Uncovering the mystery of the atrioventricular node dual-pathway electrophysiology.

With the discovery and validation of a novel index of dual-pathway electrophysiology, known as the His electrogram alternans, major advances have been made in understanding the electrical propagation through the atrioventricular (AV) node. At slow rates, AV conduction starts at the interatrial septal boundary of the node (superior nodal domain) and spreads toward the tricuspid annulus side (inferior nodal domain) in a direction perpendicular to the AV axis and across fiber orientation. Such activation results in an early, superior input into the superior His bundle. This is the fast pathway (F

Effectiveness of surgical management of the Wolff-Parkinson-White syndrome.

In this report, the surgical experience with the treatment of Wolff-Parkinson-White syndrome in 190 patients who had 210 Kent bundles has been summarized. The patients with reentry tachycardia caused by the Kent bundle were relieved by Kent division or by His division, the latter being used only in 10 percent of the patients. A malignant ventricular arrhythmia was found in 25 percent of the patients and was due to a Kent bundle that conducted an atrial flutter-fibrillation 1:1 to the ventricle. This arrhythmia was corrected by Kent interruption, since the Kent bundle alone participated in the

[Changes in electrocardiographic and angiographic images in the course of myocardial infarction].

Different types of disorders, varying in time, concerning atrio-ventricular (AV) conductivity may occur during myocardial infarction (MI). Similarly, angiography may reveal the dynamics within lesions of thrombosis in arteries. A case of 58 year-old man, who suffered from temporal AV block of all degrees, and what is more, a block in anterior fascicle or the whole left bundle branch in the bundle of His is presented. Only the second coronary arteriography revealed a thrombosis in coronary artery. Successful angioplasty resulted in relief of symptoms and conductivity disorders. Clinical course …

Supraventricular tachycardia: mechanisms and treatment.

Supraventricular tachycardia is a very commonly encountered cardiac arrhythmia in clinical practice. The term "supraventricular tachycardia" refers to a rapid dysrhythmia which originates at or above the level of the His bundle of the atrioventricular (AV) conduction system. Characteristically, except for tachycardia-related bundle branch block, the QRS complex during supraventricular tachycardia is identical to that seen in sinus rhythm. By using intracardiac recordings along with electrical stimulation, various mechanisms of supraventricular tachycardia have recently been delineated electrop

Mitral valve prolapse and conduction defects in Ehlers-Danlos syndrome.

A case of type 3 Ehlers-Danlos syndrome with associated mitral valve prolapse, right bundle-branch block, and left anterior fascicular block was studied, including His bundle electrocardiography and echocardiography. Mitral valve prolapse and HV prolongation were reported. The present study suggests that the basic underlying collagen defect in this syndrome is responsible for these findings, and that the association may be more widespread in the syndrome than currently acknowledged. The possibility also is considered that the connective tissue abnormalities of Ehlers-Danlos syndrome involving

Increasing pace mapping properties in parahissian premature ventricular contraction. Novel insight from HD grid multipolar catheter.

A 23-year-old athlete with symptomatic low burden premature ventricular contraction (PVC) with left bundle branch block morphology and inferior axis morphology was sent to our department for RV mapping and PVC ablation. Exit zone of the PVC was easily and clearly defined by the bipoles A3-A4 achieving optimal and detailed pacemapping (Panels A-C) near the His bundle (yellow dots). The spatial conformation and the smooth shape of the catheter would definitely help everyday procedures in the setting of low burden PVC/noninducible focal ventricular arrhythmia, especially when the focus is located

Paroxysmal fascicular tachycardia and ventricular tachycardia due to mechanical stimulation by a mitral valve prosthesis.

Electrophysiologic studies were performed in a woman who had two varieties of paroxysmal wide QRS tachycardia after mitral valve replacement with a Starr-Edwards prosthesis. One tachycardia originated in the left anterior fascicle; QRS complexes were 100 ms wide and resembled right bundle branch block with left posterior fascicular block, and a His bundle potential preceded each QRS by an interval of 20 ms (compared with 50 ms during sinus rhythm). The other tachycardia originated in the left ventricle. Clinical and echocardiographic observations suggested that the tachycardias were caused by

Interesting case of narrow QRS tachycardia with atrioventricular dissociation.

A 30-year-old man with no structural heart disease has been evaluated for paroxysmal palpitation with documented regular narrow QRS tachycardia that has not responded to intravenous adenosine. Surface electrocardiogram has not shown any pre-excitation. He has been taken for an electrophysiology study after informed consent. Diagnostic catheters were placed at the coronary sinus, His bundle region, and right ventricle. During catheter manipulation a regular narrow QRS tachycardia with incomplete right bundle branch block morphology and normal QRS axis similar to the clinical tachycardia got ind

Prognosis for patients with congenital heart disease and postoperative intraventricular conduction defects.

