Air-Evacuation-Relevant Hypobaria Following Traumatic Brain Injury Plus Hemorrhagic Shock in Rats Increases Mortality and Injury to the Gut, Lungs, and Kidneys.

Rats exposed to hypobaria equivalent to what occurs during aeromedical evacuation within a few days after isolated traumatic brain injury ex … multi-organ inflammation. These findings support epidemiological studies indicating that increasing aircraft cabin pressures to 4,000 ft altitude (compared with standard 8,000 ft) and limiting excessive oxygen administration will decrease critical complications during and following aeromedical transport.

Summertime Clean-Background Ozone Concentrations Derived from Ozone Precursor Relationships are Lower than Previous Estimates in the Southeast United States.

Background ozone in this study is defined as the amount of ozone that is not affected by the emissions of ozone precursors in the region of … ith high isoprene emission ozone, such as the Southeast United States. We make use of the extensive aircraft and surface observations in the Southeast in the summer of 2013. Compared to the diagnostic results using the relationship of O3-NOz (total reactive nitrogen excluding nitrogen oxides), zero-emission (model-only), and 5th percentile methods, the new …

In-Flight Emergency: A Simulation Case for Emergency Medicine Residents.

INTRODUCTION: In-flight medical emergencies are common occurrences that require medical professionals to manage patients in an unfamiliar setting with limited resources. Emergency medicine (EM) residents should be well prepared to care for patients in unusual environments such as on an aircraft. METHODS: We developed a simulation case for EM residents featuring a 55-year-old male passenger who suffers a cardiac arrest secondary to a tension pneumothorax. We conducted this case eight times during a 5-hour block of scheduled simulation time. Participants included EM residents of all training le…

Strength Analysis of a Rib-Stiffened GLARE-Based Thin-Walled Structure.

This paper presents a new product, a glass laminate aluminium-reinforced epoxy (GLARE)-based thin-walled structure with a stiffener in the f … g. The FMLs were tested at three different temperatures that corresponded to those found under real aircraft operating conditions, i.e. -60 °C, room temperature and +80 °C. It was found that the temperatures do not affect the tensile strength and shear strength of the FMLs tested. However, there was a noticeable increase in the stiffness of samples stretch …

Pneumocephalus Associated with Inappropriate Valsalva Technique.

BACKGROUND: Pneumocephalus is intracranial air normally occurring as a consequence of trauma or cranial surgery, but rarely has been reported following the habitual use of the Valsalva maneuver. We were unable to locate any similar cases of pneumocephalus associated with high performance aircraft flight, and this case report highlights an important consequence of the inappropriate use of Valsalva on ascent to altitude.CASE REPORT: A healthy 29-yr-old F-22 instructor pilot presented with recurrent pneumocephalus following the use of the Valsalva maneuver. These episodes started after an initial…

Air travel and incidence of pneumothorax in lymphangioleiomyomatosis.

BACKGROUND: Pulmonary lymphangioleiomyomatosis (LAM) is a rare disease of women characterized by multiple lung cysts leading to respiratory insufficiency and frequent pneumothorax (PT). Air travel (AT) could increase the risk of PT in LAM through rupture of subpleural cysts induced by atmospheric pressure changes in aircraft cabin. To determine whether AT increases the risk of PT in LAM, we performed a retrospective survey of members of European LAM patient associations. A flight-related PT was defined as occurring ≤30 days after AT. RESULTS: 145 women reported 207 PT. In 128 patients with av…

Diurnal variability of transportation noise exposure and cardiovascular mortality: A nationwide cohort study from Switzerland.

BACKGROUND: Most epidemiological noise studies consider 24 h average noise exposure levels. Our aim was to exploratively analyze the impact of noise exposure at different time windows during day and night on cardiovascular mortality. METHODS: We generated Switzerland-wide exposure models for road traffic, railway and aircraft noise for different time windows for the year 2001. Combined noise source equivalent continuous sound levels (Leq) for different time windows at the most exposed façade were assigned to each of the 4.41 million Swiss National Cohort adult participants. Follow-up period w…

Effects of Scavenging, Entrainment, and Aqueous Chemistry on Peroxides and Formaldehyde in Deep Convective Outflow Over the Central and Southeast United States.

Deep convective transport of gaseous precursors to ozone (O3) and aerosols to the upper troposphere is affected by liquid phase and mixed-phase scavenging, entrainment of free tropospheric air and aqueous chemistry. The contributions of these processes are examined using aircraft measurements obtained in storm inflow and outflow during the 2012 Deep Convective Clouds and Chemistry (DC3) experiment combined with high-resolution (dx ≤ 3 km) WRF-Chem simulations of a severe storm, an air mass storm, and a mesoscale convective system (MCS). The simulation results for the MCS suggest that formaldeh…

Inattentional deafness to auditory alarms: Inter-individual differences, electrophysiological signature and single trial classification.

