A Mendelian Randomization Analysis Investigates Causal Associations between Inflammatory Bowel Diseases and Variable Risk Factors.

Источник пометил публикацию как отозванную

The question of whether variable risk factors and various nutrients are causally related to inflammatory bowel diseases (IBDs) has remained … conducted to determine causal risk factors for IBD diseases. Genetic predisposition to smoking and appendectomy as well as vegetable and fruit intake, breastfeeding, n-3 PUFAs, n-6 PUFAs, vitamin D, total cholesterol, whole-body fat mass, and physical activity were related to the risk of UC (p < 0.05). The effect of lifestyle behaviors on UC was attenuated af …

[Appendicular plastron: emergency or deferred surgery: a series of 27 cases collected in the surgical clinic of the Aristide Le Dantec Hospital].

Our study aimed to evaluate the outcome of the surgical treatment of appendicular plastron after deferred or emergency appendectomy. We conducted a retrospective, descriptive study of 27 patients treated for appendicular plastron from January 2000 to 31 December 2007. Diagnosis was based on clinical examination showing a mass in the right iliac fossa, on ultrasound or made intraoperatively. All patients undergoing emergency surgery were classified in Group I while those undergoing deferred surgery were classified in Group II. 18 men and 9 women were registered, with a sex-ratio man /Woman=2. Twice perforated stump appendicitis: a case report.

Stump appendicitis is an acute inflammation of the residual appendix and one of the rare complications after appendectomy. Stump appendicitis is an under-reported and poorly defined condition related to obstruction and inflammation of the residual appendix after an appendectomy, usually by a fecolith. It remains a clinical challenge because of delayed diagnosis and subsequent treatment with increased morbidity or mortality. Herein, we describe the case of a 42-year-old male who presented with periumbilical pain with progression to generalized abdominal pain and signs of peritonitis 14 months p…

[Ileus after appendicitis in children].

A relatively frequent and alarming complication of appendicitis in children is ileus. The authors submit an analysis of 41 cases of ileus with a time interval of 3 days to 8 years after appendectomy. In eight patients conservative treatment was successful, and 33 children were operated with the finding of intraabdominal adhesions (23 times), bands (3 times), abscesses and adhesions (7 times). Two children were re-operated on account a relapsing ileus. None of the children died. Prophylaxis of ileus after appendectomy is ineffective and uncertain, therefore the authors emphasize early diagnosis…

A stump appendicitis in a child: a case report.

BACKGROUND: Stump appendicitis is a delayed complication of appendectomy. It is rare and few cases reported in the paediatric literature. The authors report on another case in a child and focus on the diagnostic peculiarities of this entity. CASE: A 9-year-old boy with previous history of open appendectomy was admitted for a right lower quadrant pain with bilious vomiting and fever. Physical examination demonstrated tenderness in the right lower quadrant and guarding over the appendectomy scar. The white blood cell count was 23.500 cells/mm3. Plain abdominal radiograph and ultrasonography rev…

Characteristics of Open and Conversion-to-Open Operation in a Minimally Invasive Era: Implications for the Physician Fee Schedule.

BACKGROUND: CPT valuation historically treated open and minimally invasive (MIS) abdominopelvic operations as interchangeable services performed on similar patients. As MIS has become the dominant approach, open operation may now represent a distinct and more complex clinical scenario. This study characterized contemporary open operation for common abdominopelvic procedures. STUDY DESIGN: The 2023 American College of Surgeons NSQIP database was analyzed for appendectomy, cholecystectomy, right colectomy, low anterior resection, and Hartmann's procedure. Cases were categorized as MIS, straight…

Evidence for genetic correlation between appendix and inflammatory bowel disease: A bidirectional Mendelian randomization study.

BACKGROUND: Observational studies highlighted a link between the appendix and inflammatory bowel disease (IBD), but proving causality has been difficult due to the lack of a clear temporal sequence. METHODS: This research used a two-sample bidirectional univariable Mendelian randomization (MR), multivariable MR and linkage disequilibrium score regression (LDSC) analyses to explore the relationship between acute appendicitis, appendectomy, and IBD. Eligible instrumental variables were screened from previous genome-wide association studies (GWAS) of European ancestry for analysis. The inverse v…

Appendicitis in the Neutropenic Patient: A Systematic Review.

