Parasellar Meningiomas.

Parasellar spaces remain particularly singular, comprising the most important neurovascular structures such as the internal carotid artery and optic, oculomotor, and trigeminal nerves. Meningiomas are one of the most frequent tumors arising from parasellar spaces. In this location, meningiomas remain mostly benign tumors with WHO grade I and a meningothelial subtype. Progestin intake should be investigated and leads mostly to conservative strategies. In the case of benign nonsymptomatic tumors, observation should be proposed. Tumor growth will lead to the proposition of surgery or radiosurgery

Intraoperative Management of an Enlarged Suprameatal Tubercle During Microvascular Decompression of the Trigeminal Nerve, Surgical and Anatomical Description: 2-Dimensional Operative Video.

The suprameatal tubercle is a variable prominence of the posterior aspect of the petrous part of the temporal bone located above the internal acoustic meatus. An enlarged suprameatal tubercle (EST) may present an obstacle during posterior fossa operations, including microvascular decompression (MVD).  In this video we present the case of a 55-yr-old woman with 2 yr of medically refractory left V2-3 typical trigeminal neuralgia. Magnetic resonance imaging (MRI) was suspicious for a compressive superior cerebellar artery (SCA) loop, and negative for any other pathological findings. A left retros

Individual variations of the superior petrosal vein complex and their microsurgical relevance in 50 cases of trigeminal microvascular decompression.

BACKGROUND: We investigated the understudied anatomical variations of the superior petrosal vein (SPV) complex (SPVC), which may play some role in dictating the individual complication risk following SPVC injury. METHODS: Microvascular decompressions of the trigeminal nerve between September 2012 and July 2016. All operations utilized an SPVC preserving technique. Preoperative balanced fast field echo (bFFE) magnetic resonance imaging, or equivalent sequences, and operative videos were studied for individual SPVC anatomical features. RESULTS: Applied imaging and operative SPVC anatomy were d

Endoscopic endonasal approach for trigeminal schwannomas: our experience of 39 patients in 10 years.

BACKGROUND: The anatomical locations involved in trigeminal schwannomas (TSs) are quite complex. The endoscopic endonasal approach provides a minimal damage access corridor to both anterior and middle skull base for surgery. Given the nerve function recovery and postoperative neurological deterioration varied in different reports, the author demonstrates his surgery tips and the functional outcomes under endoscopic surgery in one single institution. METHODS: A retrospective review of patients with TSs was undertaken to assess the outcome of endoscopic surgery from 2006 to 2016. Clinical featu

Helminth Fauna Associated with Three Neotropical Bat Species (Chiroptera: Mormoopidae) in Veracruz, México.

Bats are recognized as potential hosts of pathogens exploiting the food chain to reach them as definitive hosts. However, very little is known about their endoparasites, especially for Neotropical bats. In this study, we assessed the helminth fauna associated with 3 insectivorous bat species roosting in the same single hot cave in central Veracruz, México: Mormoops megalophylla, Pteronotus davyi, and Pteronotus personatus. During a period of 1 yr (April 2007-2008), 135 mormoopid bats in total were collected and examined for helminths. Six parasite species representing 3 types of intestinal hel

OCA2 splice site variant in German Spitz dogs with oculocutaneous albinism.

We investigated a German Spitz family where the mating of a black male to a white female had yielded three puppies with an unexpected light brown coat color, lightly pigmented lips and noses, and blue eyes. Combined linkage and homozygosity analysis based on a fully penetrant monogenic autosomal recessive mode of inheritance identified a critical interval of 15 Mb on chromosome 3. We obtained whole genome sequence data from one affected dog, three wolves, and 188 control dogs. Filtering for private variants revealed a single variant with predicted high impact in the critical interval in LOC100

Diversity, Host Specialization, and Geographic Structure of Filarial Nematodes Infecting Malagasy Bats.

We investigated filarial infection in Malagasy bats to gain insights into the diversity of these parasites and explore the factors shaping their distribution. Samples were obtained from 947 individual bats collected from 52 sites on Madagascar and representing 31 of the 44 species currently recognized on the island. Samples were screened for the presence of micro- and macro-parasites through both molecular and morphological approaches. Phylogenetic analyses showed that filarial diversity in Malagasy bats formed three main groups, the most common represented by Litomosa spp. infecting Miniopter

Usefulness of flat detector CT (FD-CT) with biplane fluoroscopy for complication avoidance during radiofrequency thermal rhizotomy for trigeminal neuralgia.

