Simultaneous two-level minimally invasive lumbar laminectomy performed with dual tubular retractor systems in a 93-year-old under spinal anesthesia: illustrative case.

BACKGROUND: Lumbar spinal stenosis is a common degenerative condition in elderly patients that often requires surgical intervention. Minimally invasive techniques combined with spinal anesthesia (SA) can reduce patient complications and improve outcomes. Here the authors present the case of an elderly patient who underwent minimally invasive surgery, using dual tubular retractors and SA to address severe lumbar stenosis. OBSERVATIONS: A 93-year-old female presented with severe back pain radiating bilaterally to her lower extremities. MRI demonstrated severe central canal stenosis at L4-5 and

Application of ultrasound-guided inferior vena cava collapsibility measurement in volume assessment for patients undergoing single-shot spinal anesthesia in total hip arthroplasty.

To explore the value of ultrasound in volume assessment during the perioperative period of single-shot spinal anesthesia for total hip arthroplasty. A total of 100 patients undergoing elective surgery under spinal anesthesia at our hospital from January 2022 to January 2024 were selected. Transthoracic echocardiography was used to measure the diameter of the abdominal aorta (Ao) before anesthesia (T1), 10 minutes after anesthesia (T2), and 30 minutes after anesthesia (T3). The inferior vena cava collapsibility index (IVC-CI) and the ratio of IVCe to Ao (IVCe/Ao) were calculated. A volume load

Atypical Clinical Presentation of Hypothermia Induced by Intrathecal Morphine in a Patient Who Underwent Total Knee Arthroplasty Under Spinal Anesthesia: A Case Report.

This case report describes a 62-year-old male who experienced paradoxical hypothermia following an elective total knee replacement (TKR) performed under spinal anesthesia with intrathecal morphine administration. The patient presented with significant hypothermia, defined as a body temperature below 35°C, approximately 45 minutes following the administration of anesthesia. The condition demonstrated resistance to conventional rewarming techniques. This condition required symptomatic treatment, leading to the administration of naloxone. After receiving two doses of naloxone, the patient's tempe

Postoperative Delirium after Dexmedetomidine versus Propofol Sedation in Healthy Older Adults Undergoing Orthopedic Lower Limb Surgery with Spinal Anesthesia: A Randomized Controlled Trial.

BACKGROUND: Delirium is a critical postoperative complication in older patients. Based on the hypothesis that intraoperative dexmedetomidine sedation would lower postoperative delirium than propofol sedation would, the authors compared the incidence of postoperative delirium in older adults, using the mentioned sedatives. METHODS: This double-blinded, randomized controlled study included 748 patients, aged 65 yr or older, who were scheduled for elective lower extremity orthopedic surgery, between June 2017 and October 2021. Patients were randomized equally into two groups in a 1:1 ratio accor

Comparative study on the manually-controlled variable-rate versus fixed-rate infusion of norepinephrine for preventing hypotension during spinal anesthesia for cesarean delivery.

STUDY OBJECTIVE: Previous studies have shown that prophylactic norepinephrine infusion is superior to intermittent bolus administration in preventing post-spinal hypotension. Nevertheless, it is still controversial whether manually-controlled variable-rate infusion is more effective than fixed-rate infusion. The purpose of the present study was to compare the efficacy of variable-rate infusion and fixed-rate infusion of norepinephrine for prophylaxis against maternal hypotension and maintaining hemodynamic stability during spinal anesthesia for cesarean delivery to determine more effective mod

Comparison of Hemodynamic Response following Spinal Anesthesia between Controlled Hypertensive and Normotensive Patients Undergoing Surgery below the Umbilicus: An Observational Prospective Cohort Study.

BACKGROUND: Hypotension and bradycardia are the most common complications associated with spinal anesthesia and more common in patients with a history of hypertension. Regular use of antihypertensive medications can prevent these complications. The occurrence of hypotension under spinal anesthesia among controlled hypertensive and normotensive patients with age 40 years and above is still debated. The objective of the study was to compare blood pressure and heart rate changes following spinal anesthesia between controlled hypertensive and normotensive patients undergoing surgery below the umbi

Lidocaine vs. Other Local Anesthetics in the Development of Transient Neurologic Symptoms (TNS) Following Spinal Anesthesia: A Meta-Analysis of Randomized Controlled Trials.

The use of lidocaine in spinal anesthesia may increase the risk of transient neurological symptoms (TNS) according to previous meta-analyses. However, the previous meta-analyses lacked data on some other local anesthetics and thus, more evaluations are still needed to compare the effect of lidocaine on the development of TNS. The objective of this study was to compare the risk of TNS according to lidocaine versus other local anesthetics in patients undergoing spinal anesthesia. A total of 39 randomized controlled trials with 4733 patients were analyzed. The incidence of TNS was 10.8% in the li

Periarticular Morphine-Induced Sphincter of Oddi Spasm Causing Severe Pain and Bradycardia in an Awake Patient Under Spinal Anesthesia: An Important Diagnostic Consideration.

Sphincter of Oddi spasm from opioids has been documented, presenting as severe epigastric pain and potentially overlooked in a differential diagnosis. We present a case of sphincter of Oddi spasm from periarticular morphine in a patient under spinal anesthesia, causing severe distress and treated effectively with glucagon. It is important for anesthesiologists using opioids to consider it as a cause of perioperative pain and be familiar with treatment as it may be refractory by conventional use of opioids for pain relief. It is also important to consider the systemic effects of periarticular a

A double-blind, placebo-controlled trial of four fixed rate infusion regimens of phenylephrine for hemodynamic support during spinal anesthesia for cesarean delivery.

BACKGROUND: The administration of prophylactic phenylephrine infusions in combination with fluid cohydration significantly reduces the incidence of hypotension in women having cesarean delivery under spinal anesthesia. The ideal dosing regimen for this purpose is not known. In this study, we investigated the dose of phenylephrine that, when administered as a prophylactic fixed rate infusion, is associated with the least interventions needed to maintain maternal systolic blood pressure (SBP) within 20% of baseline. METHODS: Women undergoing elective cesarean delivery were randomly allocated to

Comparison of Fractionated Dose Versus Bolus Dose of Hyperbaric Bupivacaine Injection in Spinal Anesthesia: Hemodynamic Stability in Elderly Patients Undergoing Lower Limb Surgeries.

BACKGROUND: Spinal anesthesia (SA) with a bolus dose provides a rapid onset but often leads to hemodynamic instability, particularly in elderly patients. Fractionated dosing of hyperbaric bupivacaine may offer a dense block with greater stability and prolonged analgesia. This study compares the effects of bolus and fractionated dosing on hemodynamic parameters in elderly patients undergoing lower limb surgeries. METHODS: In this randomized controlled trial, 60 elderly patients (n=60 patients; 30 patients per group) of more than 60 years were randomly assigned to receive either a bolus dose (G

Remimazolam used for sedation during spinal anesthesia puncture provides faster sedation effect and increase recovery rate compared to midazolam: a randomized controlled trial.

BACKGROUND: Surgical time-out is essential for patient safety, requiring patients to remain conscious to verify their identity and surgical site. In China's high-volume operating rooms, this step is crucial to prevent errors. Spinal anesthesia often induces anxiety, making sedation necessary. An ideal sedative must provide effective anxiolysis while allowing rapid recovery. This study compares remimazolam and midazolam to meet these dual demands and assess their cardiorespiratory safety. METHODS: This prospective, randomized controlled trial compared remimazolam and midazolam for sedation in
