Lingering Pain and Discomfort Despite Splinting of an Ankle Fracture-Dislocation May Imply a Misdiagnosed Bosworth Injury: A Case Report.

Bosworth fracture-dislocation is a challenging injury of the ankle highlighted by the entrapment of the proximal fibula segment behind the distal tibia. The rarity of these fractures frequently leads to unsuccessful closed reduction attempts, due to soft tissue interposition and locked dislocation of the fibula. We present a Bosworth injury initially misdiagnosed as a common Weber B lateral malleolus fracture. Increasing patient discomfort warrants a thorough re-evaluation of X-rays to identify the complexity of a Bosworth injury and ensure effective surgical treatment.

Elbow dislocation with ipsilateral radial shaft fracture. An unusual outcome.

The authors describe an uncommon injury of the upper limb, namely, dislocation of the elbow with fracture of the ipsilateral radial shaft, in a young man. After closed reduction of the joint, the radial head was found to be subluxated, and when a nonanatomic internal fixation of the radial shaft was undertaken, the radial head became dislocated. Repeat operation with removal of the implants and internal fixation with correction of rotation and angulation at the fracture site and reconstruction of the annular ligament restored normal anatomy.

Ipsilateral olecranon and distal radius fracture: A case report.

INTRODUCTION: Concomitant ipsilateral olecranon and distal radius fracture are rare injuries. Their clinical presentation is unusual and investigation and management is poorly described. PRESENTATION OF CASE: We present a 55-year-old woman patient who fell off sustaining a concomitant distal radius and olecranon fracture in the same extremity. On examination, there was gross swelling of the proximal and distal forearm and no neurovascular deficit. Radiographs confirmed distal radius and olecranon fracture. Patient was treated with open reduction and anatomic locking plate for olecranon and a

Complete dislocation of the talus unaccompanied by fracture.

Total dislocation of the talus without a concomitant fracture is an extremely rare injury. Such a case was treated by immediate closed reduction and 6 weeks of cast immobilization. At the 9-year follow-up, the patient had a full range of ankle motion without any signs of avascular necrosis or arthritis.

Percutaneous K-wire buttress technique for displaced radial neck fracture.

Radial neck fractures are uncommon injuries in adults and more often found in children, where they account for 5-8.5% of elbow injuries. It is generally agreed that an angulation of less than 30 degrees is acceptable. However, anything greater than this should be improved in an attempt to restore normal anatomy and maximize the range of movement. We describe our management of a radial neck fracture in a young lady which was significantly angulated, resulting in a restriction of movements. Attempts at a closed reduction failed and hence we proceeded with percutaneous reduction and buttressing w

Do hip replacements improve outcomes for hip fracture patients?

BACKGROUND: Hip fracture is a common problem among older Americans. Two types of procedures are available for repairing hip fractures: hip replacement and open or closed reduction with or without internal fixation. The assumption has been that hip replacement produces better functional outcomes. Although that is the common wisdom, outcome studies evaluating hip replacement for treatment of hip fracture are few and have not clearly documented its superiority. OBJECTIVES: To compare outcomes of hip fracture patients who receive hip replacement versus another stabilizing procedure (open or close

Second metacarpal cortical percentage and psoas muscle index in predicting loss of reduction after non-operative treatment of distal radius fractures in adult women.

OBJECTIVES: This study aims to investigate whether second metacarpal cortical percentage (2MCP) and psoas muscle index (PMI) were associated with loss of reduction after closed reduction and cast immobilization. PATIENTS AND METHODS: A total of 91 adult female patients with distal radius fractures (DRFs) treated with closed reduction and cast immobilization between January 2014 and June 2025 were retrospectively analyzed. Patients with standardized serial radiographs and lumbar computed tomography (CT) performed within ± 3 months of injury were included. Clinical characteristics, AO/OTA fract

Fractures of the hand.

In managing any fractures of the hand, the basic principles of fracture management are the same whether treatment is closed or open. They are 1. Accurate reduction and stabilization of the fracture 2. Elevation and edema control 3. Mobilization of the unaffected joints 4. Early range of motion of the injured digit or digits when healing processes permit. Special situations and injuries require amendments to the basic principles. Open fractures require early and thorough wound debridement. Early fixation and serial wound debridement to assure a proper environment for bone or skin grafting are e

Predictive factors for re-displacement in diaphyseal forearm fractures in children-role of radiographic indices.

Background and purpose - Manipulation and cast immobilization is the primary management for diaphyseal forearm fractures in children, and re-displacement is the most common complication. We wanted (1) to analyze the incidence of re-displacement in a group of children treated with close reduction and casting; (2) to determine predictive factors such as demographics, mechanism of injury, affected bone, fracture pattern, degree of initial displacement and angulation, and reduction accuracy; and (3) to determine the prognostic effect of previously defined radiographic indices. Patients and methods

Assessment of Population Knowledge and Awareness of Nursemaid's Elbow in Children: A Cross-Sectional Study in Al-Baha Region, Saudi Arabia.

INTRODUCTION: Supracondylar fractures of the humerus are the most frequently encountered injuries around the elbow with predominantly occurring in age group 5-7 years, with a higher incidence among male. Research has indicated that for displaced humeral supracondylar fractures, both closed and open reduction with internal fixation using K-wires results in more stable outcomes. OBJECTIVES: The purpose of this study was to determine the functional and radiological outcome of operative management of supracondylar fracture humerus in children. MATERIALS AND METHODS: It was a prospective study fr

Arthroscopic Treatment of Luxatio Erecta Humeri Associated with Greater Tuberosity Fracture, Bankart Lesion, and Partial Rotator Cuff Tear: A Case Report.