INTRODUCTION: Trigeminal neuralgia (TN) is characterized by episodes of shooting pain in the areas innervated by one or more divisions of the trigeminal nerve. The initial treatment of TN is with drugs but the increased frequency and intensity of the neuralgic episodes often force the patient to seek alternative therapies. Microvascular decompression (MVD) and radiofrequency thermal lesioning of trigeminal rootlets (RFTR) offer close to the best results for TN. MVD has the disadvantage of being an open surgical procedure with its attendant risks and longer hospital stay, whereas RFTR is a shor

Extended subtemporal transtentorial approach to the anterior incisural space and upper clival region: experience with posterior circulation aneurysms.

BACKGROUND: Although most posterior circulation aneurysms are currently treated by endovascular means, some are not amenable to this treatment modality. The narrow working window afforded by the anterolateral and lateral surgical approaches often translates into suboptimal visualization and limited maneuverability. OBJECTIVE: We present a modified technique of tentorial incision and reflection that optimizes the exposure achieved with the traditional subtemporal approach and report our clinical experience in a series of posterior circulation aneurysms. METHODS: Retrospective review of patien

Endoscopic endonasal transpterygoid approaches: anatomical landmarks for planning the surgical corridor.

OBJECTIVES/HYPOTHESIS: Endoscopic endonasal transpterygoid approaches (EETA) use the pneumatization of the sinonasal corridor to control lesions of the middle and posterior skull base. These surgical areas are complex and the required surgical corridors are difficult to predict. AIM: Define anatomical landmarks for the preoperative planning of EETAs. STUDY DESIGN: Anatomical study. METHODS: We reviewed images from high-resolution maxillofacial CT scans with (0.6-mm axial slice acquisition). Cephalometric measurements were obtained using Kodak Carestream Image Software (Rochester, NY). RESU

Contrast enhancement of the anterior eye segment and subarachnoid space: detection in the normal state by heavily T2-weighted 3D FLAIR.

PURPOSE: Fluid-attenuated inversion recovery (FLAIR) has been reported more sensitive than T(1)-weighted images in detecting low concentration gadolinium-based contrast media (GBCM) in fluid, and heavily T(2)-weighted (hT(2)W) 3-dimensional (3D) FLAIR has recently been reported even more sensitive than conventional 3D FLAIR. We investigated whether high signal of the anterior eye segment (AES) and subarachnoid space (SAS) in various locations as well as cerebrospinal fluid (CSF) in cisterns and ventricles can be detected on hT(2)W 3D FLAIR images obtained 4 hours after intravenous administrati

A new early Pleistocene hominin mandible from Atapuerca-TD6, Spain.

We present the description of a new mandibular specimen, ATD6-113, recovered in 2006 from the TD6 level of the Gran Dolina cave site in Sierra de Atapuerca, northern Spain. A detailed study of the lithostratigraphy of the top sequence of this level, the section from where all human remains have been recovered so far, is also presented. We have observed that the hominin stratum, previously defined as Aurora Stratum, represents a condensed deposit of at least six layers, which could not be distinguished in the test pit made in 1994-95. Therefore, the human fossil remains were probably deposited

PCR-induced sequence alterations hamper the typing of prehistoric bone samples for diagnostic achondroplasia mutations.

Achondroplasia (ACH) is a skeletal disorder (MIM100800) with an autosomal dominant Mendelian inheritance and complete penetrance. Here we report the screening of ancient bone samples for diagnostic ACH mutations. The diagnostic G-->A transition in the FGFR3 gene at cDNA position 1138 was detected in cloned polymerase chain reaction (PCR) products obtained from the dry mummy of the Semerchet tomb, Egypt (first dynasty, approximately 4,890-5,050 BP [before present]), and from an individual from Kirchheim, Germany (Merovingian period, approximately 1,300-1,500 BP), both of which had short stature

Partial labyrinthectomy petrous apicectomy approach to the petroclival region: an anatomic and technical study.

OBJECTIVE: The petroclival region generally is thought to be an inaccessible area in the intracranial compartment. A number of ways of reaching this area during surgery have been described, including the presigmoid petrosal approach. The partial labyrinthectomy petrous apicectomy approach is a relatively new approach to this region and is a variant of the presigmoid petrosal approach. This study aims to demonstrate the technique and the microsurgical anatomy of the partial labyrinthectomy petrous apicectomy approach and to provide a quantitative study of its exposure to compare it with other c