Inattentional deafness can have deleterious consequences in complex real-life situations (e.g. healthcare, aviation) leading to miss critical auditory signals. Such failure of auditory attention is thought to rely on top-down biasing mechanisms at the central executive level. A complementary approach to account for this phenomenon is to consider the existence of visual dominance over hearing that could be implemented via direct visual-to-auditory pathways. To investigate this phenomenon, thirteen aircraft pilots, equipped with a 32-channel EEG system, faced a low and high workload scenarii alo…

A Novel Treatment of Fear of Flying Using a Large Virtual Reality System.

BACKGROUND: Fear of flying (FoF), a common phobia in the developed world, is usually treated with cognitive behavioral therapy, most efficiently when combined with exposure methods, e.g., virtual reality exposure therapy (VRET). We evaluated FoF treatment using VRET in a large motion-based VR system. The treated subjects were seated on a moving platform. The virtual scenery included the interior of an aircraft and a window view to the outside world accompanied by platform movements simulating, e.g., takeoff, landing, and air turbulence. Relevant auditory stimuli were also incorporated. CASE R…

Mission, physical, and war stressors' impact on aircrew psychological strain.

BACKGROUND: Little is known about the relative impact of the organization of missions on aircrew well-being. Using an occupational stress model we investigate a previously little studied concept of mission stressors and determine its relative impact in comparison to physical and war stressors in the prediction of four strains in deployed aircrews. METHOD: Questionnaires were completed by 272 deployed in-aircraft crewmembers. Three new stressors were developed for this study: mission stressors, physical stressors, and war stressors. In addition, four strains were measured: PTSD, depression, sl…

A case of active tuberculosis in a cabin crew: the results of contact tracing.

INTRODUCTION: Transmission of communicable diseases on board aircraft is of considerable concern for passengers and aircrew. Previously published estimates of risk of tuberculosis (TB) transmission have been highly variable. Furthermore, very few studies have been published for active TB in aircrew. METHODS: The public health authorities advised the Medical Advisor of an airline that a cabin crewmember had been diagnosed with active TB. Contact tracing was done for the cabin crew who worked with the index case for more than 8 h. Cabin crewmembers were divided in two groups according to their …

Ground vasovagal presyncopes and fighter pilot fitness: aeromedical concerns.

BACKGROUND: Vasovagal episodes are common events but may have consequences for flight safety, particularly in high-performance aircrafts, where the autonomic nervous system is impacted during +G(z) accelerations. However, the risk is difficult to assess in the case of ground presyncopes. CASE REPORT: A 27-yr-old fighter pilot experienced a feeling of faintness at a daily briefing. He had previously shown no medical history except for a single episode. The initial physical examination was normal. During vagal maneuvers, the pilot developed a cardiac pause and a tilt-table test was positive. Ot…

Neurologic decompression sickness following cabin pressure fluctuations at high altitude.

Decompression sickness (DCS) occurs in diving, altitude chamber exposures, and unpressurized or depressurized high-altitude flights. Because DCS takes many forms, in-flight cases may be misinterpreted as hypoxia, hyperventilation, or viral illness, with resulting failure to respond appropriately. In this case, a 28-yr-old male pilot of a single-seat, tactical aircraft experienced 12 rapid pressure fluctuations while flying at 43,000 ft above sea level. He had no symptoms and decided to complete the flight, which required an additional 2 h in the air. Approximately 1 h later he began to experie…

Risk of herniated nucleus pulposus among U.S. astronauts.

INTRODUCTION: Astronauts have complained of back pain occurring during spaceflight, presumably due to the elongation of the spine from the l … ver, there was no clear increase of HNP incidence in those astronauts who were high performance jet aircraft pilots. There was evidence suggesting that the risk is increased immediately after spaceflight. CONCLUSIONS: Astronauts are at higher risk of incurring HNP, especially immediately following spaceflight.

Determination of inlet transmission and conversion efficiencies for in situ measurements of the nocturnal nitrogen oxides, NO3, N2O5 and NO2, via pulsed cavity ring-down spectroscopy.

Pulsed cavity ring-down spectroscopy is a highly sensitive method for direct absorption spectroscopy that has been applied to in situ detection of NO3, N2O5 and NO2 in the atmosphere from a variety of platforms, including ships, aircraft, and towers. In this paper, we report the development of schemes to significantly improve the accuracy of these measurements. This includes the following: (1) an overall improvement in the inlet transmission efficiencies (92 +/- 2% for NO3 and 97 +/- 1% for N2O5) achieved primarily through a reduction in the inlet residence time; and (2) the development of a c…

Recovery from unusual attitudes: HUD vs. back-up display in a static F/A-18 simulator.

OBJECTIVE: Spatial disorientation (SD) remains one of the most important causes of fatal fighter aircraft accidents. The aim of this study was to give a recommendation for the use of the head-up display (HUD) or back-up attitude directional indicator (ADI) in a state of spatial disorientation based on the respective performance in an unusual attitude recovery task. METHODS: Seven fighter pilots joining a conversion course to the F/A-18 participated in this study. Flight time will be presented as range (and mean in parentheses). Total military flight experience of the subjects was 835-1759 h (…
