[Mycobacterium persicum pulmonary disease misdiagnosed as tuberculosis: a case report].

Mycobacterium persicum (M. persicum) is a nontuberculous mycobacterium that predominantly affects the lungs, causing pulmonary lesions, and is readily misdiagnosed as tuberculosis. Here we report a 48-year-old man with AIDS and M. persicum pulmonary disease who was initially misdiagnosed with pulmonary tuberculosis and received standard anti-TB therapy, yet showed no radiological or clinical improvement. Sputum specimens were subsequently subjected to targeted next-generation sequencing (tNGS), which identified M. persicum and established the definitive diagnosis. After switching to an anti-M.

Pulmonary rehabilitation in post-tuberculosis lung disease: a systematic review.

OBJECTIVE: Tuberculosis (TB) remains a significant global health concern, resulting in illness and death for millions of individuals. Despite the availability of effective treatment options, a substantial number of people continue to suffer from long-term lung consequences known as post-tuberculosis lung disease (PTLD). PTLD may be associated with reduced exercise capacity, respiratory impairment, and diminished quality of life. Pulmonary rehabilitation (PR), an intervention that combines exercise and education to enhance pulmonary function and overall wellbeing, may offer a solution. The aim

YOLOv10-FMW: A Multiscale Convolutional Detection Method for Pulmonary Tuberculosis.

Early detection and treatment of minimal pulmonary tuberculosis (mPTB) are essential for preventing disease progression and transmission. However, the detection of mPTBs on computed tomography (CT) remains challenging because the lesions are often small, low in contrast, morphologically heterogeneous, and characterized by subtle manifestations. This study aimed to develop and evaluate a dedicated deep learning framework for detecting mPTB lesions on chest CT. The model was trained and validated using data collected from three hospitals and externally evaluated using an independent dataset from

Host- and pathogen-related determinants of pulmonary versus extrapulmonary tuberculosis.

Tuberculosis (TB) primarily manifests as pulmonary TB (PTB), but extrapulmonary TB (EPTB) remains a major clinical challenge. Distinct diagnostic and therapeutic difficulties arise from differences in immune responses, pathogen behaviour and host susceptibility. However, the factors driving disease localisation are still incompletely understood. We conducted a comprehensive narrative review of studies examining differences between PTB and EPTB in terms of epidemiology, mycobacterial factors, genetic and epigenetic determinants, host immune responses, transcriptomic profiles, cytokine and chemo

Arteriovenous fistula originating from a Rasmussen aneurysm: Beyond pulmonary tuberculosis.

Life-threatening hemoptysis is a serious complication of cavitary pulmonary tuberculosis and can result from multiple coexisting mechanisms, including vascular lesions and chronic pulmonary aspergillosis (CPA), which is frequently missed in post-tuberculosis lung disease. Therefore, we present the case of a 23-year-old immunocompetent man with cavitary pulmonary tuberculosis who developed massive hemoptysis with hemorrhagic shock and acute respiratory failure requiring mechanical ventilation. Emergency angiography identified a arteriovenous fistula and a Rasmussen aneurysm of the right pulmona

Primary Tracheal Rosai-Dorfman Disease Coexisting With Active Pulmonary Tuberculosis.

Rosai-Dorfman disease (RDD) rarely involves the trachea, and active pulmonary tuberculosis (TB) has not been reported with tracheal RDD. A 35-year-old woman presented with progressive cough, wheezing, and shortness of breath for 1 month. Chest CT revealed a tracheal mass causing > 90% airway narrowing and left lung nodules suggestive of TB. Emergency bronchoscopic resection using high-frequency electrocautery relieved the obstruction. Histopathology showed emperipolesis and immunohistochemistry was consistent with RDD. Bronchial aspirate was positive for Mycobacterium tuberculosis complex by c

An Atypical Case of Thoracic Endometriosis Syndrome Mimicking Pulmonary Tuberculosis.

