis, syphilitic meningitis, meningovascular syphilis, general paresis, and tabes dorsalis. In this report, we present a case of neurosyphilis presenting with acute confusion and agitation.

Syphilis is a chronic systemic infectious disease. Gastric involvement is uncommon. We report of a 55-year-old female patient with a history of epigastric pain and a diagnosis of Syphilis confirmed by main clinical symptoms, tolulized red unheated serum test(TRUST) and treponema pallidum hemagglutination assay(TPHA). At the same time, we still reported gastric involvement about syphilis in this patient with gastroscopy and pathology.

A 41-year-old man was referred because of a slightly pruritic eruption, and a classic case of pityriasis rosea was diagnosed on the first examination. Results of routine tests for syphilis were strongly positive, and on repeated examination the eruption had grown further. Skin biopsy specimens showed plasma cells and Treponema pallidum. All symptoms disappeared following antisyphilitic treatment. The possibility of an isomorphic response is discussed.

We evaluated hematological findings, with special reference to the monocyte count, in ten infants with congenital syphilis. One patient had a monocytic leukemoid reaction and eight had absolute monocyte counts considerably higher than normal control infants. Monocytes appear to be an essential participant in the cellular reaction to treponema pallidum. To our knowledge, this is the first report of a monocytic leukemoid reaction in congenital syphilis.

Secondary syphilis can compromise many organs and is widely known for manifesting with classic signs, such as a rash that involves palms and soles along with mucosal lesions. The absence of these manifestations, along with false negative serological tests, as seen with the prozone phenomenon, poses a difficult challenge for the clinician. When skin lesions are visible, a biopsy for pathological description and immunohistochemistry for Treponema pallidum may help aid in the confirmation and certainty of the diagnosis.