BACKGROUND Luxatio erecta humeri (LEH) is a rare injury present in only 0.5% of shoulder dislocations. Much of the relevant literature is focused on the initial management and proper reduction techniques, although the prevalence of associated injuries can reach 80%. A case of LEH associated with greater tuberosity (GT) fracture and rotator cuff (RC) tear in a young laborer managed with closed reduction and arthroscopic repair of the labrum and rotator cuff is presented. CASE REPORT A 28-year-old man presented to our hospital with severe pain in his right shoulder after a high-impact motor vehi

The use of a Kirschner wire in the treatment of a comminuted le fort I fracture: a case report.

Simultaneous fracture of the maxilla and cervical vertebrae rarely occurs in bicycling accidents. The following case report describes a simple technique for closed reduction of a severely comminuted maxillary fracture with shattering of the dentoalveolar process. The combination of a rigid external distractor halo frame on the skull, a Kirschner wire through the maxilla, and an intermaxillary wire fixation resulted in stable vertical and sagittal correction of the fragmented maxilla with adequate access and minimal manipulation and without necessitating removal of the cervical collar.

Overcoming transfusion-refractory anemia: partial splenic embolization to facilitate orthopedic surgery in β-thalassemia.

β-thalassemia is a hereditary hemolytic disorder. Non-transfusion-dependent patients typically present with mild anemic symptoms under normal conditions, but may develop severe anemia during stress states. Moreover, treatment is often challenging due to concurrent hypersplenism. This article reports a case of a young male patient with non-transfusion-dependent β-thalassemia who was admitted to the hospital due to a closed fracture of the right femur. After admission, the patient developed severe anemia and showed a poor response to blood transfusion therapy. Considering the association with hyp

Iatrogenic axillary artery injury from humeral neck fracture repair. A rare and unusual event in which and aggressive strategy was mandatory to save a child limb.

Humeral neck fracture is rarely associated with injury of the nearby axillary artery and in the English literature only 29 such cases have been reported. An injury of the distal axillary artery secondary to reduction and fixation of a humeral neck fracture is a very rare and unusual complication, to the best of our knowledge has never been reported in literature. In this paper we report the case of entrapment of the distal axillary artery in the humerus rhyme fracture in a 9-year-old child who suffered a nighttime car-accident with her father. At admission, the child presented a left humeral n

Intra-articular injuries of the elbow: pitfalls of diagnosis and treatment.

Poor results in treating fractures and dislocations about the elbow may be avoided if the surgeon is aware of the possible injuries, examines good radiographs of both elbows, and treats the injury promptly and appropriately. A displaced fracture of the lateral or medial condyle of the humerus should be suspected if there is a flake fracture of the adjoining metaphysis; open reduction and internal fixation give better results than closed reduction. A shear fracture of the capitulum humeri can only be seen on a lateral radiograph; excision of the fragment, followed by mobilization, is sufficient

Monteggia fractures in adults.

From January 1978 to December 1989, 60 adult acute Monteggia fractures were treated and followed-up for at least one year (average 3 years 9 months). Ulnar fractures were treated with open reduction and internal fixation with a dynamic compression plate or a Rush pin, and 2 with minimal ulnar displacement were treated with closed reduction. Forty radial heads had a closed reduction, 6 required an open reduction with annular ligament repair or fascial reconstruction, and 14 were resected due to comminution. The results showed a 90% union rate with a 3.5 +/- 1.2 months union period. Functional e

Incidence of Talar Osteochondral Lesions After Acute Ankle Fracture: A Retrospective Analysis.

Recent literature suggests the majority of osteochondral lesions occur in the ankle joint. Previous studies have suggested that varying incidences of talar osteochondral lesions (OCLT) are associated with ankle fractures. The primary aim of our study was to investigate the incidence of osteochondral lesions associated with acute ankle fractures as observed on computed tomographic (CT) imaging. We also compared the rates of talar osteochondral lesions in patients who had ankle fractures with dislocation and closed manual reduction (CMR) prior to open reduction with internal fixation, to those w

Open Reduction and Pin Fixation of Pediatric Lateral Humeral Condylar Fractures.

BACKGROUND: Lateral condylar fractures of the humerus are the second most common elbow fracture seen in pediatrics, behind supracondylar humeral fractures. All practicing orthopaedic surgeons should be able to identify and treat these injuries appropriately, as the intra-articular and transphyseal nature of this fracture pattern contributes to the relatively high complication rate. Treatment has evolved with time as classification systems better characterize the different types of lateral condylar fractures, aiding in decision-making for management. This video article reviews the diagnostic fi

Perforation of the Knee Joint Following Antegrade Intramedullary Nailing of a Comminuted Femoral Diaphyseal Fracture: A Case Report.

A 63-year-old man with a comminuted spiral femoral shaft fracture was treated with closed reduction and internal fixation with a cephalomedullary nail. Two weeks postoperatively, one of the two static distal interlocking bolts began backing out and was removed. The nail ultimately migrated distally and perforated the knee joint at four months postoperatively. The patient was successfully treated with an exchange nail and percutaneous bone graft to the fracture site. A single static distal interlocking bolt may be inadequate to maintain length in a healing comminuted spiral femur shaft. Multipl

Combined Monteggia and Galeazzi fractures in a child's forearm. A case report.

We report the occurrence of a previously undescribed combined Monteggia and Galeazzi fracture pattern of the left forearm in an 8-year-old boy. Previous reports do not address the combined injury described in this article, which consists of a left radius shaft fracture with dislocation of the distal radioulnar joint, and a left ulna shaft fracture with anterior dislocation of the radiohumeral joint. An excellent result was obtained with closed reduction. Radiographic evaluation is crucial in these disorders for both diagnosis and for treatment decisions. A review of the literature is provided
