[Mechanism of action, clinical indication and results of treatment of botulinum toxin].

Botulinum toxin, the most potent of the neurotoxins, produces paralysis by blocking presynaptic release of the neurotransmitter (acetylcholine) at the neuromuscular junction, with reversible chemical denervation of the muscle fibre, thereby inducing partial paralysis and atrophy. Because chemical denervation is reversible, botulinum toxin has temporary effects, the muscle being progressively reinnervated by nerve sproutings. Type A botulinum toxin (Bix-A) is available under two dosage forms: Botox and Dysport. Although the initial clinical indication was strabismus, subsequent studies have dem

The effects of botulinum toxin on ocular tissue.

This report examines the beneficial use of botulinum toxin in the treatment of ophthalmic disorders. It aims to describe the effects and uses of the toxin along with the ophthalmic conditions it is used to treat. The use of botulinum toxin as an effective treatment of ophthalmic disorders is a recent development, with the first treatment occurring in 1983.

Clostridium botulinum toxins: nature and preparation for clinical use.

C. botulinum neurotoxins are acutely toxic materials and act by inhibiting release of the neurotransmitter acetylcholine. The specific nature of this inhibition is discussed and the preparation and purification of Type A toxin specifically for clinical use is described.

Noninvasive transcranial brain stimulation for eye diseases and visual dysfunctions: A systematic review of RCTs.

Noninvasive transcranial brain stimulation (NTBS) has emerged as a potential therapeutic approach for eye diseases and visual dysfunctions. This systematic review evaluates the evidence from 23 randomized controlled trials examining the application of NTBS, including transcranial electrical stimulation and transcranial magnetic stimulation, in conditions such as amblyopia, myopia, visual field defects, glaucoma, diabetic retinopathy, macular degeneration, and blepharospasm. Studies assessed outcomes such as visual acuity, contrast sensitivity, and visual perception, with variable effects obser…

A rare ocular manifestation of mycoplasma pneumoniae infection.

INTRODUCTION: Ocular manifestations of Mycoplasma Pneumoniae infection are rare. We present a case of Mycoplasma Pneumoniae infection revealed by a recurrent retro-bulbar optic neuritis. CASE DESCRIPTION: A 38-year-old woman, initially treated for a typical retro-bulbar optic neuritis of the right eye with corticosteroid therapy at high doses, presented for a recurrent decreased vision in the same eye, associated with blepharospasm and conjunctival hyperemia. The etiological assessment revealed a recent Mycoplasma Pneumoniae infection. The patient was treated with corticosteroids and fluorqui…

Life-threatening cranial dystonia following trihexyphenidyl withdrawal.

A 64-year-old woman with blepharospasm, sustained clenching of the jaw, antecollis, and a strained, high-pitched phonation continued chronic trihexyphenidyl therapy despite the lack of any obvious benefit. Abrupt, accidental withdrawal of trihexyphenidyl triggered severe exacerbation of the cranial dystonia associated with inspiratory stridor and acute respiratory difficulties, prompting emergency admission. On indirect laryngoscopy, hyperadduction of the vocal folds was not the cause of the upper airway obstruction. A more likely cause of the inspiratory obstruction appeared to be forward ben…

Posttraumatic stress disorder in electric burn patients.

It was hypothesized that burn injuries electric in origin may have a poor prognosis for rehabilitation and return to work. Ten electrically injured burn patients were compared to seven patients whose burns were nonelectric, after all 17 had shown clinically significant emotional problems during rehabilitation. Results of psychological tests from both groups were also compared to results in three other groups: psychiatric, chronic pain, and blepharospasm patients. Results indicated that patients with electric burns had the most severe psychopathologic symptoms and the least likelihood of return…

Orbital Rosai-Dorfman Disease: A Case Report and Literature Review.

A 53-year-old male presented with dropping of the right eyelid associated with decreased visual acuity for 4 months. He also complained of vertical diplopia especially when looking down. Ophthalmological examination revealed right blepharospasm associated with right hypertropia. There was palpable mass at the inferomedial aspect of the right eye. Magnetic resonance imaging revealed abnormal signal intensity in the right orbit inferior aspect occupying the orbital floor and measured 2.7 cm × 2.5 cm × 1.2 cm and showed enhancement on the postcontrast study. The patient underwent complete excisio…

Advanced spheroidal degeneration.

Spheroidal degeneration, involving the cornea and/or the conjunctiva, is characterized by amber-colored homogeneous, translucent spherules in the corneal stroma, Bowman's membrane, and subepithelium. The condition has a higher prevalence in areas with extreme temperatures, low humidity, high wind, and presence of sand. We report the case of a 46-year-old man with a 10-year history of gradual progressive diminution of vision, severe blepharospasm, and photophobia. Examination revealed bilateral plaques of amber-colored nodules covering about half of the cornea. Superficial keratectomy was perfo…

How Many Dystonias? Clinical Evidence.

Literary reports on dystonia date back to post-Medieval times. Medical reports are instead more recent. We review here the early descriptions and the historical establishment of a consensus on the clinical phenomenology and the diagnostic features of dystonia syndromes. Lumping and splitting exercises have characterized this area of knowledge, and it remains largely unclear how many dystonia types we are to count. This review describes the history leading to recognize that focal dystonia syndromes are a coherent clinical set encompassing cranial dystonia (including blepharospasm), oromandibula…

Feline thelaziosis caused by Thelazia callipaeda in Portugal.

Thelazia callipaeda (Spirurida, Thelaziidae) is a nematode that lives in the conjunctival sac of domestic and wild carnivores, rabbits and humans causing mild to severe symptoms (e.g., conjunctivitis, lacrimation, epiphora, blepharospasm, keratitis and even corneal ulceration) in infected animals. This report describes an autochthonous case of thelaziosis in a cat from the central region of Portugal, representing the most occidental record of thelaziosis in Europe. Adult nematodes recovered from alive animal were morphological identified as T. callipaeda. A portion of the mitochondrial cytochr…

[Pantogam active in the complex treatment of dystonic hyperkineses].

Efficacy of pantogam active in the complex treatment of facial and cervical dystonic hyperkineses was studied in 37 patients (25 women, 12 men) with dystonic torticollis, blepharospasm, oromandibular dystonia with illness duration more than 6 months. In these patients, the previous treatment had no positive effect. Pantogam active was prescribed in dose 900-1800 mg/day during 12 weeks. Efficacy was assessed with the TWSTRS, the HADS and the CGI-I. Pantogam active as add-on to basic treatment led to the reduction of severity in dystonic hyperkineses on the average 2-2,5 months from baseline. Th…
