Uric acid stones and hyperuricosuria.

Recent work has highlighted the strong relationships among obesity, diabetes, and the metabolic syndrome as causes of low urinary pH. Low urinary pH in turn is the major urinary risk factor for uric acid stones. Unlike calcium stones, uric acid stones can be dissolved and easily prevented with adequate urinary alkalinization. Recognizing the relevant risk factors should lead to increased identification of these radiolucent stones. The cornerstone of therapy is raising urinary pH; xanthine dehydrogenase inhibitors should be used only when urinary alkalinization cannot be achieved.

[Bone and Men's Health. Microscopic hematuria and bone mineral density].

Microscopic hematuria is frequently seen during routine health check-ups. A recent epidemiological study has shown that the number of red blood cells in urine is inversely correlated with bone mineral density (BMD) in men and women over 50 years of age. This paper discusses mechanisms explaining the relationship between microscopic hematuria and BMD. Patients with urolithiasis have lower BMD than normal people, suggesting that calculi may cause both microscopic hematuria and decreased BMD. On the other hand, elderly individuals with albuminuria have a higher incidence of non-vertebral fracture…

The diagnostic performance of machine learning based detection of urinary tract stones: a systematic review and meta-analysis.

BACKGROUND: Urolithiasis is a prevalent urological condition, and Non-Contrast Computed Tomography (NCCT) is the gold standard for diagnosis. In recent years, there has been growing interest in investigating machine learning (ML)- based detection of urolithiasis and the wider potential of AI in urology. PURPOSE: To synthesise the diagnostic accuracy of ML-based UTS detection on NCCT and in externally validated cohorts. MATERIALS AND METHODS: We performed a systematic review and bivariate meta-analysis of studies evaluating ML for detecting urinary stones. We used QUADAS-2 to assess the risk …

Renal atrophy and acute contralateral obstructive uropathy secondary to 2,8-dihydroxyadenine nephropathy: A case report based discussion on diagnostic pitfalls, FTIR spectroscopy, and literature review.

APRT deficiency leads to 2,8-DHA nephropathy. It manifests as recurrent radiolucent calculi and progressive renal failure. These stones are misidentified as uric acid stones. We report a 30-year-old male with recurrent left-sided nephrolithiasis, resulting in a poorly functioning left kidney. He subsequently presented with acute obstructive uropathy in his hypertrophied right kidney. FTIR post-ureteroscopy identified the composition as 2,8-DHA. This case underscores the necessity of precise stone analysis in recurrent urolithiasis. Increased awareness and knowledge of the limitations of some m…

A huge ureteric stone in a functionally, metabolically and anatomically normal kidney: an alliance of rarity.

Urinary stones are a common health problem, necessitating frequent outpatient visits and hospital admissions. Ureteric stones demand special attention, given their propensity for upstream hydroureteronephrosis and loss of renal function. They are usually predisposed by diverse anatomical, functional or metabolic abnormalities of the urinary tract and have an early symptomatic presentation. We report an exceptional clinical presentation of a huge ureteric stone yet functional renal moiety with no obvious anatomical or metabolic predilection for urolithiasis, and its minimally invasive managemen…

Acute life-threatening hyperkalaemia in a patient with giant hydronephrosis: a case report.

A 52-year-old man with a history of urolithiasis presents to the emergency department with a sudden, sharp, continuous right flank colicky pain. Laboratory workup demonstrates acute kidney injury with a mild hyperkalaemia. During the observation period, the patient develops an atypical broad complex sinus bradycardia and eventually short asystolic periods. This was caused by a severe therapy-resistant hyperkalaemia, wherefore emergency haemodialysis was necessary. Radiographic results showed a giant hydronephrosis with a blowout of the right kidney and an obstructing calculi of 21 mm in the di…

First case report of staghorn calculi successfully removed by mini-endoscopic combined intrarenal surgery in a 2-year-old boy.

Less-invasive therapy for pediatric urolithiasis is available due to the miniaturization of equipment and improved optics; however, surgical treatment strategies, especially for large calculi, remain controversial. We describe here our experience of treating a 2-year-old boy with left renal staghorn calculi with a single session of mini-endoscopic combined intrarenal surgery in the prone split-leg position with pre-ureteral stenting and the directional enhanced flow imaging ultrasound technique. This is the first report of successful pediatric mini-endoscopic combined intrarenal surgery withou…

Hypercalcemia of malignancy associated with renal cell carcinoma in a dog.

A 10 yr old castrated male Siberian husky was evaluated for polyuria, polydipsia, a retroperitoneal mass, and urolithiasis. A marked elevation in Ca was noted on initial blood work, and results of additional testing were consistent with hypercalcemia of malignancy, including an elevated parathyroid hormone-related peptide (PTHrp) value. Based on clinical signs, blood work, diagnostic imaging, and cytology results, unilateral renal neoplasia was suspected. Following a complete right nephrectomy and cystotomy, histopathologic examination confirmed a diagnosis of renal cell carcinoma (RCC). Five …
