Cutaneous leukocytoclastic vasculitis in dermatomyositis suggests malignancy.

BACKGROUND: Dermatomyositis (DM) is a rare connective tissue disease which has been shown to be associated with an underlying malignancy. O … 22%) cases. The skin biopsies of all patients showed features of DM; in 7 cases, a leukocytoclastic vasculitis was detected. Four of the 5 cases with an associated malignancy demonstrated histologically a vasculitis in lesional skin, compared to only 3 out of 18 cases without malignancy (p < 0.05, Fisher's exact test). CONCLUSION: Our data suggest that vasc …

IgA antineutrophil cytoplasmic antibodies in cutaneous vasculitis.

BACKGROUND: Antineutrophil cytoplasmic antibodies (ANCA) of the IgA isotype have, for the most part, been detected in patients with Henoch-Schönlein purpura (HSP) or inflammatory bowel disease. OBJECTIVES: We have evaluated the prevalence of IgA ANCA in a series of patients with different causes of cutaneous vasculitis. METHODS: Forty consecutive patients with histologically proven leucocytoclastic vasculitis were included in the study: 18 had systemic vasculitis as well as cutaneous lesions, 10 of whom were diagnosed as having HSP, and 22 had only cutaneous vasculitis (with no identified ca…

Hypocomplementemic urticarial vasculitis or systemic lupus erythematosus?

The 2 patients presented here showed the typical signs of hypocomplementemic urticarial vasculitis syndrome (HUVS). During follow-up, there was an inverse correlation between anti-C1q autoantibody titer and C1q antigen concentration in serum in both patients over a period of 2 years. The first patient had nephritis characterized by immune deposits in glomeruli and around the tubules. The histological findings, C1q deposits, and presence of tubuloreticular inclusions in capillary endothelial cells suggested a disease process identical to systemic lupus erythematosus (SLE). The second patient, a…

Livedo vasculitis associated with regional cytophagic histiocytoses.

We report a 70-year-old woman who developed recurrent multiple ulcers and residual atrophic scars on her legs and feet. The histology showed fibrinoid material in the dermal vessel walls causing partial to complete occlusion of the lumina. Capillary tufting and dilatation were also noted in the dermis. There was no vasculitis or panniculitis. Extravasated red blood cells were seen throughout the dermis; these were engulfed by benign-appearing histiocytes around the dermal-fat junction. As she was generally well and there was no evidence of the presence of cytophagic cells in her extracutaneous…

[Vasculitis during human acquired immunodeficiency virus infection].

Vasculitis is among the dysimmunity-mediated disorders seen in patients infected with the human immunodeficiency virus (HIV). Several cases have been reported. The central and peripheral nervous systems were the main targets, although many other organs may be involved. There were wide variations in clinical presentation, clinico-pathologic patterns, and the stage of the HIV infection at vasculitis onset. Causes included drugs and infections, particularly some opportunistic infections. In some cases, the vasculitis seemed to be directly due to the HIV. The etiopathogenesis remains unclear and t…

[Leprous neuropathy--observation from the standpoint vasculitis].

Pathogenesis of leprous neuropathy was studied from the standpoint of vasculitis. In vasculitic syndrome, mononeuritis multiplex is the most specific peripheral nerve injury. It occurs most frequently in small vessel vasculitis. Among them, the incidence is highest in microscopic polyangitis (MPO), Churg-Strauss Syndrome (CSS), and Wegener's granulomatosis (WG). All of them are categorized as ANCA associated disease. Because the disease activity is parallel with ANCA titer and ANCA is related directory to the pathogenesis of necrotizing vasculitis as well as the occurrence of peripheral neurop…

[Macrolide antibiotic-induced vasculitis (Churg-Strauss syndrome)].

BACKGROUND: Macrolides are known to have relatively few side effects and are prescribed in cases of allergic reaction to penicillin. The new macrolides, for example Azithromycin and Roxithromycin, are increasingly preferred over erythromycin at the ear, nose, and throat out-patient department due to improved oral reabsorption (acid resistance), better penetration into tissue, prolonged half-life, extended antibacterial activity, modest side effects, and better pharmacokinetics. There are only few case reports concerning side effects of macrolides. We report on the appearance of a Churg Strauss

Rheumatoid vasculitis manifesting as intra-abdominal hemorrhage.