Intraventricular conduction defects are common following repair of various forms of congenital heart disease. Such defects may affect adversely the long-term prognosis of patients in whom cardiac hemodynamics were adequately restored. Review of previously published studies suggests that the site of the conduction defect may be the reason for the different prognoses reported for patients from different institutions. The so-called "trifascicular block" pattern which sometimes occurs following open heart surgery is probably due to a more extensive lesion to the branching and penetrating parts of

Anatomic, electrical, and hemodynamic characterization of Bachmann bundle in the swine heart.

BACKGROUND: Bachmann bundle area (BBA) pacing is an attractive alternative to traditional right atrial appendage (RAA) pacing, but its hemodynamic benefits are poorly understood. OBJECTIVE: The purpose of this study was to evaluate hemodynamic effects of BBA pacing and structural characteristics of the aligned myocytes within BBA. METHODS: Five swine were subjected to volume overload by intravenous infusion of 3 L of 0.9% saline solution during 30 minutes. Medtronic 3830 pacing leads were placed in the RAA and BBA. Rates were alternated between the 2 sites at 110, 120, and 130 beats/min, wit

Physiological pacing with a DF-1 single chamber defibrillator in a patient with permanent atrial fibrillation and heart block: A case report.

We report a case report of a 68-year-old man with chemotherapy-induced cardiomyopathy and uncontrolled permanent atrial fibrillation. Cardiac resynchronization therapy implantation and atrioventricular junction ablation were planned. DF-1 single chamber defibrillator was connected to lead's defibrillation and a lead destinated to left bundle branch area pacing. This system leads to reduce costs by one-third, improve battery longevity, and provide a more physiological pacing.

Atrioventricular block during atrioventricular nodal reentrant tachycardia is not always benign.

A 70-year-old Japanese woman was admitted to our hospital because of transient second degree atrioventricular (AV) block. An electrophysiologic study (EPS) was performed, and Mobitz type II infra-Hisian block during atrial pacing at a rate of 130/min was noted. An AV nodal reentrant tachycardia (AVNRT) was induced by ventricular pacing at a rate of 180/min, and 2:1-3:1 infra-Hisian block during AVNRT was observed. The AV block and AVNRT rarely occurred in the clinical setting, and the patient did not complain of any symptoms related to these arrhythmias. Therefore, the patient refused permanen

[Radiofrequency ablation of atrioventricular conduction during the 5th month of pregnancy].

The authors report the case of radiofrequency ablation of atrioventricular conduction in a 5 months pregnant woman who had hypertrophic cardiomyopathy. The indication of this procedure was a poorly tolerated resistant supraventricular tachycardia with foetal distress. A dual-chamber rate-assisted pacemaker programmed in the VVIR mode was implanted during the same procedure normally, with normal delivery of a healthy child at 8 months' gestation.

Intraventricular conduction disturbances after correction of tetralogy of Fallot: can bifascicular and trifascicular block be diagnosed from the surface ECG?

To determine the origin of conduction disturbances commonly seen in the ECG after correction of Fallot's tetralogy, 36 children underwent electrophysiologic studies; these included endocardial recordings of right ventricular (RV) and left ventricular (LV) apical activation and His-bundle recordings. The surface ECG showed a pattern of complete right bundle branch block (RBBB) in 22, and RBBB with left-axis deviation (LAD)--so-called bifascicular block--in nine; the remaining five had RBBB, LAD, and a long PR interval, often considered as indicating trifascicular block. Eight of 22 patients wit

Functional characteristics of retrograde conduction in a pacing model of "endless loop tachycardia".

A pacing model was designed that stimulated "endless loop tachycardia," a complication found in the new generation of DDD (atrioventricular [AV] universal) pacemakers. The functional characteristics of the train of ventricular impulses simulating endless loop tachycardia were studied during both AV sequential pacing and basic ventricular drive. AV sequential pacing, by causing a decrease in ventriculoatrial (VA) conduction time of the first beat of the endless loop tachycardia, was associated with a decrease in the cycle length at which VA block occurred in 9 of 12 patients. The site of block

Retrograde (ventriculoatrial) conduction.

Interest in retrograde VA conduction has been renewed with the advent of tachycardias induced by physiologic pacemakers with atrial sensing capabilities. Accurate representation of ventriculoatrial conduction requires detailed electrophysiologic analysis during sinus rhythm, during tachycardias whether or not associated with accessory pathways, and during ventricular pacing studies. Retrograde conduction should be assessed in patients considered for implantation of atrial sensing and tracking pacemakers (VAT, VDD, DDD), until technologic advances overcome the problems of endless loop tachycard

Spontaneous manifestation of dual AV nodal pathways resulting in complex patterns of AV conduction.

Dual intranodal pathways are not uncommonly demonstrated at electophysiological study especially in patients with intranodal re-entrant tachycardias. This type of tachycardia is the most common spontaneous manifestation (albeit indirect) of dual AV nodal pathways. Other forms of spontaneous expression of dual atrio-His conduction are rare. In this report we describe a patient who exhibited complex atrioventricular conduction patterns over two intranodal pathways.
