Ureteroscopy in children: is there a need for ureteral dilation and postoperative stenting?

INTRODUCTION: Ureteroscopic stone manipulation and extraction is the standard of care for distal stone disease in the adult population. Recently, with refinements in instrumentation, these standards have been applied in pediatrics. Here, we investigate the role of ureteral dilation and the need for postoperative stenting after ureteroscopy. MATERIALS AND METHODS: Twenty-nine children (21 male, eight female) with a mean age of 11.0 (2.5-17.5) years underwent 34 ureterscopic procedures (21 right, 13 left) to address ureteral stones in 27 (23 distal, 3 mid and 1 proximal), surveillance of the up

Upper tract transitional cell carcinoma following treatment of superficial bladder cancer with BCG.

Eighty-two consecutive patients treated with intravesical bacillus Calmette-Guerin (BCG) for recurrent superficial bladder tumors were evaluated for development of metachronous upper tract tumors (UTT). All patients had normal upper tract studies within three months of starting BCG treatment. With a median follow-up of sixty-two months (range 25 to 124), 11 patients (13.4%) were found to have UTT. The median interval between initiation of BCG therapy and diagnosis of the UTT occurrence was thirty-eight months (range 7 to 110). All patients were asymptomatic when the UTT was diagnosed. An abnor

Kidney-sparing immunotherapy for lynch syndrome-associated upper tract urothelial carcinoma following colon cancer: a case report.

Lynch syndrome (LS) is a hereditary cancer predisposition syndrome associated with multiple malignancies, including colorectal cancer and upper tract urothelial carcinoma (UTUC). We report a 61-year-old woman who initially presented with ascending colon adenocarcinoma and underwent laparoscopic right hemicolectomy. Immunohistochemistry showed loss of MLH1 and PMS2 expression, suggesting deficient mismatch repair. Ten months later, she developed a right renal pelvic mass, which was confirmed by ureteroscopic biopsy as invasive high-grade urothelial carcinoma with squamous differentiation rather

Optimizing antibiotic duration and percutaneous nephrostomy timing in urosepsis complicated by upper ureteral stones: a retrospective study.

BACKGROUND: Urosepsis is a potentially life-threatening condition caused by urinary tract infections and is usually associated with urolithiasis-induced obstruction. No established guidelines exist regarding the optimal timing of antibiotic administration, appropriate duration of urinary drainage, or scheduling of definitive ureteroscopic lithotripsy (URSL) in patients with urolithiasis-induced obstructive acute pyelonephritis (APN). Therefore, this study aimed to investigate the key factors influencing the management of urosepsis secondary to upper ureteral stones, specifically focusing on th

Enhanced stone-free rates with suctioning ureteral access sheath vs. traditional sheath in retrograde intrarenal surgery: a systematic review and meta-analysis.

BACKGROUND: As a safe approach to the upper urinary tract, flexible ureteroscopic lithotripsy (fURL) is a widely accepted treatment for nephrolithiasis. Sometimes, this technique can rely on the natural expulsion of stones, increasing the risk of infections and stone recurrence. To mitigate these issues, some studies tried to use a suctioning ureteral access sheath (S-UAS). METHODS: A systematic review was conducted across multiple databases for trials comparing S-UAS with traditional (T-UAS) in retrograde intrarenal surgery (RIRS). The primary endpoint was the stone-free rate (SFR), while ad

Does Duty Cycle Modification Affect Temperature During Thulium Fiber Laser Lithotripsy?

OBJECTIVES: Although the thulium fiber laser (TFL, 1940 nm) efficiently fragments stones, concerns have been raised regarding heat production. Recently, a TFL with a modified duty cycle (MDC) was designed to reduce heat generation. The purpose of this study was to compare heat generation between the MDC and conventional TFL. METHODS: Ten millimeter BegoStone phantoms were placed in the proximal ureter of a kidney and ureter model. The model was submerged in a 35°C saline bath, with continuous saline irrigation (22°C) maintained at 15 mL/min through a ureteroscope. Temperature was measured usi

Retained Intrarenal Guidewire Fragment After Endourological Stone Surgery: Antegrade Percutaneous Snare Retrieval and Literature Review.

Background and Clinical Significance: Retained intrarenal foreign bodies are rare adverse events after endourological stone surgery. Guidewire fracture or detachment is uncommon and can trigger infection, obstruction, or encrustation if unrecognized. We report antegrade percutaneous snare retrieval of a retained hydrophilic guidewire tip and provide a concise literature review (seven PubMed-indexed intrarenal cases identified by a structured search) to inform diagnosis, management, and prevention. We also clarify the clinical rationale for an antegrade versus retrograde approach and the sequen

The Effect of Preoperative Tamsulosin on Ureteral Navigation, Operation, and Safety: A Systematic Review and Meta-Analysis.

INTRODUCTION: Urolithiasis is one of the most common diseases in the world, and at present, ureteroscopy (URS) is the first choice for its treatment. Although the effect is good, there is a risk of insertion failure of ureteroscope. Tamsulosin, as an α-receptor blocker, has the function of relaxing ureteral muscles, and can help stones to be discharged from ureteral orifice. In this study, we aimed to determine the effect of preoperative tamsulosin on ureteral navigation, operation, and safety. METHODS: This study was conducted and reported according to the meta-analysis extension of the Pref

Impact of Preoperative Ureteral Stenting in Retrograde Intrarenal Surgery for Urolithiasis.

Background and Objectives: Ureteral stent insertion passively dilates the ureter. Therefore, it is sometimes used preoperatively before flexible ureterorenoscopy to make the ureter more accessible and facilitate urolithiasis passage, especially when ureteroscopic access has failed or when the ureter is expected to be tight. However, it may cause stent-related discomfort and complications. This study aimed to assess the effect of ureteral stenting prior to retrograde intrarenal surgery (RIRS). Materials and Methods: Data from patients who underwent unilateral RIRS for renal stone with the use o

Biological and prognostic implications of biopsy upgrading for high-grade upper tract urothelial carcinoma at nephroureterectomy.

OBJECTIVES: Technical limitations of ureteroscopic (URS) biopsy has been considered responsible for substantial upgrading rate in upper tract urothelial carcinoma (UTUC). However, the impact of tumor specific factors for upgrading remain uninvestigated. METHODS: Patients who underwent URS biopsy were included between 2005 and 2020 at 13 institutions. We assessed the prognostic impact of upgrading (low-grade on URS biopsy) versus same grade (high-grade on URS biopsy) for high-grade UTUC tumors on radical nephroureterectomy (RNU) specimens. RESULTS: This study included 371 patients, of whom 11

Study of modified technique of ileal neobladder-Frog neobladder.

PURPOSE: Orthotopic neobladder is a well-established technique for continent urinary diversion after radical cystectomy. In this study, we evaluated a new Frog ileal neobladder technique. Since the reconstructed neobladder appears like a frog, the name Frog Neobladder was given to it. We have used two isoperistaltic ileal segments and implanted ureters in the nondetubularized proximal end of the ileal segment. SUBJECTS AND METHODS: This was a prospective, single-center (tertiary care hospital) study conducted from February 2008 to January 2018. Study patients were aged 39-94 years with biopsy
