OBJECTIVES: Anaphylaxis is a sudden onset multiorgan allergic reaction that infrequently but regularly causes fatalities which may be preventable with appropriate organ support. There is limited data about the type of organ failure leading to death or near-fatal episodes resulting in permanent neurological disability. To assist clinicians facing anaphylaxis in diverse clinical settings, we aimed to quantify the frequency of organ failure type contributing to death or neurological disability from anaphylaxis according to allergen trigger.
DESIGN: Systematic review of published peer-reviewed li…

This narrative review aimed to explore mental health issues among children with food-induced anaphylaxis (FIA) and their parents and to identify possible risk factors. A review of the scientific literature from 2005 to 2025 (MEDLINE, PubMed, PsycINFO) was done, and 31 papers were selected. Results were presented according to DSM-V diagnostic categories. An additional paragraph describes other relevant psychological problems related to the mental health of children with FIA and their parents. Results showed that a number of young patients (44%-54%) may experience anxiety, depression, stress dis…

Hydatid disease, caused by Echinococcus granulosus, commonly affects the liver. Traumatic cyst rupture is rare, and when complicated by anaphylaxis, it poses a life-threatening emergency. A 33-year-old male patient presented with severe abdominal and chest pain after falling from a height. Imaging revealed multiple hepatic hydatid cysts, one of which ruptured. Shortly after the CT scan, the patient developed hypotension, erythema, pruritus, and throat tightness consistent with anaphylaxis. He was treated with epinephrine, corticosteroids, antihistamines, oxygen, and intravenous fluids, stabili…

Alligator mississippiensis meat is an uncommon, exotic dish that has increasingly become easily accessible across the United States. This presents a unique issue for patients with food allergies, as the cross-reactivity of reptile meat with fish and poultry allergy is not well-recognized. While there has been an established connection between those with fish allergy also reacting to reptile meat, it is even less known that poultry allergy may also have cross-reactivity with reptile meat. We present a case of a 25-year-old male patient who is able to consume fish but is allergic to chicken and

OBJECTIVE: To describe the clinical presentation, management, and outcome of a dog with a peritoneal-pericardial diaphragmatic hernia and cardiac tamponade thought to be secondary to anaphylaxis.
CASE SUMMARY: A 10-year-old spayed female Weimaraner presented for an episode of acute collapse. At the time of presentation, the patient was bright, ambulatory (although limping on the left thoracic limb), and had normal perfusion parameters. A stinger was removed from the left thoracic paw pad, and the patient was treated with diphenhydramine. Thoracic and abdominal point-of-care ultrasound identif…

Patients in outpatient dialysis settings, including InCenter and Home Therapies, have the potential to experience hypersensitivity reactions from multiple sources, such as dialyzer membranes, other products unique to dialysis, and new medications. Awareness of co-factors, frequently present in the end stage kidney disease population, is crucial in managing reactions effectively. Dialysis staff must act quickly to identify anaphylaxis. This article provides clinicians with the critical information required to prevent and mitigate life-threatening emergencies, including symptom identification an…

Bananas contain different allergens, and reactions vary in severity. Thaumatin-like proteins (TLPs) present high stability during digestion and heat treatment. Data regarding reaction severity and acquisition of tolerance are lacking. A two-year-old child presented with anaphylaxis after eating a banana. He was treated with corticosteroids and antihistamines on the way to the emergency department (ED) by his mother. Adrenaline was not administered as he had already improved upon arriving at the ED. The child had tolerated all fruits introduced into his diet, including bananas. He had never eat…

BACKGROUND: Perioperative anaphylaxis is rare but is associated with significant morbidity. This complication has been well described in France by the GERAP (Groupe d'Etude des Réactions Anaphylactiques Périopératoires), a network focused on its study. The epidemiology of perioperative anaphylaxis is evolving, influenced by environmental factors and clinical practice. The aim of this study was to update the epidemiology of perioperative anaphylaxis in France.
METHODS: This multicentre retrospective study was performed in 26 allergy clinics of the GERAP network in 2017-8.
RESULTS: There were …

Idiopathic anaphylaxis (IA) is a severe allergic reaction without identifiable external triggers, presenting significant challenges in diagnosis and management. However, growing evidence suggests that many cases classified as idiopathic may actually be driven by cofactors such as exercise, hormonal fluctuations, medications, or hidden allergens. This mini-review explores the evolving understanding of IA, highlighting the role of these cofactors in triggering or amplifying anaphylactic reactions. It emphasizes how advances in diagnostic tools, including component-resolved diagnostics, are helpi…

A 14-year-old girl presented to our hospital with food-dependent exercise-induced anaphylaxis (FDEIA), possibly caused by rice. Despite experiencing four previous episodes of suspected FDEIA, she did not seek medical attention at her own discretion. On the fifth occurrence of symptoms, the general practitioner suspected FDEIA and referred the patient to our hospital. The only common factor in all five episodes was the consumption of rice, leading to the examination of the patient under suspicion of FDEIA caused by rice. Skin prick test results were positive for bran and polished rice, and exer…

PURPOSE OF REVIEW: This review aims to identify phenotypes at-risk of Hymenoptera venom-induced anaphylaxis (HVA), focusing on different perspectives (epidemiological, clinical, and therapeutic) in order to adapt future preventive strategies.
RECENT FINDINGS: HVA remains one of the leading causes of anaphylaxis, with a broad pattern of symptoms. Although most cases occur outside healthcare settings, data indicate a high emergency admission rate due to insect stings. Mortality is often underestimated because of the lack of witnesses and difficulties in recognizing the signs and the culprit. Ta…

A 13-year-old female with a history of congenital left lower leg lymphedema, multiple food allergies, including an immunoglobulin E mediated severe cow's milk allergy, and well-controlled moderate persistent asthma was hospitalized with left lower leg erysipelas and Group A Streptococcus septicemia. While hospitalized, immediately after exposure to cow's milk protein as an inactive ingredient within a probiotic, she developed anaphylaxis with respiratory failure requiring intubation. This is only the third reported case of anaphylaxis due to a probiotic. Additionally, it raises issues inherent…

BACKGROUND: Multiple anaphylaxis mediators have been demonstrated to provoke migraine-like attacks following intravascular infusion, suggesting that anaphylaxis could cause headache; however, headache is rarely noted in anaphylaxis and is not included among diagnostic criteria.
OBJECTIVE: Our study objectives were to estimate the frequency of headache among prospectively enrolled emergency department (ED) patients with anaphylaxis and assess the association of headache with patient and anaphylaxis characteristics.
METHODS: A cross-sectional study of ED patients aged ≥5 years who met anaphyla…

Anaphylaxis is an acute, potentially life-threatening severe allergic reaction commonly caused by foods, insect stings, and medications. Intramuscular epinephrine is the cornerstone of treatment for anaphylaxis in order to reverse immediate symptoms and prevent progression to life-threatening hemodynamic or respiratory collapse. By nature of its mechanism of action, epinephrine may induce a number of neurovascular-related adverse effects; even at usual therapeutic doses. Rarely described adverse events include transient ischemic attacks, ischemic stroke, intracerebral hemorrhage, and myocardia…

Anaphylaxis is an acute, potentially life-threatening systemic allergic reaction for which there are no known reliable preventative therapies. Its primary cell mediator, the mast cell, has several pathophysiologic roles and functions in IgE-mediated reactions that continue to be poorly understood. Recent advances in the understanding of allergic mechanisms have identified novel targets for inhibiting mast cell function and activation. The prevention of anaphylaxis is within reach with new drugs that could modulate immune tolerance, mast cell proliferation and differentiation, and IgE regulatio…
