Abnormalities of the myocytes in ischaemic cardiomyopathy.

Acute myocardial ischemia and subsequent reperfusion result in biochemical and ionic changes in cardiac myocytes which cause contracture of the muscle and a reduced contractile force. Whether changes observed in single myocytes isolated from ischaemic ventricles are a direct consequence of the acute insult, or develop more slowly due to subsequent alterations in load and neurohumoural environment, is controversial. Myocytes from ischemic hearts have a similar contraction amplitude to those from non-failing hearts at physiological or maximally activating levels of ca2+. This could be partly due

Heart transplantation for dilated cardiomyopathy.

Between 1988 and 1994, 23 patients underwent heart transplantation for dilated cardiomyopathy. The age of the 13 boys and 10 girls was from 8 months to 16 years (mean 7.1 years). Selection criteria included failure to thrive despite maximal antifailure treatment and/or intravenous inotrope dependence. The aetiology of cardiomyopathy was idiopathic (n = 13), congenital (n = 3), anthracycline induced (n = 4), Barth's syndrome (n = 1), and maternal systemic lupus erythematosus (n = 2). The waiting period of heart transplantation ranged from one day to 147 days (mean 22 days). Maintenance immunosu

Irreversible cardiomyopathy due to thyrotoxicosis.

We report 4 adult patients with thyrotoxicosis accompanied by irreversible low-output heart failure. Each patient showed elevated plasma levels of thyroid hormone and prolonged low-output heart failure even after thyroid function returned to normal. Specimens of the right ventricular myocardium stained with anti-beta myosin heavy-chain MAb showed a normal staining pattern with a predominance of the beta-form. Our observations suggest that thyrotoxicosis may be one possible cause of irreversible cardiomyopathy.

Management of arrhythmias in hypertrophic cardiomyopathy.

In the management of hypertrophic cardiomyopathy the goals should be the control of symptoms, and the identification and treatment of those at high risk. Arrhythmias, particularly atrial fibrillation and nonsustained ventricular tachycardia, are common in adult patients with hypertrophic cardiomyopathy. Atrial fibrillation has long been thought to herald an ominous prognosis, but this is probably not the case, and in the majority of patients atrial fibrillation can be controlled without accelerated symptomatic deterioration. Uncontrolled observations indicate that low-dose amiodarone may be th

Clinical genetics of dilated cardiomyopathy.

The pathogenesis of dilated cardiomyopathy (DCM) remains controversial. Over the last few years there has been a gradual accumulation of evidence suggesting that familial forms of DCM are common and are most likely due to the transmission of a rare autosomal dominant gene. Despite these observations absolute proof that familial DCM is genetic is not available and there are many unanswered questions about the molecular basis of familial DCM. Is familial DCM caused by a single rare genetic defect or are there a number of genes involved which can be transmitted by different mechanisms? What facto

Enteroviral RNA in dilated cardiomyopathy.

The exact cause of dilated cardiomyopathy (DCM) remains uncertain. However, a possibility of transition from coxsackievirus-infected myocarditis to DCM has been suspected. We investigated the role of enteroviral infection in the pathogenesis of DCM. The nested reverse transcriptase polymerase chain reaction (nRT-PCR) was used to detect enteroviral RNA in 45 endomyocardial biopsy tissues obtained from 35 patients with DCM and 10 patients (controls) with other non-infectious cardiac diseases. Enteroviral RNA was detected in 17 (49%) of the 35 patients with DCM. The progression to cardiac failure

[Atypical forms of hypertrophic cardiomyopathy].

UNLABELLED: Hypertrophic cardiomyopathy is characterized by a diverse clinical and morphological spectrum. In this report we describe five patients with atypical hypertrophic cardiomyopathy and significantly dilated atria. Diagnostic difficulties are stressed. All patients underwent clinical examination, noninvasive studies including 2-D echocardiogram, cardiac catheterization. In three patients endomyocardial biopsy was taken and examined by light microscopy. All but one patient were men, ranging in age from 28 years to 40 years at initial examination. In all patients disproportion between mi

Peripartum cardiomyopathy: a case report.

Peripartum cardiomyopathy is characterized by congestive left ventricular failure appearing during the last trimester of pregnancy or during the 6 months following delivery, without any specific etiology or pre-existing cardiac disease. The present report describes the case history of a woman presenting with severe cardiac failure immediately after the delivery of twins.

Mitochondrial myopathy and cardiomyopathy in siblings.

Two siblings with infantile lactic acidosis and mitochondrial myopathy are described. The first child, a girl, died at 5 months of age from severe lactic acidosis after about 3 weeks of progressive muscular hypotonia. The younger brother had congenital lactic acidosis but no other symptoms until 6 months of age when progressive muscle weakness appeared. Treatment with dichloroacetate lowered the serum lactic acid level but did not affect his clinical condition. At 13 months of age, cardiomyopathy was diagnosed and he died at the age of 29 months of circulatory failure. Both children had mitoch

Cardiomyopathy in phaeochromocytoma: a case report.

This case report describes the anaesthetic management of a patient with bilateral phaeochromocytoma and cardiomyopathy. The hypertension and supraventricular tachycardia commonly seen during manipulation of this tumour was controlled by administration of sodium nitroprusside and verapamil. Verapamil allowed effective management of supraventricular rhythm disturbances and arterial blood pressure, and cardiac index remained unchanged during and after phaeochromocytoma removal. As the haemodynamic side-effects of the calcium blocking agent are readily reversed by intravenous calcium chloride, it

Risk factors in alcoholic cardiomyopathy.

Although a relationship has been suggested between abstinence from alcohol and improvement in left ventricular (LV) function, no long-term studies in large groups of patients have been done to confirm this impression or to demonstrate an effect on survival. To address these questions, the authors analyzed the outcome in 105 male patients with alcoholic cardiomyopathy and 64 control male patients with nonalcoholic dilated cardiomyopathy. Survival data were available for all patients. The correlates of survival were assessed via the Cox proportional hazards model. Variables considered were age,

Hypertrophic cardiomyopathy in infancy and childhood.

Hypertrophic cardiomyopathy (HCM) presented in 10 children under 2 years of age (group 1) and in 5 between 3 and 8 years (group 2). The clinical, ECG, chest radiographic and echocardiographic features are reviewed and prognosis over a mean follow-up period of 3.5 years is reported. Patients in group 1 had more symptoms and 7 had evidence of heart failure at some stage; all had ECG abnormalities. Group 2 patients presented with murmurs and only 1 had heart failure. Medical management of these patients is discussed and the importance of accurate diagnosis stressed, since HCM may have a poor prog

Hypertrophic cardiomyopathy and thyroid diseases.

The incidence ratio of thyrotoxicosis among 14 patients with thyroid diseases associated with hypertrophic cardiomyopathy was compared with that in 7686 patients with thyroid diseases, to find the possible correlation between thyrotoxicosis and hypertrophic cardiomyopathy. Thyrotoxicosis did not predominate over other thyroid diseases in the 14 patients with hypertrophic cardiomyopathy. Thus, the occurrence of hypertrophic cardiomyopathy does not appear to be associated with thyrotoxicosis.
