Maintenance antimicrobials in high risk urologic pediatric patients.

As an alternative to the practice of obtaining repeated laboratory cultures for patients at high risk of renal impairment from recurrent UTI, a program of bone monitoring using a mail-in culture dipspoon was started. A study involving 454 children with neurogenic bladder or other urologic abnormalities showed (1) that the incidence of UTI infection in patients for whom long term antimicrobial therapy had been prescribed was not significantly lower than that in patients who were not on antimicrobials and (2) that at least 50% of dipspoons inoculated due to presentation of UTI symptoms showed no

Cistus x incanus L. extract as a complex polyphenolic blend with retained anti-adhesive and anti-inflammatory properties in a model of E. coli-induced UTI following simulated digestion.

ETHNOPHARMACOLOGICAL RELEVANCE: Cistus x incanus L. is a Mediterranean plant traditionally used to treat infective conditions, including urinary tract infections (UTI). However, experimental validation against uropathogenic Escherichia coli (UPEC) is limited. AIM OF THE STUDY: To investigate the antibacterial and anti-inflammatory effects of a hydroalcoholic extract of aerial parts (CE) in an E. coli-induced UTI model, evaluating stability and efficacy after intestinal digestion. MATERIALS AND METHODS: T24 bladder epithelial cells were infected with UPEC CFT073 and treated with CE before and

Long-term antibiotic treatment of chronic bacterial prostatitis. Effect on bacterial flora.

The bacterial flora in patients referred with chronic bacterial prostatitis were studied. Only 13% had Gram-negative bacteria in significant numbers but 43% had Gram-positives using the same criteria. Half of the patients were symptom-free by the end of a 12-week course of antibiotics and remained so after 6 months; in one-third the symptoms were unchanged 6 months after completing treatment. The relief of symptoms correlated with the disappearance of white blood cells in the expressed prostatic secretion (EPS) and with a lowered pH in the EPS. Thus only 1 of the 14 patients without symptoms a

Urinary tract infections in the female patient.

Recent investigations into the pathogenesis of urinary tract infections have led to significant understanding of the interaction between uropathogens and the urogenital tract. This understanding provides the potential for the development of new types of therapy. A practical approach to the management of lower urinary tract infections in female patients is discussed, as are new therapies for nonbacterial cystitis, including the urgency-frequency syndrome.

[Renal colic].

INTRODUCTION: Renal colic accounts for 5-10% of all emergency department visits, making it a common condition in acute medicine. The typical clinical presentation is an early indication of urolithiasis. DIAGNOSIS: Diagnostic measures include laboratory tests, ultrasound, and low-dose noncontrast computed tomography (CT) scans. Kidney, ureter, bladder (KUB) plain film radiography has been widely replaced by low-dose noncontrast CT with similar radiation dosage. In special patient groups such as children or pregnant women, ionizing radiation should be avoided if possible. TREATMENT: General me

The use of suction drains at burch colposuspension and postoperative infectious morbidity.

To evaluate whether the use of suction drains in the paravescical spaces is associated with the occurrence of postoperative infectious complications. Patients who underwent to a Burch colposuspension to treat urinary stress incontinence were included. Thirty-seven patients had the placement of suction drains in the paravescical spaces while 25 patients did not. Postoperative infectious morbidity was defined as the occurrence of at least one of the following events: febrile morbidity, sepsis, pelvic infections, severe wound infection and infected subfascial haematoma. Febrile morbidity was defi

A bacterial interference strategy for prevention of UTI in persons practicing intermittent catheterization.

STUDY DESIGN: Non-randomized pilot trial. OBJECTIVES: To determine whether Escherichia coli 83972-coated urinary catheters in persons with spinal cord injury (SCI) practicing an intermittent catheterization program (ICP) could (1) achieve bladder colonization with this benign organism and (2) decrease the rate of symptomatic urinary tract infection (UTI). SETTING: Outpatient SCI clinic in a Veterans Affairs hospital (USA). METHODS: Participants had neurogenic bladders secondary to SCI, were practicing ICP, had experienced at least one UTI and had documented bacteruria within the past year.

Effect on uropathogens of prophylaxis for urinary tract infection in spinal cord injured patients: preliminary study.

Spinal cord injured patients are highly prone to urinary tract infections. The high frequency of recurrences, the problems with drug resistance and the difficulties associated with diagnosis complicate the management. In a preliminary retrospective study of 30 patient files, we discovered that prophylactic antimicrobial therapy with trimethoprim-sulfamethoxazole, significantly reduced the incidence of symptomatic urinary tract infections. The prevention of infection resulted in cheaper healthcare expenses than treatment. ONe problematic outcome was that antibiotic therapy resulted in a dramati

Single-dose versus 3-day cotrimoxazole prophylaxis in transurethral resection or greenlight laser vaporisation of the prostate: study protocol for a multicentre randomised placebo controlled non-inferiority trial (CITrUS trial).