Rheumatoid vasculitis, an extra-articular component of rheumatoid arthritis, causes a wide spectrum of manifestations that range from clinically insignificant to life-threatening disease. As a systemic necrotizing arteritis, rheumatoid vasculitis is usually characterized by end-organ ischemia. Herein we describe a patient with abdominal pain and syncope due to intra-abdominal hemorrhage from a ruptured aneurysm of the inferior pancreaticoduodenal artery in the setting of rheumatoid vasculitis. Although the intra-abdominal hemorrhage was the unusual manifestation of rheumatoid vasculitis in thi…

[CNS vasculitis after ophthalmic herpes zoster infection].

We present a case of cerebral vasculitis secondary to infection with Varicella Zoster Virus which appeared in a 27 year old patient after a latency period of eight weeks. We emphasize the usefulness of angioresonance as a fitting means of evolutive follow up without the need to carry out control cerebral angiography. We likewise point out the usefulness of acyclovir as the sole treatment necessary in non-complicated cases.

Vasculitis, Kawasaki disease, and hemolytic uremic syndrome.

Aside from Henoch-Schönlein purpura and Kawasaki disease, vasculitis in childhood is rare. The etiology, clinical characteristics, and therapy of Kawasaki disease continue to be important areas of investigation, and a number of important observations recently were published. Recent clinical descriptions of other vasculitis disorders such as cutaneous polyarteritis and Wegener's granulomatosis are also reviewed in this article. Hemolytic uremic syndrome is an acute disorder of childhood characterized by thrombocytopenia, microangiopathic hemolytic anemia, and acute nephropathy. Treatment of chi…

Anti-neutrophil cytoplasmic antibody-associated paraneoplastic vasculitis.

A 68 year old man presented with a systemic necrotizing vasculitis and elevated levels of anti-neutrophil cytoplasmic antibody (ANCA) which responded to treatment with steroids and cyclophosphamide, with a decrease in the titre of ANCA until its disappearance. Four months later he presented with weakness, loss of weight, aphonia and dysphagia. A computerized tomography scan showed a solid mass in the anterior mediastinum, and histological studies revealed an undifferentiated adenocarcinoma. Vasculitis improved although the malignancy progressed and ANCA was persistently negative. Our case demo…

[Vasculitis syndrome--clinical classification and diagnostic approaches].

Vasculitis syndrome comprises a broad spectrum of diseases associated with vasculitis, which is characterized by inflammation and damage to blood vessels. Clinical features associated with vasculitis usually appear depending upon size of damaged vessels, but rarely correlate with primary disease itself. Vasculitis must be considered in the differential diagnosis for therapeutic approaches, particularly in patients with serious vasculitis occurring irreversible organ damage due to rapidly progressive systemic vasculitis, such as Wegener's granulomatosis, polyarteritis nodosa. These patients req…

Essential cryofibrinogenaemia, leukocytoclastic vasculitis and chronic purpura.

Cryofibrinogenaemia refers to the presence of cold-precipitable proteins in plasma but not in serum. It is usually associated with malignancy, thromboembolic diseases or various inflammatory processes; rarely it may be essential. The most common clinical presentations of cryofibrinogenaemia are cold-intolerance, purpura, skin necrosis and ulcers. We describe a middle-aged woman with essential cryofibrinogenaemia, leukocytoclastic vasculitis, and chronic purpura for over 25 years with several exacerbations. In patients with otherwise unexplained purpura or skin necrosis, determination of plasma…

Myelodysplasia, vasculitis and anti-neutrophil cytoplasm antibodies.

A cutaneous or systemic vasculitis occurs in myelodysplasia as well as in myeloproliferative and lymphoproliferative disorders. The most common lesion is a leucocytoclastic vasculitis, with neurological or joint involvement occurring less often. The vasculitis may appear contemporaneously with or precede the clinical onset of the blood dyscrasia. Occasionally the lesions respond dramatically to the use of steroids but in general, patients with vasculitis have a worse prognosis than those with uncomplicated myelodysplasia. Vasculitis and myelodysplasia appear together too often for the associat…

Cerebral vasculitis in acute post-streptococcal glomerulonephritis.

Neurological complications in acute post-streptococcal glomerulonephritis (APSGN) have been traditionally attributed to hypertensive encephalopathy. A 9-year-old girl with biopsy-documented APSGN developed seizures at a time she was normotensive and biochemically well balanced. Computed tomography of her brain was consistent with vasculitis, a finding which was also clinically supported by apparent vasculitic involvement of other organ systems. All clinical, laboratory and radiological abnormalities resolved with recovery from the APSGN. We suggest that on rare occasions neurological complicat…
