[Examination of the risk factors for infectious urolithiasis formation in patients receiving urinary diversion or urinary reconstruction using intestine--comparison of reservoir and ileal conduit].

Urinary risk factors and preventive factors for infectious calcium (Ca) urolithiasis formation, such as citrate, oxalate, Ca, phosphate, magnesium, and uric acid were examined more than 2 months after performing a urinary reservoir in 12 patients (R group) and ileal conduit in 10 patients (C group). There was no difference in the positive rate of risk factors between the two groups and also no difference in the positive rate of more than two risk factors between the two groups. Although four patients with more than two risk factors in the R group suffered from reservoir stones all of which inc

Management of intravesical self-inflicted sharp objects in children: 10-year single-center experience.

INTRODUCTION: Intravesical foreign bodies (FBs) are rare and have interesting pathology for urologists. There has been an increase in reports of intravesical FBs in the last few decades, but they are still considered to be rare in children, especially young girls. Here we present our experience in the assessment and management of intravesical self-inserted sharp objects in children. PATIENTS AND METHODS: We reviewed the records of children with self-introduced intravesical FBs admitted to our hospital during the last 10 years. Twenty-four cases were included in this study (20 girls and 4 boys

Do we really need kidneys-ureters-bladder radiography to predict stone radiopacity before treatment with shockwave lithotripsy? Development and internal validation of a novel predictive model based on computed tomography parameters.

PURPOSE: To produce and validate a predictive model based on CT parameters for calculating the probability of a stone to be visible on fluoroscopy of shockwave lithotripsy (SWL) and to compare its accuracy to that of kidneys-ureters-bladder (KUB) radiography. METHODS: We retrospectively analyzed 306 patients (sample group) who underwent an SWL between March 2011 and August 2012. A multivariate analysis of several parameters extracted from the preoperative CT scan was conducted to identify independent predictors for radiopacity on SWL fluoroscopy. The results were used for the creation of a pr

Persistent ureteric stump syndrome post ureteric re-implantation following impacted distal ureterolithiasis: Experience from tertiary hospital. Case report.

INTRODUCTION AND IMPORTANCE: Ureteric stump syndrome [USS] is a series of febrile recurrent lower abdominal pain, urinary tract infections, and hematuria that sometimes present with empyema as a rare complication. The ureteric stump is left after ureteric re-implantation due to an impacted stone at the Vesical-ureteric junction [VUJ], or after nephrectomy of a non-functional kidney due to a distal stone; the ureteral stump forms a source of infection to the urinary bladder, in addition to long-term obstructive stones left at the ureteric stamp. This usually cause chronic irritation of the muco

From contraception to calculus: copper-T migration resulting in vesical calculus formation.

A woman in her 40s presented with persistent lower urinary tract symptoms (LUTS) over 6 months, including suprapubic pain and dysuria. Initial investigations were inconclusive until non-contrast CT revealed a large bladder stone encapsulating an intrauterine contraceptive device (Copper-T). She underwent successful perurethral cystolithotripsy using Holmium:YAG laser, resulting in complete stone and device removal. This case underlines the importance of suspecting migrated intrauterine devices in unexplained LUTS and highlights minimally invasive endourological techniques as effective manageme

[A clinical study of bladder stone with spinal cord injury in subacute stage].

Bladder stones that form in patients with spinal cord injury in the subacute stage can cause significant morbidity. In this retrospective study, we investigated the factors associated with bladder stone formation. The study population consisted of 141 men and 24 women (median age: 59 years) who were admitted to our hospital for rehabilitation, from January 2005 to December 2006. We investigated urinary pH, bacterial species in urine and duration of indwelling urethral catheter in the acute stage after spinal cord injury. Thirty-one patients (16.3%) were diagnosed with bladder stones and underw

Acute relief or future prevention: is urology ready for preventive health care?

The approach to clinical medicine has evolved over the last 20 years to incorporate therapeutic strategies to prevent long-term negative outcomes rather than simply treat acute events. As a result, new treatment paradigms have been developed in various disease areas. These paradigms arise from clinical trials that show a correlation between risk reduction and decreases in painful, traumatic, or fatal events. The field of urology has been relatively slow to adopt the concept of disease prevention. Several areas of clinical urology do employ prophylactic or metaphylactic therapies, although thes

The agreement among urological experts on the diagnostic management of patients with common urological problems.

OBJECTIVE: To assess the level of agreement among randomly selected international urologists on the diagnostic management of patients with prostate cancer, bladder cancer, urinary stones or lower urinary tract symptoms (LUTS) arising from benign prostatic hyperplasia (BPH). METHODS: A computer program was used to provide an unbiased format of 53 simulated patients, comprising 13 with prostate cancer, 10 with bladder cancer, 10 with stones in the upper urinary tract and 20 with LUTS from BPH. For each case, the history was provided to the user while information from 60 diagnostic tests could b

Acute urinary retention: causes and treatment.

Acute urinary retention often is secondary to obstruction of the bladder or distal genitourinary system, which may be induced by any of a number of medical or surgical conditions. Treatment varies according to the cause.

Percutaneous suprapubic stone extraction for posterior urethral stones in children: efficacy and safety.

OBJECTIVE: To evaluate the safety and efficacy of percutaneous suprapubic stone extraction (PSPSE) for pediatric posterior urethral stones. METHODS: Between July 2007 and June 2010, 54 boys presenting with acute urinary retention due to posterior urethral stones underwent PSPSE. Patients were a mean age of 66.4 months (range, 8-180 months). The stone size was 0.7-1.9 cm. Patients were placed under general anesthesia, and a 7F urethroscope was used to pushback the stone to the bladder. A 3-mm suprapubic puncture with a scalpel was performed, followed by insertion of a straight narrow hemostat

[When is it meaningful to investigate hematuria? Macroscopic hematuria--investigate always. Microscopic hematuria--symptoms and age decide].

All patients (n = 578) referred during one year and for whom hematuria was mentioned in the referral form were monitored following urological evaluation including urography and cystoscopy. Evaluation of macroscopic hematuria was often associated with significant findings at both urography (stones) and cystoscopy (bladder tumors). The situation was the same even if not as pronounced for evaluation of microscopic hematuria with concomitant urinary tract symptoms. The evaluation of asymptomatic microscopic hematuria was, however, very rarely associated with significant findings, which were moreov
