Diagnosis, medical treatment, and stepwise mechanical circulatory support for fulminat myocarditis.

Fulminant myocarditis is one of the most challenging diseases. We sought to examine the outcomes of our multidisciplinary treatment strategy for fulminant myocarditis. A retrospective review of consecutive 30 patients with fulminant myocarditis was conducted. Of the 30 patients, 25 required mechanical circulatory support (MCS). Percutaneous extracorporeal membrane oxygenation (ECMO) was the first-line therapy to rescue the patients and inserted in 23 of them. The other 2 were implanted with temporary ventricular assist device (t-VAD) with extracorporeal centrifugal pump(s). Sixteen of the ECMO

Lineage-specific regulation of PD-1 expression in early-stage hepatocellular carcinoma following 90yttrium transarterial radioembolization - Implications in treatment outcomes.

BACKGROUND: Hepatocellular carcinoma (HCC) remains one of the leading causes of cancer-related deaths in the world. Liver-directed therapies, including 90Yttrium (90Y) radioembolization, play an integral role in the management of HCC with excellent response rates. This has led to clinical trials of immunotherapy in combination with 90Y. Elevated PD-1 expression and lymphopenia were recently shown as risk factors for disease progression in early-stage HCC treated with liver-directed therapies. The aim of this study was to investigate PD-1 expression dynamics in bridge/downstage to transplant in

The Efficacy of Short-Term Bridging Strategies With High- and Low-Dose Prednisolone on Radiographic and Clinical Outcomes in Active Early Rheumatoid Arthritis: A Double-Blind, Randomized, Placebo-Controlled Trial.

OBJECTIVE: In active early rheumatoid arthritis (RA), glucocorticoids are often used for bridging, due to the delayed action of methotrexate. This study was undertaken to compare the effect of 3 bridging strategies, including high-dose and low-dose prednisolone, on radiographic and clinical outcomes. METHODS: Adult RA patients from 1 rheumatology hospital and 23 rheumatology practices who presented with moderate/high disease activity were randomized (1:1:1) to receive 60 mg prednisolone (high-dose prednisolone [HDP]) or 10 mg prednisolone (low-dose prednisolone [LDP]) daily (tapered to 0 mg w

A Comprehensive Review on Engineering Release at the Molecular Level: Role of Functional Groups in Polysaccharide-Based Drug Delivery.

Stimuli-responsive drug-delivery systems (SRDDS) have transformed precision medicine by enabling spatiotemporal control over therapeutic release, significantly reducing off-target toxicity while enhancing efficacy at the disease site. Naturally occurring polysaccharides, such as hyaluronic acid, chitosan, alginate, and dextran stand out as premier scaffolds for these "smart" nanoplatforms due to their inherent biocompatibility, biodegradability, and abundance of reactive sites for molecular engineering. This review explores the versatility of polysaccharide functionalization, detailing how the

Management of direct oral anticoagulants in patients undergoing elective surgeries and invasive procedures: Updated guidelines from the French Working Group on Perioperative Hemostasis (GIHP) - September 2015.

Since 2011, data on patients exposed to direct oral anticoagulants (DOAs) while undergoing invasive procedures have accumulated. At the same time, an increased hemorrhagic risk during perioperative bridging anticoagulation without thrombotic risk reduction has been demonstrated. This has led the GIHP to update their guidelines published in 2011. For scheduled procedures at low bleeding risk, it is suggested that patients interrupt DOAs the night before irrespective of type of drug and to resume therapy six hours or more after the end of the invasive procedure. For invasive procedures at high b

Therapeutic strategies in epithelial ovarian cancer.

Ovarian cancer is the most lethal gynecologic malignancy. It appears that the vast majority of what seem to be primary epithelial ovarian and primary peritoneal carcinomas is, in fact, secondary from the fimbria, the most distal part of the fallopian tube. Treatment of epithelial ovarian cancer is based on the combination of cytoreductive surgery and combination chemotherapy using taxane and platinum. Although clear cell type is categorized in indolent type, it is known to show relatively strong resistance to carboplatin and paclitaxel regimen and thus poor prognosis compared to serous adenoca

[CAR-T cell therapy in clinical practice].

Chimeric antigen receptor (CAR) T-cell therapy has demonstrated remarkable clinical efficacy in relapsed or refractory B-cell acute lymphoblastic leukemia, B-cell lymphoma, and multiple myeloma. In Japan, five CAR-T products have already been approved and have rapidly achieved widespread adoption in real-world clinical practice. CAR-T therapy involves multiple complex steps, including autologous T-cell collection, manufacturing, bridging therapy, infusion, and management of adverse events, each of which requires careful optimization. Disease relapse or progression after CAR-T therapy also rema

Biological activation of prolonged delayed fracture healing following abaloparatide under stable circular external fixation: successful limb salvage of a Gustilo type III-C pilon fracture in severe immune thrombocytopenia-a case report.

