Holmes Tremor Following Midbrain Abscess in an Immunocompromised Patient: A Case Report.

Holmes tremor (HT) is a rare occurrence due to insults located at the midbrain. We are reporting a case of HT, which occurred in a retrovira … d of a tremor that characteristics of HT with repeated computed tomography brain revealed resolving brain abscess. This case highlights an important reminder of potential neurological sequelae due to midbrain abscess and the importance of recognition of HT. The management of HT is challenging, involving the initial use of dopaminergic agonists with a potential …

Cerebral abscess due to an abnormal drainage of the superior vena cava.

INTRODUCTION: Causes of brain abscesses are multiple and often unclear. We present the case of a pregnant woman who developed a temporal brain abscess revealed by an epileptic seizure. OBSERVATION: Perfusion lung scan was performed because she had a mild hypoxemia: absence of contrast into the pulmonary arteries was suggestive of a massive intracardiac shunt or an anomalous drainage of the superior vena cava. Contrast-enhanced echography showed an abnormal drainage of the superior vena cava into the left atrium. CONCLUSION: Although rare, this anomaly can exist in adults. This report emphasi…

Brainstem abscess treated conservatively.

BACKGROUND: Brainstem abscess is a rare condition with a variety of treatment approaches. In this paper, we report an unusual case of a brainstem abscess with a positive outcome in an immunocompetent patient who was treated with antibiotic therapy. CASE DESCRIPTION: A 22-year-old female presented with bilateral tetraparesis that was worse on the left hemibody, appendicular tremor, and left upper eyelid ptosis. Brain magnetic resonance imaging showed an abscess in the pons and midbrain due to possible nocardiosis. She was treated with dexamethasone, phenytoin, vancomycin, and meropenem for 8 w

Nocardia asteroides cerebral abscess in a renal transplant recipient: short report.

Opportunist central nervous system infections occur in about 5% to 10% of all renal transplant recipients, but reports of brain abscesses are very rare (1). Nocardia asteroides cerebral abscesses are scarce intracranial lesions. They account for only 2% of brain abscesses (2). Published data about these lesions have taken the form of short reports, small cases series and reviews. A universally accepted and effective treatment approach has not yet been established. We present a renal transplant patient with a cerebral abscess caused by Nocardia asteroides.

Infectious lesions of the brain stem.

Infectious lesions of the brain stem are rare and include primarily abscess and encephalitis. The most common etiologic agents for abscess formation are Streptococcus sp., Staphylococcus sp., and M. tuberculosis. Encephalitis is associated most often with L. monocytogenes and herpes simplex virus infection. Classical brain stem syndromes are uncommon with brain stem infections, and CSF obstruction can be seen with neurocysticercosis. The diagnosis of these lesions has been greatly aided by CT and MR imaging. Microsurgery and stereotaxis are both appropriate techniques for the treatment of brai

[Gas shadow on CT scan following treatment in metastatic brain tumor--an autopsy case].

This is an autopsy case report of a 52-year-old woman with a brain abscess presenting as an intracerebral gas shadow on CT scan. She was admitted to our facility in June 1977, with disorientation and motor weakness of the right upper extremities. CT scan revealed two separate mass lesions in the frontal and occipital lobes on the left side. She was diagnosed as having metastatic carcinoma, and was treated by tumor removal, radiation therapy, and intraneoplastic local chemotherapy. Seventeen months later, she was readmitted with decreased mental activity, hemiconvulsion of the right side and hi…

Cerebellar abscesses: a review of seventeen cases.

Seventeen cases of cerebellar abscesses are reviewed. The incidence was 18% of all brain abscesses. Thirteen were males, and four were females. Headache was the most common presenting symptom. These abscesses have a better prognosis for survival and neurological function than supratentorial abscesses. They are less likely to be encapsulated. Continued neurological decline in the face of adequate surgical treatment of a single abscess should alert the physcian to the possibility of a second abscess.

Cerebellar abscess associated with pulmonary arteriovenous fistula and hereditary hemorrhagic telangiectasia--case report.

A 57-year-old male with a past history of bilateral pulmonary arteriovenous fistulas (PAVFs) experienced a sudden onset of headache and gait disturbance. There was a family history of PAVF and recurrent epistaxis. He had diffuse telangiectasia of the tongue, and hereditary hemorrhagic telangiectasia was diagnosed. Neuroimaging revealed a brain abscess in the right cerebellar hemisphere, which was successfully aspirated under ultrasound guidance. The PAVFs were resected afterwards in two-staged operation. No recurrence of the abscess has been observed. Cerebral abscesses complicated by PAVF are…

Potential otogenic complications caused by cholesteatoma of the contralateral ear in patients with otogenic abscess secondary to middle ear cholesteatoma of one ear: A case report.

BACKGROUND: Otogenic brain abscess caused by middle ear cholesteatoma is a potentially serious and life-threatening complication in the ear, nose, and throat clinic. The mortality rate associated with otogenic brain abscesses is 8%-26.3%. Recently, in China, the incidence of brain abscess secondary to middle ear cholesteatoma has started to increase due to antibiotic resistance. CASE SUMMARY: A 55-year-old male presented hearing loss in the right ear and headache for 1 mo in 2018. Computed tomography (CT) showed an area of low density in the right middle ear and mastoid and auditory ossicle d…

Flare up of tuberculous abscess following stereotactic aspiration--case report.

A 16-year-old non-immunocompromised and otherwise healthy patient presented with a tuberculous brain abscess. The abscess was aspirated stereotactically. Following the aspiration the disease process flared up and multiple daughter abscess cavities were discovered. The daughter abscesses were removed via craniotomy. Stereotactic aspiration requires care to prevent such flare up of the disease.

[Stereotaxic brain puncture. Indications and results].

228 stereotactic biopsies for the diagnosis of brain lesions were evaluated. In 98.2% a diagnosis could be established. The diagnosis was made of brain tumor in 79.3%, of brain abscess in 7.5% and of encephalitis in 3.5% of cases. In 2.2% of the patients a pathological finding was obtained without a clear histological definition. No histological diagnosis could be made in 1.8% of the patients. A severe complication of brain biopsy occurred in 2.2% of the patients (1 death, 1 abscess, 3 increased impairments). The stereotactic procedure appears to have a low mortality and morbidity, and to be h…

Frontal sinus osteoma complicated by extended intracranial mucocele and cerebral abscess: neurosurgical strategy of a rare clinical entity.

The combination of paranasal sinus osteoma, mucocele and brain abscess is rare. Only one other case has been reported so far, where the cerebral abscess was diagnosed intraoperatively. We report here on a second patient with combined frontal sinus osteoma, intracerebral mucocele as well as cerebral abscess. However, in our case, the diagnosis of cerebral abscess was reached prior to surgery, which allowed for a tailored treatment strategy with abscess puncture under neuronavigational guidance and, after 3 weeks of antibiotic therapy with documented abscess shrinkage, osteoma and mucocele resec…

Listeria Bacteremia Presenting With Cerebral Abscess and Endocarditis in an Elderly Patient With Chronic Immune Thrombocytopenia.

Central nervous system involvement by Listeria monocytogenes usually presents as meningitis, meningoencephalitis or, less frequently, rhombencephalitis. Listerial brain abscesses are rare. Moreover, only 5-8% of listerial bacteremia is complicated by infective endocarditis (IE). A 70-year-old man with chronic immune thrombocytopenia (ITP) presented to our emergency department with acute onset of altered mental status and right-sided weakness. He was afebrile, with no heart murmurs or peripheral IE stigmata. Neurologic examination showed disorientation, expressive aphasia, and right-sided hemip…
