Between 1988 and 1996, 23 male patients with bladder cancer underwent bladder substitution after cystectomy, using either the hemi-Kock, Hautmann, and Reddy procedures. The mean postoperative follow-up period was 36 months, with a range of 3 to 85 months. There were no perioperative deaths, and early postoperative complications occurred in 7 patients (30%); transient urine leak from the pouch in 4, wound infection in 3 and pyelonephritis in 2 patients. Between the years 1984 and 1993, a total of 72 female patients underwent endoscopic suspension of bladder neck for treatment of stress urinary incontinence at the King Hussein Medical Center and Hamad General Hospital, Amman, Jordan. We reviewed the first 25 urethral lengthening and reimplantation procedures done at our institution in 24 patients. All patients had failed to achieve dryness on medical or surgical therapy for total urinary incontinence. A total of 32 patients had neurogenic incontinence. The formation of a bowel reservoir of large capacity at low pressure by using small and large bowel (ileocaecal segment) has proved reliable for achieving continent urinary diversion (n = 80), for bladder augmentation (n = 42) as well as for total bladder replacement (n = 24). Encouraged by the results we obtained in our adult patients, we have used this technique during the last 3.5 years in 29 children. A retrospective study of cases seen between 1975 and 1985 showed 24 tumors of upper urinary tract secondary to primary bladder localizations. These secondary localizations developed during evolution of multi-resected recurring bladder tumors, within a mean period of 15 months after discovery of a secondary vesicorenal reflux. This reflux is acquired constantly during the course of these tumors and it is undoubtedly responsible for the onset of these upper tract tumoral grafts. The most frequent cause of contrast media extravasation from the renal pelvis is a nephrourolithiasis. There are frequent causes enumerated in the literature, that are also characterized by a mechanical obstruction of the contrast media excretion. The presented case reports are characterized by a technical dependency, namely a long angiographic investigation and an overfilling of the urinary bladder without any indication of an organical excretory impediment. The clinical consequence of such observations should be discussed. 150 ml contrast media should not be exceeded during one examination.