BACKGROUND: Delayed fracture healing may persist despite adequate mechanical stability, reflecting biological insufficiency rather than mechanical failure. Although abaloparatide has osteoanabolic effects, its role in prolonged delayed fracture healing remains unclear. CASE PRESENTATION: A 59-year-old Japanese man with severe immune thrombocytopenia (platelet count approximately 5000/microL) sustained a Gustilo type III-C open pilon fracture after a crush injury. Emergency management included aggressive debridement, posterior tibial artery and vein repair, and circular external fixation. Beca

Efficacy and safety of inotuzumab ozogamicin as the definitive therapeutic option prior to allogeneic hematopoietic stem cell transplantation for pediatric relapsed/refractory B-cell acute lymphoblastic leukemia.

Among patients with relapsed/refractory (R/R) B-cell acute lymphoblastic leukemia (ALL), those who failed to respond to CD19-directed immunotherapies, there are limited options available pre-HSCT. This study aimed to evaluate the efficacy and safety of Inotuzumab ozogamicin (InO) as the definitive treatment pre-HSCT in patients with R/R B-cell B-ALL. Sixteen patients were enrolled, ten of whom failed treatment with prior CD19-directed immunotherapies, and two of whom relapsed after first HSCT. All patients achieved morphologic complete remission; 13 patients achieved negative measurable residu

Azacitidine combined with venetoclax alleviates AML-MR with TP53 mutation in SDS: a case report and literature review.

Shwachman-Diamond syndrome (SDS) is an autosomal recessive genetic disease, which is prone to transform into myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). TP53 mutation is a driving factor involved in the transformation of SDS into MDS/AML, and in the evolution of MDS to AML. Allogeneic hematopoietic stem cell transplantation (Allo-HSCT) is the only curable approach, however, challenge remains regarding the balance between efficacy and the high risk from treatment-related toxicity and mortality to achieve temporary disease control before transplantation to gain time and oppo

Single-agent gemcitabine enabling successful bridging to allogeneic haematopoietic stem-cell transplantation in refractory peripheral T-cell lymphoma, not otherwise specified.

A man in his 20s presented with fever, night sweats and marked leucocytosis (94×109/L). Cervical lymph node biopsy confirmed peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS). The disease was refractory to brentuximab vedotin plus cyclophosphamide, doxorubicin and prednisone, SMILE and pralatrexate. Subsequently, single-agent gemcitabine (1000 mg/m² on days 1, 8 and 15) induced complete remission, unconfirmed (CRu), after two cycles, enabling reduced-intensity allogeneic haematopoietic stem-cell transplantation from an HLA one-locus-mismatched related donor in the host-versus-graf

Successful rechallenge with azacytidine and venetoclax after sustained treatment-free remission in a relapsed acute myeloid leukemia patient: a case report.

Combined therapy with venetoclax and hypomethylating agents has significantly improved the outcome of unfit patients ineligible for intensive chemotherapy. A recently published exploratory analysis of the VIALE-A trial reported that up to 51% of patients achieving remission survived more than 2 years. These data along with those from reallife settings, lead to questioning how long it is appropriate to continue treatment in long-term survivors. Accordingly, recent retrospective studies suggested the feasibility of suspending therapy in selected patients while maintaining prolonged responses. Al

Advances in pathogenesis research and challenges in treatment development for acute myeloid leukemia.

Acute myeloid leukemia (AML) develops when hematopoietic stem cells acquire chromosomal and genetic abnormalities, transforming into leukemia stem cells (LSCs) and further gaining driver mutations. Advances in genomic analysis have identified numerous new gene mutations involved in AML development. Recent research has shown that individuals with germline mutations in genes like DDX41 and CEBPA develop AML upon acquiring additional somatic mutations, and the latest WHO classification separates AML with such mutations into distinct disease groups. LSCs are regulated by different metabolic proces

Biomarker-directed therapy in multiple myeloma.

PURPOSE OF REVIEW: Multiple myeloma is currently treated with a one-size-fits-all approach despite significant heterogeneity in patient outcomes and disease molecular constitution. A personalised approach would tailor therapy to unique patient or disease characteristics. RECENT FINDINGS: Well established prognostic biomarkers such as cytogenetic risk and patient frailty status are being evaluated as potential predictive biomarkers. Specifically, treatment intensity can be augmented in high-risk patients or conversely attenuated in those at lower risk or lower ability to withstand treatment to

Recurrence-free survival of a hepatocellular carcinoma patient with tumor thrombosis of the inferior vena cava after treatment with sorafenib and hepatic resection.

Sorafenib (Nexabar, Bayer, Berlin, Germany), one of multikinase inhibitors, can infrequently downstage advanced hepatocellular carcinoma (HCC). There are some reports that sorafenib in combination with other modalities, such as transcatheter arterial chemoembolization (TACE) or radiation therapy, could represent a bridge to surgery. We have observed a progressive HCC case with hepatic vein tumor thrombosis proceeding to the inferior vena cava (IVC-HVTT) convert to a state of feasible curative resection after a multidisciplinary treatment which included sorafenib. The patient underwent a succes
