Surgical approaches for resection of vestibular schwannomas: translabyrinthine, retrosigmoid, and middle fossa approaches.

Surgical removal remains one of the key treatment modalities for vestibular schwannomas. A team approach between a neurotologist and a neurosurgeon offers the patient the expertise of both specialties and maximizes the chances for an optimal outcome. Vestibular schwannomas can typically be resected through 1 of 3 main surgical approaches: the translabyrinthine, the retrosigmoid, or the middle fossa approaches. In this report and videos, the authors describe and illustrate the indications and surgical techniques for the removal of these tumors.

Are evolutionary hypotheses for motion sickness "just-so" stories?

Vertebrates have evolved rapidly conditionable nausea and vomiting reflexes mediated by gut and brainstem receptors, clearly as a defense against neurotoxin ingestion. In 1977 Treisman proposed that sensory orientation linkages to emetic centers evolved for the same reason, and that motion sickness was an accidental byproduct. It was an "adaptationist" explanation for motion sickness, since it assumed that evolution has shaped all phenotypic traits for survival advantage. Treisman's "poison" theory is plausible, and frequently cited as the accepted scientific explanation for motion sickness. H

Transmastoid labyrinthectomy for disabling vertigo in a patient with internal auditory canal pathology.

OBJECTIVE: Document the use of transmastoid labyrinthectomy to treat disabling vertigo secondary to a lesion in the internal auditory canal. PATIENT: A 69-year-old man with nonserviceable left hearing experienced disabling attacks of vertigo refractory to medical measures. Magnetic resonance imaging revealed a small left intracanalicular lesion with an irregular configuration and modest enhancement, suggesting either an unusual acoustic neuroma or a cavernoma. Tumor size remained stable on serial imaging, and the patient declined microsurgical resection, stereotactic radiation, or intratympan

[Clinical anatomy of the temporal bone: taxonomy, topography and important variations].

Detailed knowledge of the complicated anatomy and topography of the temporal bone is a crucial basis for successful ear surgery. In particular, the topographical relations to the essential neighbouring structures (e.g. intratemporal vessels and nerves, neighbouring spaces, especially the endocranium) are of vital importance. Furthermore, typical landmarks play an essential role in topographic orientation. However, this knowledge cannot be acquired from lectures or books, but can only be achieved by independently performing numerous dissections and drill exercises on the temporal bone, once a t

Change of extracellular ascorbic acid in the brain cortex following ice water vestibular stimulation: an on-line electrochemical detection coupled with in vivo microdialysis sampling.

BACKGROUND: Ascorbic acid (AA) represents one of the most important enzyme co-factors, antioxidants and neuromodulators and plays an important role in the cerebral system. Increasing evidence has suggested that AA could treat certain kinds of vertigo diseases such as Meniere's disease. To elucidate the neurochemical functions associated with AA in vertigo, the change of extracellular AA in the brain cortex following caloric vestibular stimulation (CVS) was evaluated. METHODS: An on-line electrochemical detection was coupled with in vivo microdialysis to continuously monitor the change of extr

Expression of cochlin in the vestibular organ of rats.

The COCH gene mutated in autosomal dominant sensorineural deafness (DFNA9) encodes cochlin, a major constituent of the inner ear extracellular matrix. Cochlin constitutes 70% of the inner ear protein and cochlin isoforms can be classified into three subgroups, p63s, p44s and p40s. Symptoms of some DFNA9 patients are consistent with those of Ménière's disease. Here, we report the expression of cochlin in the vestibular organ of rats using isoform-specific antibodies that recognize all three isoforms. Cochlin is highly expressed in the stromata of the maculae of otolithic organs and cristae of s

Desensitization to strong vestibular stimuli improves tolerance to simulated aircraft motion.

INTRODUCTION: The extent to which tolerance acquired in one motion environment can be transferred to another has not been fully elucidated. This study compares three provocative motion stimuli and whether tolerance acquired from cross-coupled motion and torso rotation can be transferred to simulated aircraft motion. METHOD: There were 18 healthy subjects (9 men and 9 women), 18-44 yr of age, who were divided into 3 groups. Each group was exposed to the following desensitization procedure in a pseudo-random repeated measures design. In condition D1, subjects were exposed to 4 consecutive days

"Conflicting" motion cues to the visual and vestibular self-motion systems around 0.06 Hz evoke simulator sickness.

