[Treatment of diaphyseal fractures of long tubular bones by functional intraosseous osteosynthesis].

A clinical analysis of treatment of 328 diaphyseal fractures of long bones in 304 patients by functional intraosseous osteosynthesis which is not to be followed by plaster immobilization has shown that the function was reestablished long before the consolidation. The functional intraosseous synthesis was performed without drilling of the bone marrow cavity, without special appliances or devices. Terms of invalidism are shorter than traditional ones. Consolidation of the fractures takes place within 5-10 months. The nails are removed at the same terms. The method is of low cost, easy in use, re

Dynamic compression plating versus locked intramedullary nailing for humeral shaft fractures in adults.

BACKGROUND: Surgical fixation of fractures of the shaft of the humerus generally involves plating or nailing. It is unclear whether one method is more effective than the other. OBJECTIVES: To compare compression plating and locked intramedullary nailing for primary surgical fixation (surgical fixation of an acute fracture or early fixation following failure of conservative treatment) of humeral shaft fractures in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (February 2011), The Cochrane Library 2011, Issue 1, MEDLINE and EMBASE (b

[Prévot nailing, an elegant method. Experiences with 38 personal cases].

Endomedullar elastic stabilization with titanium Prévot nails is an excellent method of treating long-bone fractures in children. Without touching the epiphyseal line, this method guarantees early functional follow-up treatment with minimal operative trauma. For this reason, this method can compete successfully with common methods like extension treatment, external fixation and osteosynthesis with plates. From October 1990 to October 1993 we treated more than 40 fractures with Prévot nails and checked 38 of them clinically and radiologically. All 17 femur fractures healed correctly from an ana

Calcitonin Gene-Related Peptide Antagonist Use and Atypical Femur Fractures: A Case Report.

CASE: A 56-year-old woman presented with bilateral hip pain status postmechanical fall. Imaging confirmed bilateral AFFs with lateral cortical beaking. The patient was found to be on a calcitonin gene-related peptide (CGRP) antagonist, Erenumab, for treatment of recurrent migraines. CGRP is a neuropeptide and vasodilator produced by neurons in both the central and peripheral nervous system. She was treated with bilateral femoral intramedullary nails, followed by postoperative rehabilitation with physical therapy and outpatient follow-up visits. CONCLUSION: Erenumab is a CGRP antagonist, which

[Research progress of dyskeratosis congenita].

Dyskeratosis congenita (DC) is a rare disease and a genetic heterogeneity of bone marrow failure, characterized by muco-cutaneous triad of mucosal leukoplakia, abnormal skin pigmentation, nails dystrophy and often involving multiple organs or systems. The inheritance patterns of DC include X-linked recessive, autosomal dominant and recessive patterns. However, the inheritance patterns in 30%-40% of DC patients remained unknown. Dyskeratosis congenita is difficult to diagnose because of its genetic and clinical heterogeneity. This article will review and discuss the state-of-the-art progresses

Femoral and Tibial Reconstruction for Skeletal Metastases and Pathologic Fractures.

Skeletal metastases of the femur and tibia leading to pathologic fractures or large skeletal defects can be managed with surgical reconstruction, resulting in improved patient outcomes and functionality. The indications for femoral and tibial reconstruction are dependent on several factors, including goals of management, age, comorbidities, site and extent of the lesion, soft-tissue involvement, and history of radiation or other systemic therapy. The goal of reconstruction of large bone defects is to restore anatomy and function while minimizing the risk of complications, implant failure, and

Surgical options for the treatment of simple bone cyst in children and adolescents.

BACKGROUND: There are several treatment options for simple bone cysts, with treatment depending mainly on the experience and preference of the surgeon and the extension and location of the cyst. OBJECTIVES: To assess our experience with the surgical treatment of bone cyst lesions in pediatric patients at one institution by the same group of surgeons. METHODS: The study group comprised 60 patients (43 boys, 17 girls) treated surgically for monostatic lesions between January 2002 and July 2007. The mean age at surgery was 11.8 years (range 4-17 years). Mean follow-up was 4.2 years. Most of the

Treatment of Radiation-Associated Fractures: A Critical Analysis Review.

