Solitary plasmacytoma of the upper cervical spine: therapeutic considerations.

Solitary plasmacytomas are localized plasma cell malignancies involving bone marrow (solitary bone plasmacytoma, SBP) or extramedullary tissues (solitary extramedullary plasmacytoma, SEP). The upper cervical spine involvement by SBP is a rare and more challenging condition. The authors describe a patient with solitary plasmacytoma involving all the elements of C2 vertebra. Severe neck pain was the only manifestation of disease. The patient underwent an occipito-cervical stabilization with a partial decompression through a single posterior approach. The authors stress the importance of early di

Acute traumatic atlas fractures: management and long term outcome.

Fractures of the 1st cervical vertebra (C1) represent 7% of all acute cervical spine fractures. Isolated atlas fractures are most commonly bilateral or multiple fractures through the ring of C1. Frequently (44% of cases), the atlas will be fractured in combination with the axis. Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute atlas fractures is reviewed. Nonoperative external immobilization was used in 53 patients (with 1 failure)

Cervical injuries under flexion and compression loading.

Cervical spine segmental tests were performed to determine the specific patterns of initial cervical injury in response to loading just beyond the point of structural failure. Well-defined combinations of flexion rotation and compression translation were applied to segments with varying degrees of disc and facet degeneration. Twelve cervical spine specimens (from human cadavers ages 52-91 years), each consisting of three vertebrae (two motion segments) from the middle (C2-C5) or lower (C5-T1) regions, were subjected to pure flexion rotation (seven specimens) or to combined flexion rotation and

Iliac end-to-end structural autograft for complex failure after occipital cervical fusion: illustrative case.

BACKGROUND: Occipital cervical fusion (OCF) is an effective method for immobilizing the cranial cervical junction (CCJ). OCF failure is uncommon but can be associated with kyphosis after laminectomy. Revision OCF after long-segment cervical laminectomy is challenging, as grafting options are more limited. OBSERVATIONS: The patient was a 53-year-old man with pseudarthrosis, posterior displacement of the occipital plate, a chronic type II odontoid fracture with anterior subluxation of the fractured dens, a fracture of the C1 vertebrae with anterior displacement, and a previous 6- to 7-cm lamine

Cervicogenic Dizziness in an 11-Year-Old Girl: A Case Report.

Cervicogenic dizziness (CGD) is a syndrome of neck pain accompanied by a false sensation of unsteadiness and dizziness due to neck pathology. An 11-year-old girl presented with neck pain and dizziness for four months. According to the patient's statement, her complaints were likely related to the prolonged smartphone use for texting. Sagittal radiograph showed cervical kyphosis, anterior wedging of several vertebrae, and mild anterolisthesis of C2 on C3 and C3 on C4. These findings might be present in as physiological variants in children. However, continuous static stress in the minor variant

Outcomes following surgical management of cervical chordoma: A review of published case reports and case series.

AIM: This review aimed to summarize the clinical outcomes in relation to tumor resection margins of cervical chordomas. METHODS: Studies that described any surgical intervention for cervical chordoma were identified. Cervical chordomas with cranial or spinal extension, purely retropharyngeal chordomas or where resection type was not reported, were excluded from the study. RESULTS: Seventy-six articles were obtained and these reported a total of 195 patients. Seventy-six percent cervical chordoma patients had intralesional resection with adjuvant radiotherapy. Ninety-two percent chordoma recu

Phenotypic integration in the carnivoran backbone and the evolution of functional differentiation in metameric structures.

Explaining the origin and evolution of a vertebral column with anatomically distinct regions that characterizes the tetrapod body plan provides understanding of how metameric structures become repeated and how they acquire the ability to perform different functions. However, despite many decades of inquiry, the advantages and costs of vertebral column regionalization in anatomically distinct blocks, their functional specialization, and how they channel new evolutionary outcomes are poorly understood. Here, we investigate morphological integration (and how this integration is structured [modula

3D geometry and quantitative variation of the cervico-thoracic region in Crocodylia.

This study aims to interpret the axial patterning of the crocodylian neck, and to find a potential taxonomic signal that corresponds to vertebral position. Morphological variation in the cervico-thoracic vertebrae is compared in fifteen different crocodylian species using 3D geometric morphometric methods. Multivariate analysis indicated that the pattern of intracolumnar variation was a gradual change in shape of the vertebral series (at the parapophyses, diapophyses, prezygapohyses, and postzygapohyses), in the cervical (C3 to C9) and dorsal (D1-D2) regions which was quite conservative among

Comparative anatomy and 3D geometric morphometrics of the El Sidrón atlases (C1).

The first cervical vertebra (atlas, C1) is an important element of the vertebral column because it connects the cranial base with the cervical column, thus helping to maintain head posture and contributing to neck mobility. However, few atlases are preserved in the fossil record because of the fragility of this vertebra. Consequently, only eight well-preserved atlases from adult Neandertals have been recovered and described. Here, we present nine new atlas remains from the El Sidrón Neandertal site (Asturias, Spain), two of which (SD-1643 and SD-1605/1595) are sufficiently well preserved to al

[Excisional surgery of hypopharyngeal cancer and primary reconstruction of the defect].