Neutropenia increases the complexity of surgical decision-making in cases of acute appendicitis. In this systematic review, we discuss medical vs surgical management and timing of appendectomy in the neutropenic adult patient. We queried databases utilizing the key words "neutropenia" and "appendicitis." The search identified 999 articles of which 481 articles were reviewed after duplicates were removed. Studies with pediatric patients, single case studies, and abdominal pain in neutropenic patients not caused by appendicitis were excluded. Seven studies remained in this review accounting for …

Myxoglobulosis in the appendix.

Myxoglobulosis is a specific type of mucocele consisting of mucoid material. It is characterized by opaque, transparent globules that resemble "fish eggs" or "frogspawns". It is generally diagnosed incidentally during an appendectomy or an autopsy. In this paper, we aim to present the case of a 58-year-old male patient who was referred to our hospital because of abdominal pain and loss of appetite. The patient underwent an appendectomy. Opaque intraluminal globules were found in the appendectomy material. The globules resembled pearls and they were 2-3 mm in diameter. After histopathological e…

[Diverticular disease of the appendix].

The incidence of appendiceal diverticulosis in pathologic specimens is 0.004-2.1%. Diverticular disease of the appendix is classified as congenital (true) or acquired (false). The clinical presentation differs from that of acute appendicitis. The average age is older, the pain is often intermittent, and while localized in the right lower abdominal quadrant, is of longer duration. No further treatment besides appendectomy is needed. Since a high rate of perforations, peritonitis and lower gastrointestinal bleeding have been reported as complications, it is recommended that in those with an inci…

[Acute appendicitis : can surgery be avoided ?].

Acute appendicitis has traditionally been treated with appendectomy. Over the past decade, several randomized trials have proposed using antibiotics as an alternative in selected patients with uncomplicated acute appendicitis. Approximately 55 to 60 % of patients treated with antibiotics avoid surgery, but the risk of recurrence or complications, particularly abcess formation, remains significant. Careful patient selection with CT imaging is therefore essential to exclude complicated appendicitis or the presence of an appendicolith. Furthermore, nearly 1 % of patients undergoing appendectomy a…

Stump appendicitis.

Stump appendicitis is a rare and late complication following appendectomy and can often be overlooked. Our case details a 42-year-old male who presented to the Emergency Department with right-sided lower abdominal pain, nausea, and vomiting. A computed tomography scan of his abdomen and pelvis demonstrated a tubular, fluid-filled structure with surrounding inflammatory changes at the level of the patient's appendectomy clips with a 2.3-cm calcified intraluminal stone. Findings were concerning for stump appendicitis with appendicolith. He was admitted and taken to the operating room for a lapar…

Delayed Detection of a Carcinoid Tumor after Conservative Therapy for Appendicitis in a 13-Year-Old Boy and a 17-Year-Old Girl.

Acute appendicitis is common in children and adolescents. Recently, conservative antibiotic treatment is regarded to be a safe approach to treat uncomplicated appendicitis. It is already established as initial treatment in cases of perforated appendicitis with perityphlitic abscess, commonly followed by interval appendectomy. We report on a 13-year-old boy with uncomplicated appendicitis and a 17-year-old girl with complicated, perforated appendicitis and perityphlitic abscess in whom initially successful antibiotic treatment led to a delay in detection of a carcinoid tumor (neuroendocrine tum…

Prevalence and clinical implications of diverticulosis of the vermiform appendix.

OBJECTIVE: The epidemiology and the aetiology of inflammatory diseases of the vermiform appendix remain poorly understood. The prevalence of appendiceal diverticulosis and diverticulitis in patients undergoing appendectomy for suspected acute appendicitis was investigated. METHODS: A retrospective study was completed on patients who underwent appendectomy for suspected acute appendicitis. Pathology reports of all patients were screened for diverticula of the vermiform appendix. Patients with either diverticulitis of the vermiform appendix or normal appendicitis were compared. RESULTS: Out of…

Crohn Disease Infrequently Affects the Appendix and Rarely Causes Granulomatous Appendicitis.

Data from previous studies suggest Crohn disease of the appendix accounts for ∼25% of granulomatous appendicitis cases. However, we have found that granulomatous inflammation in appendectomy specimens rarely heralds Crohn disease. We suspect that appendiceal involvement by Crohn disease is uncommon, even when patients have severe ileocolonic inflammation. We performed this study to determine the prevalence and nature of appendiceal inflammation among patients with Crohn disease. We reviewed 100 ileocolic specimens with strictures and fistulizing Crohn disease for the nature and distribution of…