BACKGROUND: Transurethral resection of the prostate (TURP) and Greenlight laser vaporisation (GL) of the prostate are frequently performed urological procedures. For TURP, a single-dose antimicrobial prophylaxis (AP) is recommended to reduce postoperative urinary tract infections. So far, no international recommendations for AP have been established for GL. In a survey-based study in Switzerland, Germany and Austria, urologists reported routinely extending AP primarily for 3 days after both interventions. We therefore aim to determine whether single-dose AP with cotrimoxazole is non-inferior t

Clinical predictors to guide additional evaluation after a first febrile urinary tract infection in children.

PURPOSE: Additional evaluation after pediatric febrile urinary tract infection (UTI) is required to identify children at risk of long-term renal complications. Because these complications are difficult to directly assess in clinical practice, we sought to identify clinical predictors that may guide additional evaluation, using subsequent intervention as a pragmatic indicator of clinically significant urological concern following a first febrile UTI. METHODS: We retrospectively reviewed 2,155 children aged < 10 years with a first febrile UTI. Clinical factors at diagnosis were analyzed to iden

Diagnostic evaluation and treatment of UTIs in children with neurogenic bladder.

OBJECTIVE: Children with neurogenic bladder (NGB) are at increased risk for urinary tract infections (UTIs), but there is a lack of guidelines to assist clinicians in diagnosing and treating these children. Our objective was to describe the presentation and treatment of provider diagnosed UTIs in children with NGB compared to children with vesicoureteral reflux (VUR) and to assess the proportion of children with NGB who met a consortium definition of UTI. STUDY DESIGN: We included children <18 years old with either VUR or NGB who were diagnosed in the emergency department with a febrile UTI i

Emergence of recurrent urinary tract infection: Dissecting the mechanism of antimicrobial resistance, host-pathogen interaction, and hormonal imbalance.

Urinary tract infection is one of the most common infections worldwide, causing numerous deaths every year. The gut-bladder axis has been recently found to be a key factor in initiating UTI pathogenesis, along with the imbalance in the gut microbiome, which is associated with advanced susceptibility to rUTI. The patients who suffer from UTIs are, more often than not, the ones who have the lowest levels of butyrate-producing gut bacteria. Antibiotics cause dysbiosis in the gut and increase the growth of uropathogenic strains. Moreover, the gut-vagina and vagina-bladder axes are involved in UTIs

[Purple urine bag syndrome (PUBS): about two cases at the Regional University Hospital Center (CHUR) of Ouahigouya].

Purple urine bag syndrome (PUBS) is an uncommon complication of urinary tract infection. It mainly affects elderly patients with constipation, chronic urinary catheter and prolonged bed rest. Several bacteria belonging to two different groups, the group converting indoxyl urinary sulphate into indoxyl and the group alkalizing urine by the production of urease, are associated with it. We here report two cases of young patients aged 30 and 16 years, living in Burkina Faso, with uremic encephalopathy associated with severe infection and cloudy urine with purple urine bag. Unlike cases reported in

Controversies in Treating Asymptomatic Bacteriuria and Urinary Tract Infection: A Case Based Review of Antibiotic Use in Renal Transplant Patients and its Impact on the Development of Resistance.

There is excessive use of both broad spectrum and niched antibiotics for urinary tract infections (UTIs) in hospital and ambulatory setting in spite of clear guidelines on appropriate use. Majority of antibiotics prescribed in United States for UTIs are for nonspecific indications such as positive urine cultures in the absence of symptoms, etc. For these conditions especially asymptomatic bacteriuria, a large proportion of the antibiotics prescribed are unlikely to provide clinical benefit to patients. Asymptomatic bacteriuria (ASB) is a common finding in healthy women and persons with underly

Diagnosis, prevention and treatment of urinary tract infections in older people.

Urinary tract infections (UTIs) are common in older people, with the prevalence increasing with age in both sexes. UTI is a frequent reason for emergency admission to hospital. There are many conditions that contribute to older people being more at risk of UTI and the main preventive strategy is to avoid the use of indwelling urethral catheters. Where an indwelling catheter is inserted its continued use should be regularly reviewed and the catheter removed, especially if the reason for insertion is incontinence and the person becomes additionally incontinent of faeces. Diagnosis of UTI can be

[Comparative studies on activities of antimicrobial agents against causative organisms isolated from patients with urinary tract infections (2002). II. Background of patients].

A total of 615 bacterial strains isolated from 491 patients diagnosed as having urinary tract infections (UTIs) in 13 institutions in Japan were supplied between August 2002 and July 2003. Then, the clinical background of patients were investigated such as sex, age, and type of infections and kind of bacteria, frequency of bacteria isolation by age and infections, bacteria and infections by timing of antibiotics administration, and bacteria and infections by surgical procedures. The bacterial strains were divided by the age and sex of the patients and the types of infections. In males, the num