A combination of total laryngopharyngectomy and primary reconstruction of the cervical esophagus is the operation of choice in the treatment of advanced hypopharyngeal cancer. For the excision, the most important task for the surgeon is to estimate the adequate safety margins superiorly and inferiorly. In order to ensure safer margins, animal experiments on lymph-flow in the submucosal layer of the hypopharyngeal cavity that is considered to be responsible for invisible submucosal invasion of cancer, were undertaken. Based on the results of both these experiments and clinical observations of m

[Acute torticollis and its roentgen analysis].

It has hitherto been assumed without exception that A-P films of acute torticollis demonstrate neither malposture nor subluxation or (worse still) luxation of the atlas. In contrast, there is ventral subluxation of the atlas in all cases, although in about one-half of them this only become manifest in ventral flexion, with a tendency to form an angle between the odontoid process and the first cervical vertebra. In conformity with the head posture in acute torticollis, X-ray findings in the head joints are normal on A-P films. Fundamental to acute torticollis is that when the head is held in ce

Prevertebral hematoma in cervical spine injury: incidence and etiologic significance.

In a consecutive series of 30 hospitalized cervical injury patients, 18 proved to have widening of the prevertebral soft tissue space due to hematoma. The upper limit of normal width in 50 noninjury patients was used as a reference. Hematoma almost exclusively manifested at C1-C4 and proved to be closely related to fractures of anterior elements of the spine. Relatively small hematomas were encountered in odontoid fractures and compression fractures of vertebral bodies; relatively large hematoma in disruptive hyperextension injury. Large hematomas are explained by extensive anterior ligamentou

Low skeletal muscle mass is predictive of dose-limiting toxicities in head and neck cancer patients undergoing low-dose weekly cisplatin chemoradiotherapy.

BACKGROUND: The dose-limiting effect of CT-assessed low skeletal muscle mass (LSMM) measured at the level of the third cervical vertebra has been found in head and neck cancer patients receiving high-dose cisplatin chemoradiotherapy. The aim of this study was to investigate the predictive factors for dose-limiting toxicities (DLTs) using low-dose weekly chemoradiotherapy. MATERIALS AND METHODS: Head and neck cancer patients receiving definite chemoradiotherapy with weekly 40 mg/m2 body surface area (BSA) cisplatin or paclitaxel 45 mg/m2 BSA and carboplatin AUC2 were consecutively included and

Sport Preparticipation Screening for Asymptomatic Atlantoaxial Instability in Patients With Down Syndrome.

Down syndrome (DS) is a clinical syndrome comprising typical facial features and various physical and intellectual disabilities due to extra genetic material on chromosome 21, with one in every 1000 babies born in the United Kingdom affected. Patients with Down syndrome are at risk of atlantoaxial instability (AAI). Although AAI can occur in other conditions, such as rheumatoid arthritis, this position statement deals specifically with patients with DS and asymptomatic AAI. Atlantoaxial instability, also referred to as atlantoaxial subluxation, is defined as increased movement between the firs

Morphological variation of carotid artery bifurcation level in digital angiography.

Knowing of the level of carotid artery bifurcation (CB) is important for vascular surgery in the neck, radical neck dissections, carotid sinus baroreceptor stimulation, catheterisations, and aneurysms. The aim of this study was to determine the CB level in relation with the cervical vertebral levels, compare them on the right and the left sides, and investigate the relation of CB level with the length of neck. In this study, 100 conventional carotid angiographies were performed. The CB level was determined in relation with 10 different levels which were the levels of the cervical vertebrae and…

Cervical dystonia in Bechterev disease resulting in atlantoaxial rotatory subluxation and cranio-cervical osseous fusion.

STUDY DESIGN: We report on a 46-year-old man with atlantoaxial rotatory subluxation and cranio-cervical osseous fusion secondary to cervical dystonia (CD) and concomitant ankylosing spondylitis (AS). OBJECTIVE: To describe treatment of CD under these unusual circumstances. SUMMARY OF BACKGROUND DATA: CD may cause several complications due to enhanced spinal degeneration. Primary and secondary nonresponders to conservative treatment may benefit from neurosurgical procedures including denervation procedures or functional stereotactic neurosurgery. However, if spinal fixation has occurred, usua

Spontaneous synchronized neural activity in decerebrate gallamine-paralysed cats.

In decerebrate cats paralysed with gallamine, over a period of several days there develops a remarkable synchronization of discharge in widely different motor nerves throughout the body, including intercostal nerves and limb nerves. These discharges are also in synchrony with slow waves approximately 100 ms in duration in the inferior olive. The slow waves and discharges are at first irregular and only weakly synchronized, but become increasingly strongly synchronized and by about the fourth day exhibit a strong 6-8 Hz rhythm. The degree of synchronization is greater the lower the end-tidal CO

The evolution of prophylactic neck irradiation in nasopharyngeal carcinoma: Changing concepts and irradiation ranges.

The approach to prophylactic neck irradiation in nasopharyngeal carcinoma (NPC) has undergone significant changes. For decades, prophylactic … or boundary of the neck CTV is determined either by the distance from the lowest positive LN or two cervical vertebrae below the lowest positive LN, facilitating more individualized CTV delineation and prophylactic neck irradiation. Additionally, omitting lower-risk neck lymphatic drainage CTVs and irradiating only visible LNs in these areas are also being explored. …