»: Fractures that result from radiation-induced bone damage are a recognized adverse effect of radiation therapy (RT). »: The mechanisms of damage from RT are thought to be related to impaired vascularity, aberrations in osteoclast and osteoblast-mediated bone turnover, and compromise of cortical and trabecular microarchitecture. »: Treatment of radiation-associated fractures is challenging, with rates of delayed union and nonunion of >50%. »: Fracture management strategies, each with its own profile of risks and benefits, include prophylactic intramedullary nailing of long bones, open redu

Treatment of a Ballistic Radius Fracture with Segmental Bone Loss Using the Masquelet Technique in a Child: A Case Report.

CASE: A 4-year-old boy sustained an accidental self-inflicted gunshot wound to the left forearm. Radiographs revealed a comminuted mid-diaphyseal ballistic radius fracture with a critical-sized bone defect. The fracture was treated with the placement of a flexible intramedullary nail and antibiotic cement spacer, followed by second-stage bone grafting and open reduction and internal fixation of the radius 6 weeks later. Four months after the second-stage procedure, the radial defect healed appropriately without complications. CONCLUSION: In this case of a pediatric comminuted mid-diaphyseal r

Zimmer Natural Nail and ELOS nails in pertrochanteric fractures.

BACKGROUND: Pertrochanteric fractures of the femur in the elderly are very common. As the average age of the population increases, the incidence of such fractures also raises, resulting in high healthcare costs. The type of surgical devices employed for their surgical management influences these costs. METHODS: A comparative clinical study was conducted on patients operated by one single surgeon between December 2018 and November 2020 in a high-volume regional referral centre. All patients who received a Zimmer Natural Nail (ZNN) or ELOS devices were included. RESULTS: In 119 (66.48%) of the

Subungual melanoma: histological examination of 50 cases from early stage to bone invasion.

Subungual melanoma is a rare form of malignant melanoma. It is extremely difficult to differentiate it histologically from benign melanonychia striata or melanocytic nevus, especially in the early stage. We divided 50 cases of subungual melanoma into four groups according to clinical progress, and examined their histological findings in each respective stage. In the early stage (19 cases), atypical melanocytes were polygonal showing slight nuclear atypia with no mitoses at all. In six out of 19 cases (31.6%), the atypical melanocytes proliferated more in the hyponychium than in the nail matrix

Management of a case of melorheostosis deformity and pain with intramedullary nailing.

Melorheostosis is a rare sclerosing bone dysplasia characterized by the thickening and irregular ossification of the affected bones. This disease can affect any bone in the skeletal system, with the long bones of the lower extremities being more frequently involved. Symptoms include pain, oedema, joint stiffness, muscle atrophy and bone deformity. Management of this disease is challenging and not standardized. It depends on the symptoms, and it varies from physical therapy to pharmacological approach or surgery to correct limb deformities, muscle and tendon contractures and joint stiffness. Th

Chemotherapy Release From Bortezomib-Impregnated Polymethylmethacrylate-Coated Intramedullary Nails: A Novel In Vitro Study for a Local Chemotherapy Delivery Device.

Bortezomib (BAN) is a proteasome inhibitor approved for the treatment of multiple myeloma and lymphoma. Despite its efficacy in various tumor models, systemic administration can result in toxicity to healthy organs. The purpose of this study is to evaluate the elution profile of BAN from PMMA cement for the local treatment of orthopedic tumors. BAN solution (5 mg; 2 mg/mL) was mixed with Simplex cement (40 g, Stryker), followed by injection of cement into an antibiotic cement nail mold (13 mm) to coat a 10 mm titanium femoral nail (DePuy Synthes). Once the cement polymerized, the nail was cut

Late symptomatic heterotopic ossification of the patellar tendon after medial parapatellar intramedullary nailing of the tibia.

This article describes a case of a 21-year old man who presented with symptomatic heterotopic ossification of the patellar tendon 3.5 years postinjury. The patient sustained an open tibia fracture during a fall and was treated by reamed intramedullary nailing through a medial paratendinous approach. Radiographic evidence of heterotopic ossification in the patellar tendon was first noted at the 4-week follow-up. He presented at 3.5 years postinjury with new-onset anterior knee pain and reported no interval trauma to the knee. Radiographs revealed a well-circumscribed area of calcification withi
