Emergency obstetric and newborn care: A priority measure to achieve rapid decline in maternal and newborn mortality in sub-Saharan Africa.

For more than three decades, sub-Saharan Africa has invested heavily in the five WHO pillars of safe motherhood.1,2 Governments, development partners, professional associations and civil society organizations have promoted family planning, strengthened antenatal care, expanded skilled birth attendance, improved postnatal care, and, where permitted by law, advocated for safe abortion services. These interventions have undoubtedly contributed to improved maternal and newborn health and have saved countless lives. Yet the region continues to account for approximately 70% of global maternal deaths

Efforts towards a COVID-19 vaccine.

To many scientists and political authorities, the development of a vaccine against Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) will be the way to restore normality to civil life in this time of a devastating pandemic. Expectations for a vaccine are high while the case numbers continue to rise. As of mid-August 2020, more than 20 million people have been infected and more than 760 000 lives have been lost worldwide. The threat of this virus to health, the economy and to society is so great that the wish for a fast track vaccine is understandable, but how realistic is it? This s

A New Commitment Pathway for Offenders With Serious Mental Illness: Expedited Diversion to Court-Ordered Treatment.

The authors propose a new form of civil commitment that would benefit individuals with serious mental illness involved with the criminal justice system. This population has complex needs rooted in comorbid conditions, alienation from treatment and support systems, and poor access to care. Although many dollars are spent on costly assessments and hospitalization of jail detainees with serious mental illness to ensure that they are competent to stand trial, these detainees typically do not receive adequate services during incarceration or after release and recidivate at high rates. The proposed

Unintended Consequences of Screening for Ebola.

Ebola virus disease (EVD) reached the United States in September 2014, leading the Centers for Disease Control and Prevention to publish screening guidelines to identify patients with high-risk exposures at their first point of contact with the health care system. In West Africa, the burden of EVD is superimposed on the trauma of decades of civil war, violence, and poverty. Therefore, an important consideration in implementing screening procedures in the United States is the potential to unintentionally exacerbate posttraumatic stress disorder, or add additional stress from stigma and discrimi

Follicular lymphoma in situ presenting as dermatopathic lymphadenopathy.

Introduction. Follicular lymphoma in situ (FLIS) is a recently described entity with few cases recognized worldwide. To our knowledge, this is the first FLIS reported from West Africa. Case Presentation. We present the case of a 48-year-old civil servant with axillary lymphadenopathy discovered on routine mammography. On histology, a predominant reactive change with aggregates of melanophages was seen prompting a diagnosis of reactive lymphadenopathy. Immunohistochemistry done in our laboratory showed CD10 and Bcl-6 positive germinal centres with a small population of Bcl-2 positive germinal c

The clinical management of diabetic foot in the elderly and medico-legal implications.

Diabetic foot is a complex and challenging pathological state, characterized by high complexity of management, morbidity and mortality. The elderly present peculiar problems which interfere on one hand with the patient's compliance and on the other with their diagnostic-therapeutic management. Difficult clinical management may result in medico-legal problems, with criminal and civil consequences. In this context, the authors present a review of the literature, analysing aspects concerning the diagnosis and treatment of diabetic foot in the elderly which may turn out to be a source of professio

[Assessment of malingering and dissimulation in the examination of subjects with mental and psychosomatic disorders].

The consideration of response bias is essential in the assessment of mental and psychosomatic disorders. In forensic contexts, as compared to clinical evaluations, an elevated rate of response distortions must be expected. Assessment methods should be selected with respect to the risk of either malingering or defensiveness. The choice of assessment strategies and instruments depends on the legal context of the examination. It is recommended that the consistency and plausibility of data from different levels and sources should be systematically analysed. Psychological test results should be int

The impact of presumed consent laws and institutions on deceased organ donation.

