Missed Addisonian crisis in surgical wards.

Three women were admitted to the surgical wards with acute gastro-intestinal symptoms and despite good clinical (pigmentation, hypotension) and biochemical (hyponatraemia, acidosis) evidence of Addisonian crisis had unnecessary investigations with delay in the diagnosis and appropriate management of this medical emergency. Clinicians should take careful note of the typical electrolyte upset seen in Addisonian crisis.

Congenital tuberculosis and associated hypoadrenocorticism.

Two cases of probable congenital tuberculosis occurring in the ten-year period from 1966 to 1976 are reported. One infant with confirmed congenital tuberculosis also had symptomatic hypoadrenocorticism. Tuberculosis should be considered in the differential diagnosis of the seriously ill infant.

NALP1 in vitiligo-associated multiple autoimmune disease.

BACKGROUND: Autoimmune and autoinflammatory diseases involve interactions between genetic risk factors and environmental triggers. We searched for a gene on chromosome 17p13 that contributes to a group of epidemiologically associated autoimmune and autoinflammatory diseases. The group includes various combinations of generalized vitiligo, autoimmune thyroid disease, latent autoimmune diabetes in adults, rheumatoid arthritis, psoriasis, pernicious anemia, systemic lupus erythematosus, and Addison's disease. METHODS: We tested 177 single-nucleotide polymorphisms (SNPs) spanning the 17p13 linkag

The pivotal role of etiology in managing Non-CAH primary adrenal insufficiency in pediatric and adolescent populations: A Single-Center study.

BACKGROUND: Primary adrenal insufficiency (PAI) in children, outside of congenital adrenal hyperplasia (CAH), represents a rare and genetically diverse group of disorders often leading to diagnostic delays. Advances in molecular genetics have uncovered a wide spectrum of non-CAH etiologies, yet real-world data from pediatric cohorts remain limited. METHODS: We retrospectively reviewed 11 children diagnosed with non-CAH PAI at a single tertiary care center between March 2015 and December 2024. Clinical, biochemical, and genetic findings were evaluated using targeted Sanger sequencing or next-ge

Rate of fracture in patients with glucocorticoid replacement therapy: a systematic review and meta-analysis.

PURPOSE: The association between glucocorticoid replacement therapy for adrenal insufficiency (AI) and osteoporosis is unclear. Fracture is a major cause of morbidity in patients with osteoporosis. This study aims to determine if patients on glucocorticoid replacement therapy for AI have an increased rate of fractures compared to the general population. METHODS: We included all studies with adult patients receiving glucocorticoid replacement therapy for either congenital adrenal hyperplasia (CAH), primary adrenal insufficiency (PAI), or secondary adrenal insufficiency (SAI). Studies without f

News about the genetics of congenital primary adrenal insufficiency.

Primary adrenal insufficiency (PAI) is characterized by impaired production of steroid hormones due to an adrenal cortex defect. This condition incurs a risk of acute insufficiency which may be life-threatening. Today, 80% of pediatric forms of PAI have a genetic origin but 5% have no clear genetic support. Recently discovered mutations in genes relating to oxidative stress have opened the way to research on genes unrelated to the adrenal gland. Identification of causal mutations in a gene responsible for PAI allows genetic counseling, guidance of follow-up and prevention of complications. Thi

Pericarditis with cardiac tamponade and addisonian crisis as the presenting features of autoimmune polyglandular syndrome type II: a case series.

OBJECTIVE: To present 3 cases of cardiac tamponade and addisonian crisis as the presenting features of autoimmune polyglandular syndrome (APS) type II. METHODS: We present the clinical features, physical examination findings, radiographic findings, and laboratory investigations in 3 patients and provide an overview of the published cases of pericarditis and APS type II found in a MEDLINE search. RESULTS: Patient 1, a 34-year-old woman, and Patient 3, a 35-year-old woman, had known autoimmune thyroid disease. Patient 2 was a 58-year-old man with no known history of autoimmune disease. All pre

Ovarian adrenal rest tissue in congenital adrenal hyperplasia--a patient report.

We report a young girl who died in an Addisonian crisis due to previously undiagnosed congenital adrenal hyperplasia (CAH), in whom ovarian adrenal rest tissue was detected at postmortem histopathological examination. This is a very rare complication in female patients with CAH with only two previously reported cases.

Mineralocorticoids in the management of primary adrenocortical insufficiency.

Plasma renin activity (PRA) and plasma volume (PV) were determined in 22 adult patients treated for Addison's disease (AD) and reporting at the clinic for follow-up. Mean PRA was thrice the upper limit of normal (9.1 +/- 7.1 ng/ml/h (SD)) and mean PV was decreased (87% +/- 11 (SD)), consistent with residual hypovolemia in most patients, despite conventional treatment with both fluorocortisol (FF) and cortisone acetate. There was an inverse relationship between PRA and PV. Both PRA and PV were significantly correlated with FF dosage. On the other hand, no correlation was found between PV and ei

[Extended ACTH rapid test for differentiation of primary and secondary adrenal insufficiency].

In 30 resting normal persons, 5 ambulant normal persons and 3 patients with disorders of the pituitary-adrenal-system before and 30 minutes after intravenous injection of 0,25 mg synthetic adrenocorticotrophin (tetracosactid, Synacthen) plasma cortisol and aldosterone levels were evaluated. The evaluation of the corticoids was continued over 240 minutes in intervals of 30 minutes. The basal cortisol and aldosterone levels of the resting normal persons and ambulant persons ordinarily doubled 30 minutes after ACTH application. The plasma cortisol level of a steroid-treated patient with lupus ery
