BACKGROUND: When the ear is presented with two simultaneous pure tones, the cochlea produces intermodulation products called distortion product otoacoustic emissions (DP-OAEs). When cochlear function is affected by noise or ototoxic drugs, DP-OAEs decrease in amplitude or disappear. The aim of this study was to investigate whether DP-OAEs are a sensitive indicator of hearing loss in pilots, for whom flying time presumably correlates with exposure to steady noise.

METHODS: The authors conducted a retrospective analysis of all air medical transports of patients in the Fire Department of the City of New York EMS (emergency medical services) Command for the period of January 1, 1996, to December 31, 1999. These data were evaluated for frequency of air medical transport, patient condition at time of flight, and necessity of air evacuation.

The paper presents cases of surgery of brachiocephalic atherosclerotic lesions with asymptomatic stenosis in civilian pilots reviewed by a Flight Certification Board. Cerebrovascular diseases varying in their clinical presentation (syncopes, transitory ischemic attacks or strokes) may culminate in acute conditions and, consequently, threaten flight safety. There is an exigent need of law-guided regulations for flight certification boards on how to manage cases of pilots with a history of cerebrovascular disease.

This paper reports on differences between the clinical and radiographic detection of caries, on both occlusal and approximal surfaces, in a cohort of Royal Air Force (RAF) recruits (mean age 18.9 years). It was found that the clinical examination detected only 30 per cent of the total lesions found by both methods combined. The radiographic examination found 201 per cent extra occlusal dentinal lesions and 192 per cent extra approximal dentinal lesions. Available evidence implicates alcohol consumption as a major risk factor for almost all types of injury. An exceptionally strong relationship is noted between alcohol and motor vehicle accidents--particularly single-vehicle crashes. Recognition of the association between alcohol and trauma is important not only for proper care and for treatment of the underlying alcoholism, but to stimulate enactment of preventive measures aimed at reducing the risk of alcohol-associated injuries.