Background: Thoracic endometriosis syndrome (TES) is a rare form of endometriosis characterized by the presence of functioning endometrial tissue in the thoracic cavity. Patients are women of reproductive age, with a genetic link as a significant risk factor. Patients present with long-standing chest symptoms and signs that mimic pulmonary tuberculosis (PTB). The crucial issue for establishing the diagnosis is the cyclicity of signs and symptoms which occur along with the menstrual cycle. Clinical Case: A 34-year-old businesswoman had recurrent pelvic pain and heavy menses for 6 years, recurre

Lower Lobe, Higher Suspicion: An Atypical Presentation of Pulmonary Tuberculosis.

Pulmonary tuberculosis (PTB) is a lower respiratory tract infection that most commonly presents as an upper lobe cavitation on imaging. However, atypical lesions, such as lower lung field tuberculosis (LLF-TB), can occur in some individuals, especially with underlying risk factors and comorbidities. We report a case of a 26-year-old male construction worker who presented with multiple episodes of hemoptysis, night sweats, high-grade fevers, and unintentional weight loss. Upon arrival, a complete medical history was taken, which was insignificant except for occupational exposure to silica parti

LncRNA C5orf64 polymorphisms (rs12518552 and rs2950218) decreases pulmonary tuberculosis susceptibility.

BACKGROUND: Pulmonary tuberculosis (PTB) remains a significant global health issue, with genetic factors playing a crucial role in susceptibility. Long noncoding RNA (lncRNA) C5orf64 has been implicated in immune responses and cancer, but its association with PTB risk has not been fully explored. METHODS: Genomic DNA was extracted from peripheral blood samples of 955 participants (474 PTB cases and 481 controls). Rs12518552 and rs2950218 in C5orf64 were genotyped using the Agena MassARRAY system. Logistic regression analysis was performed to assess the association between these polymorphisms

Diagnostic potential of WNT signaling gene methylation in pulmonary tuberculosis.

OBJECTIVES: The WNT signaling pathway plays important roles in the pathophysiology of pulmonary tuberculosis (PTB). This study aimed to assess the association of the DNA methylation levels of WNT signaling pathway genes and PTB susceptibility, and determine whether the methylation levels might serve as valuable biomarkers for PTB diagnosis. METHODS: We collected blood samples from 98 PTB patients and 96 normal controls, and extracted DNA samples. The methylation levels of promoter region in WNT signaling pathway genes (SFRP1, WNT3A, CTNNB1, WIF-1, DKK-1, LRP5, LRP6) were detected by MethylTar

Radiological features of chronic pulmonary histoplasmosis: Easily mistaken for tuberculosis.

Histoplasma capsulatum var capsulatum is an endemic respiratory pathogen presenting in various forms including miliary histoplasmosis, acute and chronic pulmonary histoplasmosis, and acute or subacute disseminated disease. The differential diagnosis of chronic pulmonary histoplasmosis (CPH) is broad, encompassing bacterial, fungal and malignant aetiologies. PubMed was searched for relevant articles on the radiological characteristics of CPH and the most common differential diagnoses of tuberculosis and chronic pulmonary aspergillosis. Distinguishing infectivity in patients with pulmonary tuberculosis using deep learning.

INTRODUCTION: This study aimed to develop and assess a deep-learning model based on CT images for distinguishing infectivity in patients with pulmonary tuberculosis (PTB). METHODS: We labeled all 925 patients from four centers with weak and strong infectivity based on multiple sputum smears within a month for our deep-learning model named TBINet's training. We compared TBINet's performance in identifying infectious patients to that of the conventional 3D ResNet model. For model explainability, we used gradient-weighted class activation mapping (Grad-CAM) technology to identify the site of les

Pulmonary tuberculosis associated with immune checkpoint inhibitors: a pharmacovigilance study.

We examined case reports of immune checkpoint inhibitors (ICIs) associated pulmonary tuberculosis (PT) using data from the Food and Drug Administration Adverse Event Reporting System database. Disproportionality analysis was performed by using the reporting OR (ROR) with relevant 95% CI. A total of 74 cases of PT related to ICIs therapy were identified. ICIs were significantly associated with over-reporting frequencies of PT (ROR=3.16, 95% CI: 2.51 to 3.98), while the signal was differed between anti-programmed death-1/ligand-1 and anti-cytotoxic T lymphocyte antigen-4 agents. Most indications