The basic question this research addressed was, how does simulator sickness vary with simulated motion frequency? Participants were 11 women and 19 men, 20 to 63 years of age. A visual self-motion frequency response curve was determined using a Chattecx posture platform with a VR4 head-mounted display (HMD) or a back-projected dome. That curve and one for vestibular self-motion specify a frequency range in which vestibular and visual motion stimuli could produce conflicting self-motion cues. Using a rotating chair and the HMD, a third experiment supported (p < .01) the hypothesis that conflict

Adaptive changes in the angular VOR: duration of gain changes and lack of effect of nodulo-uvulectomy.

Alterations in the gain of the vertical angular vestibulo-ocular reflex (VOR) are dependent on the head position in which the gain changes were produced. We determined how long gravity-dependent gain changes last in monkeys after four hours of adaptation, and whether the adaptation is mediated through the nodulus and uvula of the vestibulocerebellum. Vertical VOR gains were adaptively modified by rotation about an interaural axis, in phase or out of phase with the visual surround. Vertical VOR gains were modified with the animals in one of three orientations: upright, left-side down, or right-

Vestibular-evoked postural responses in the absence of somatosensory information.

In order to investigate the ways in which sensory channels interact to control balance, we measured the postural response evoked by galvanic vestibular stimulation (GVS) in a rare subject (I. W.) with a large-fibre sensory neuronopathy. I. W. has no sensations of cutaneous light touch and movement/position sense below the neck, and without vision he has no knowledge of where his limbs and body are in space. He was tested with and without vision while seated. With eyes closed, I. W.'s responses to pure vestibular stimuli were an order of magnitude larger than those of healthy controls. In other

Vestibular influences on autonomic cardiovascular control in humans.

There is substantial evidence that anatomical connections exist between vestibular and autonomic nuclei. Animal studies have shown functional interactions between the vestibular and autonomic systems. The nature of these interactions, however, is complex and has not been fully defined. Vestibular stimulation has been consistently found to reduce blood pressure in animals. Given the potential interaction between vestibular and autonomic pathways this finding could be explained by a reduction in sympathetic activity. However, rather than sympathetic inhibition, vestibular stimulation has consist

Coriolis effects and motion sickness modelling.

Coriolis effects are notorious in relation to disorientation and motion sickness in aircrew. A review is provided of experimental data on these Coriolis effects, including the modulatory effects of adding visual or somatosensory rotatory motion information. A vector analysis of the consequences of head movements during somatosensory, visual and/or vestibular rotatory motion stimulation revealed that the more the sensed angular velocity vector after the head movements is aligned with the gravitoinertial force vector, the less nauseating effects are experienced. It is demonstrated that this is a

Vestibular influences on cat lumbar paravertebral muscles.

Recordings were made from nerves innervating ventral lumbar paravertebral muscles (quadratus lumborum) during natural vestibular stimulation in vertical planes. The best direction of vestibular stimulation for producing an increase in nerve activity was near nose-up pitch in two-thirds of the animals, but was near nose-down pitch in the others. The response gain (re. position) was flat across the frequency range of 0.05-1 Hz, and the responses persisted throughout 11-s static tilts. These data suggest that otolith organs activated by pitch influence activity in the lumbar paravertebral muscles

Pressor response elicited by nose-up vestibular stimulation in cats.

The purpose of the present study was to determine whether selective activation of vestibular receptors produces changes in blood pressure. Blood pressure was recorded during trapezoidal head rotations in cats with extensive denervations to eliminate nonlabyrinthine inputs that could be produced by the movements. Large (50 degrees) nose-up trapezoidal head tilts produced an increase in blood pressure of approximately 18 mmHg; ear-down tilt produced little change in blood pressure. The changes in blood pressure began approximately 1.4 s after the plateau of the stimulus. The responses to nose-up

Otoneuroscience and space vestibulo-neuroscience in the new century.

From a clinically oriented point of view, efforts of otoneuroscience should be directed toward an improvement of the management of dizzy and vertiginous patients. At present, the main obstacles in this field are: (1) the lack of specific therapies for many of the clinical diagnoses. This reduces the necessity of clinical vestibular diagnosis to a pragmatic exclusion of the few diseases with therapeutic consequences; (2) the lack of feedback to confirm or disprove clinical diagnoses; (3) poor knowledge of the pathophysiology of many of the otoneurological diseases; experiments in animal models
