The human postural coordination mechanism is an example of a complex closed-loop control system based on multisensory integration [9,10,13,14]. In models of this process, sensory data from vestibular, visual, tactile and proprioceptive systems are integrated as linearly additive inputs that drive multiple sensory-motor loops to provide effective coordination of body movement, posture and alignment [5-8, 10, 11]. In the absence of normal vestibular (such as from a toxic drug reaction) and other inputs, unstable posture occurs. This instability may be the result of noise in a functionally open-l

A unilateral labyrinthectomy was performed on anesthetized adult albino rats. Brain [14C]2-deoxyglucose (2DG) uptake was measured autoradiographically 3.5 h to 20 days later and compared to sham-operated controls. In the vestibular nuclei (nn.) of labyrinthectomized subjects, large left-right differences of 2DG uptake occurred, which decreased over time. The equalization of vestibular nuclear 2DG uptake paralleled behavioral compensation of body, neck and head postural abnormalities, and known equalization of vestibular nuclear cell firing rates during compensation. There was a small differenc

The depression of the postural activity induced by intravenous injection of eserine sulphate (0.1 mg/kg), an anticholinesterase, has been studied in precollicular decerebrate cats. The extensor and flexor monosynaptic reflexes elicited by single shock stimulation of the GS, P1-FDHL and DP nerves are tonically depressed during the episodes of postural atonia induced by the anticholinesterase. A further phasic depression of the monosynaptic reflexes occurs during the bursts of rapid eye movements (REM) typical of these episodes. These changes in spinal reflex activity closely resemble the tonic

PURPOSE OF REVIEW: This article describes an approach to the diagnosis and management of acute constant dizziness, one of the most unnerving presentations in medicine. Patients with acute constant dizziness can be completely debilitated by the symptoms. Most cases are caused by a self-limited disorder, typically vestibular neuritis. However, a significant proportion of cases harbor a stroke that could be life threatening. Discriminating a self-limited disorder from a life-threatening disorder can be challenging and often hinges on findings, which may be subtle, from the ocular motor examinatio

Secondary somatoform dizziness and vertigo (SVD) is an underdiagnosed and handicapping psychosomatic disorder, leading to extensive utilization of health care and maladaptive coping. Few long-term follow-up studies have focused on the assessment of risk factors and little is known about protective factors. The aim of this 1-year follow-up study was to identify neurootological patients at risk for the development of secondary SVD with respect to individual psychopathological disposition, subjective well-being and resilient coping. In a prospective interdisciplinary study, we assessed mental dis

The initial development of the cranial nerves was studied in 245 human embryos of stages 10-23 (4-8 postfertilizational weeks). Significant findings in the human embryo include the following. (1) Neuronal migration is a characteristic feature in the development of all the cranial nerves at stages 13-18, with the exception of the somatic efferent group. (2) The somatic efferent and the visceral efferent neurons are arranged respectively in ventrolateral and ventromedial columns (stages 13-17). (3) The ventrolateral column gives rise to somatic efferent nuclei; the neurons of the hypoglossal ner

Patients with neurological gait disorders often present to their doctor with the key symptoms of dizziness and gait unsteadiness (e.g. cerebellar ataxia, progressive supranuclear palsy). In vestibular syndromes, on the other hand, the gait disturbance is a leading sign and many aspects of the syndrome can be recognized from the analysis of posture and gait (e.g. direction of falls). For therapy in particular it is important to better understand the physiological control of posture and gait to adapt rehabilitation programs. We recently succeeded in visualizing the hierarchic network for postura

Future research in the neurosciences can best be understood in the context of NASA's life sciences goals in the near term (1990-95), mid term (1995-2000), and long term (2000 and beyond). Since NASA is planning short-duration Spacelab and International Microgravity Laboratory (IML) flights for many years to come, the acute effects of exposure to microgravity will continue to be of experimental and operational interest in the near term. To this end, major new areas of research will be devoted to ground-based studies of preflight adaptation trainers and their efficacy in preventing or reducing t

OBJECTIVE: Hearing loss is a common symptom in patients with cochleovestibular schwannoma. Clinical and histologic observations have suggested that the hearing loss may be caused by both retrocochlear and cochlear mechanisms. Our goal was to perform a detailed assessment of cochlear pathology in patients with vestibular schwannoma (VS).
STUDY DESIGN: Retrospective analysis of temporal bone histopathology.
SETTING: Multi-center study.
MATERIAL: Temporal bones from 32 patients with unilateral, sporadic VS within the internal auditory canal.
MAIN OUTCOME MEASURES: Sections through the cochlea

Progress has been made in the diagnosis and treatment of inner ear disorders. Autoimmune inner ear disorders and Ménière's disease (MD), the prototype inner ear disease, are highlighted in this review of current knowledge and contemporary dietary, medical, surgical, and vestibular rehabilitation therapy. A number of other peripheral vestibular disorders are presented and contrasted with MD.

BACKGROUND: Chronic unilateral vestibulopathy (CUVP) is one of the most common causes of chronic dizziness/vertigo. The brain functional mechanisms of CUVP are currently unclear. The study aimed to clarify changes in brain topological properties and subnetwork functional connectivity in CUVP patients, elucidating the neural mechanisms behind their poor dynamic compensation.
METHODS: A total of 44 participants were included (22 CUVP patients and 22 age- and sex-matched healthy controls). Resting-state functional MRI was performed on all subjects. Network-Based Statistics (NBS) analysis was con

Vestibular neuritis (VN) is characterised by acute vertigo due to a sudden loss of unilateral vestibular function. A considerable proportion of VN patients proceed to develop chronic symptoms of dizziness, including visually induced dizziness, specifically during head turns. Here we investigated whether the development of such poor clinical outcomes following VN, is associated with abnormal visuo-vestibular cortical processing. Accordingly, we applied functional magnetic resonance imaging to assess brain responses of chronic VN patients and compared these to controls during both congruent (co-

BACKGROUND: We investigated how response asymmetries and deficit side response amplitudes for head accelerations used clinically to test the vestibular ocular reflex (VOR) are correlated with caloric canal paresis (CP) values.
METHODS: 30 patients were examined at onset of an acute unilateral peripheral vestibular deficit (aUPVD) and 3, 6, and 13 weeks later with three different VOR tests: caloric, rotating chair (ROT), and video head impulse tests (vHIT). Response changes over time were fitted with an exponential decay model and compared with using linear regression analysis.
RESULTS: Recov

The purpose of this study was to evaluate the usefulness of the neutrophil to lymphocyte ratio (NLR) in a differential diagnosis and follow-up of patients with peripheral vertigo. Twenty patients with benign positional paroxysmal vertigo (BPPV) and 20 patients diagnosed with vestibular neuritis (VN) were included in the study. Serum samples were analysed at the initial presentation and on the seventh day of admission retrospectively. The WBC (white blood cell) count was 10500±2100 /mm3, the neutrophil count was 4700±1100/mm3, the lymphocyte count was 5000±1200/mm3 and the NLR was 0.9±0.2 in th

Synaptic activity was recorded with sharp microelectrodes during rest and during 0.3-Hz sinusoidal stimulation from bouton afferents identified by their efferent-mediated inhibitory responses. A glutamate antagonist, 6-cyano-7-nitroquinoxaline-2,3-dione (CNQX) decreased quantal size (qsize) while lowering external Ca(2+) decreased quantal rate (qrate). Miniature excitatory postsynaptic potentials (mEPSPs) had effective durations (qdur) of 3.5-5 ms. Their timing was consistent with Poisson statistics. Mean qsizes ranged in different units from 0.25 to 0.73 mV and mean qrates from 200 to 1,500/s
