Academic ability and teamworking in medical students.

BACKGROUND: Teamworking is an essential skill for a doctor to develop in order to work effectively, and is required in the UK as part of the General Medical Council (GMC) Good Medical Practice guidance. Assessment of teamwork may be difficult, however, with medical school assessments being more commonly focused on knowledge and individual skills. We aim to explore the link between academic ability as measured at final medical examinations and teamworking. METHODS: All final-year medical students were asked to attend a simulation session in an immersive 22-bed simulated ward, which used a comb

Physician Decision-Making in the Setting of Advanced Illness: An Examination of Patient Disposition and Physician Religiousness.

CONTEXT: Little is known about patient and physician factors that affect decisions to pursue more or less aggressive treatment courses for patients with advanced illness. OBJECTIVES: This study sought to determine how patient age, patient disposition, and physician religiousness affect physician recommendations in the context of advanced illness. METHODS: A survey was mailed to a stratified random sample of U.S. physicians, which included three vignettes depicting advanced illness scenarios: 1) cancer, 2) heart failure, and 3) dementia with acute infection. One vignette included experimental

A decision tree model for chondral lesion management in the knee based on clinical and mathematical integration.

BACKGROUND: Focal chondral defects of the knee present a complex therapeutic challenge due to the heterogeneous nature of lesion characteristics and patient-specific factors. Despite multiple available treatment modalities, no universally accepted decision-making framework currently exists to guide individualized care. This study proposes and validates a clinically grounded, mathematically structured decision tree algorithm to stratify treatment options based on key anatomical, biomechanical, and demographic parameters. METHODS: A comprehensive literature review was performed to define eviden

Service-oriented subscription management of medical decision data in the intensive care unit.

OBJECTIVES: This paper addresses the design of a platform for the management of medical decision data in the ICU. Whenever new medical data from laboratories or monitors is available or at fixed times, the appropriate medical support services are activated and generate a medical alert or suggestion to the bedside terminal, the physician's PDA, smart phone or mailbox. Since future ICU systems will rely ever more on medical decision support, a generic and flexible subscription platform is of high importance. METHODS: Our platform is designed based on the principles of service-oriented architect

Moving Past "Avoid All Nuts": Individualizing Management of Children with Peanut/Tree Nut Allergies.

It has been common practice to tell patients with allergy to peanut or tree nuts to avoid all nuts. Evidence that unnecessary avoidance of peanuts and eggs is associated with increased risk for developing anaphylaxis to those foods has changed how allergists view previous recommendations to avoid foods that have not caused a reaction. In the absence of evidence, collaborative decision making between clinicians and families should be used to decide whether to avoid tree nuts and how to safely introduce tree nuts into the diet. This article discusses the options for introducing tree nuts to chil

Approach an appropriate decision on fetus with endocardial fibroelastosis in collaboration with cardiovascular profile score: A case report.

INTRODUCTION: Fetal endocardial fibroelastosis (EFE) is a kind of rare fetal cardiac malformation characterized by the diffuse thickening of the ventricular endocardium. The diagnosis of fetal EFE depends on the echocardiographic features which are still confused that how to make an appropriate pregnant decision due to the conflict between high prenatal mortality and acceptable prognosis once after birth. Here, we seriously built a 4-gradation recommendation system based on cardiovascular profile score (CVPS) to supply a prediction of clarified pregnant outcomes with EFE and provide a practica

A pathway to personalization of integrated treatment: informatics and decision science in psychiatric rehabilitation.

Personalization of treatment is a current strategic goal for improving health care. Integrated treatment approaches such as psychiatric rehabilitation benefit from personalization because they involve matching diverse arrays of treatment options to individually unique profiles of need. The need for personalization is evident in the heterogeneity of people with severe mental illness and in the findings of experimental psychopathology. One pathway to personalization lies in analysis of the judgments and decision making of human experts and other participants as they respond to complex circumstan

On the use of vignettes for eliciting test-selection strategies.

Decision-support systems often include a strategy for selecting tests in their field of application. This strategy in essence captures procedural knowledge and serves to provide support for the reasoning processes involved. Generally, a test-selection strategy is offered in which tests are selected sequentially. For our field of application, we noticed that such a strategy would be an oversimplification, and decided to acquire knowledge about the actual strategy used by the experts. To this end, we composed a method that comprised an unstructured interview to gain general insight in the test-s

Personal career decisions during medical training are not complicated, they are complex.

BACKGROUND: For medical training to be deemed successful, in addition to gaining the skills required to make appropriate clinical decisions, trainees must learn how to make good personal decisions. These decisions may affect satisfaction with career choice, work-life balance, and their ability to maintain/improve clinical performance over time-outcomes that can impact future wellness. Here, the authors introduce a decision-making framework with the goal of improving our understanding of personal decisions. METHODS: Stemming from the business world, the Cynefin framework describes five decisio

Bronchiectasis: Treatment decisions for pulmonary exacerbations and their prevention.

Interest in bronchiectasis has increased over the past two decades, as shown by the establishment of disease-specific registries in several countries, the publication of management guidelines and a growing number of clinical trials to address evidence gaps for treatment decisions. This review considers the evidence for defining and treating pulmonary exacerbations, the approaches for eradication of newly identified airway pathogens and the methods to prevent exacerbations through long-term treatments from a pragmatic practice-based perspective. Areas for future studies are also explored. Watch
