In an attempt to determine the most appropriate procedure for surgical treatment of concomitant ulcers, gastric secretion and pathological findings were studied. Acidification of the gastric cardia has been shown to increase lower esophageal sphincter pressure (LESP). The mechanism by which this phenomenon occurs remains unknown. This study was undertaken to examine the effect and mechanism of action of proximal gastric acidification on LESP in the dog model. We report a rare case of a 52 year old patient with multiple gastric wall abscesses. In 1987 a selective proximal vagotomy was performed. Because of non-specific gastric symptoms in 1993 an endoscopy and an endoscopic ultrasound was performed at another hospital which resulted in the likely diagnosis of multiple leiomyomas. Selective upper abdominal sympathectomy increased basal acid output in rats but was without effect on stimulated acid output, serum gastrin concentration, and gastric mucosal histidine decarboxylase activity. The sympathectomy was verified by fluorescence histochemistry and determination of tissue catecholamines. Thirty-eight patients undergoing reoperation for recurrent peptic ulceration have been reviewed after at least 3 years. All were considered to be in Visick grade IV before reoperation. There were no operative deaths. Thirty-three of the patients ended with both a vagotomy and antral resection. After reoperation 17 patients (45 per cent) remained in Visick grades III and IV. The incidence of duodenogastric reflux (DGR) was studied in 241 of 376 patients operated upon for duodenal ulcers.