This article purports to advance the literature on the impact of presumed consent laws on deceased donation rates by examining the interactions between a presumed consent legal regime and other customs and institutions, using data on health expenditure, death rates caused by cerebro vascular diseases, motor vehicle accidents and homicides, legislation, legal systems, family consent, civil rights and liberties and donor registry systems, for 24 countries over a 14-year period. Countries in which presumed consent is enacted exhibit significantly higher donation rates only if family consent is ro

"Dancing on eggs": Charles H. Bynum, racial politics, and the National Foundation for Infantile Paralysis, 1938-1954.

In 1938, President Franklin D. Roosevelt and his law partner Basil O'Connor formed the National Foundation for Infantile Paralysis (NFIP) to battle the viral disease poliomyelitis. Although the NFIP program was purported to be available for all Americans irrespective of "race, creed, or color," officials encountered numerous difficulties upholding this pledge in a nation divided by race. In 1944, NFIP officials hired educator Charles H. Bynum to head a new department of "Negro Activities." Between 1944 and 1954, Bynum negotiated the NFIP bureaucracy to educate officials and influence their nat

Patrick Browne (ca. 1720-1790), Irish physician, historian and Caribbean botanist: a brief biography with an account of his lost medical dissertations.

Patrick Browne (ca. 1720-1790), a native of County Mayo, Ireland, studied medicine in Paris, graduated from the University of Rheims in 1742, and briefly continued his studies at Leiden before practising as a doctor at St Thomas's Hospital, London. Subsequently, he lived for many years in the Caribbean, in Antigua, Jamaica, Saint Croix and Montserrat, but retired to County Mayo in 1771. Browne published The civil and natural history of Jamaica in 1756 a most significant work in terms of botanical nomenclature, which included new names for 104 genera, and he promised also a volume of medical es

Is there a relationship between illness and creativity? An analysis of the life and poetry of A. V. Koltsov.

Considerable interest has been shown in the relationship between illness and creativity. A study of life of A. V. Koltsov, a famous Russian poet of the nineteenth century, can make a valuable contribution to this subject because he was, to a most unusual extent, a self-made man. Unlike most "men of achievement", he did not come from an eminent family or a famous educational establishment. He was born in a small provincial town, on the fringe of the civilized world, and he spent less than eighteen months at school. If there is such a thing as "natural genius", Koltsov has a strong claim to be c

Nuclear Disaster Medical Response and Preparedness: Perceived Radiation Risk in Ongoing Decommissioning Work and Future Strategies.

This study aimed to assess the situations involving potential risks of over-exposure to radiation faced by workers during the decommissioning of the Fukushima Daiichi Nuclear Power Plant (FDNPP), and to prepare effective medical countermeasures. While deaths have occurred at regular intervals at the FDNPP site, they cannot be attributed to direct effects of radiation. Radiation exposure among decommissioning workers is strictly maintained within regulatory limits, and no deterministic or stochastic radiation effects have been observed. High-risk areas include the reactor buildings of Units 1-4

Reimagining Construction and Renovation of Health Care Facilities During Emergence from a Pandemic.

The built environment has been integral to response to the global pandemic of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). In particular, engineering controls to mitigate risk of exposure to SARS-CoV-2 and other newly emergent respiratory pathogens in the future will be important. Anticipating emergence from this pandemic, or at least adaptation given increasing administration of effective vaccines, and the safety of patients, personnel, and others in health care facilities remain the core goals. This article summarizes known risks and highlights prevention strategies for dail

LGBTQs in the city, queering urban space.

Since the 1960s, lesbian, gay, bisexual, transgender and queer (LGBTQ) culture has developed in big cities and metropolises everywhere (not only in the West, but also in Asia, Latin America and indeed Africa). This essay examines how cities provide the spatial conditions necessary for the formation of such emancipatory movements based on identity politics and strategies which transcend binary gender dualism. The starting point of this investigation is my thesis that only urban life enables LGBTQ individuals to live their lives fully, realize their (sexual) identities, and furthermore organize
