Coccidioidomycosis as a rare cause of pneumonia in non-endemic areas: a short exposure history should not be ignored.

Coccidioidomycosis is endemic in the south-western USA. Two cases of infection in travellers returning to Hong Kong are described. A previously healthy patient who had travelled to an endemic area for a short time was successfully treated with fluconazole. A second patient with comorbidities and more prolonged exposure had disseminated and eventually fatal disease, despite prolonged administration of anti-fungal agents. Although coccidioidomycosis is a rare disease in Hong Kong, it should always be considered when there is a relevant travel history. Even a short period of travel to an endemic

Coccidioidomycosis meningitis and syndrome of inappropriate antidiuretic hormone.

The syndrome of inappropriate antidiuretic hormone (SIADH) secretion has been well described in patients with meningeal spread from metastatic carcinomatosis and bacterial or mycobacterial infections. We describe a 39-year-old white man who was diagnosed with coccidioidomycosis pneumonia 7 years before presentation. He displayed evidence for meningitis with the onset of SIADH. We reviewed the diagnosis of coccidioidomycosis and radiological findings in the central nervous system. Last, we discussed the findings that led to the diagnosis of SIADH.

Diagnosis, treatment, and biosafety protection of imported highly pathogenic cutaneous fungal infection: a case-based study of cutaneous coccidioidomycosis.

Coccidioidomycosis is a significant fungal disease primarily endemic to the southern United States, northern Mexico, and South America. Coccidioidomycosis is non-endemic in China, with only approximately 47 cases reported over the past 63 years. Recognizing that increased global travel is altering regional disease epidemiology, the Chinese Health Commission has classified Coccidioides species as highly pathogenic pathogens. The diagnosis of coccidioidomycosis poses a major challenge in China for medical institutions without biosafety level 3 (BSL-3) laboratories. In November 2021, we diagnosed…

A point-source outbreak of Coccidioidomycosis among a highway construction crew.

Coccidioidomycosis is an infection caused by inhaling spores of the soil fungus Coccidioides immitis (hereafter termed Cocci). Cocci is endemic in certain areas of California. When soil containing the fungus is disturbed, as during earth-moving activities, respirable Cocci spores can become airborne and be inhaled by persons in the vicinity. This article describes a cluster of seven Cocciodioidomycosis cases among a highway construction crew that occurred in June/July 2008 in Kern County, CA, which is among the most highly endemic regions for Cocci in California. The exposures spanned no more …

Divergence in the approach to tumor necrosis factor α-inhibitor recipients with coccidioidomycosis.

BACKGROUND: Tumor necrosis factor α-inhibitors (TNFIs) have been associated with increased risk of certain fungal infections, including coccidioidomycosis. The optimal treatment approach to coccidioidomycosis in TNFI recipients is unknown. METHODS: We constructed an anonymous, voluntary survey for practicing pulmonary and infectious disease physicians in the state of Arizona regarding approach to TNFI patients with coccidioidomycosis. RESULTS: There is no current consensus on managing these patients. CONCLUSIONS: Further research is necessary to determine the optimal approach to TNFI recipi

[Pulmonary coccidioidomycosis:a case report and review of the literature].

OBJECTIVE: To improve the understanding of the clinical manifestations of pulmonary coccidioidomycosis. METHODS: A case of pulmonary coccidioidomycosis was reported, and the literature was reviewed. The epidemiologic, clinical and diagnostic aspects of coccidioidomycosis were discussed. RESULTS: A 74 year old male was admitted to the hospital because of physical examination revealing lung space occupying lesions for 9 months and cough for 2 weeks. Lung puncture biopsy was carried out and the diagnosis of cryptococcosis was established in another hospital. After 6 months' therapy with flucona

Coccidioidomycosis in California: regional outbreak, global diagnostic challenge.

Beginning in 1991, case reports of coccidioidomycosis in California increased dramatically, pursuant to a variety of natural and demographic factors. This highly infectious fungal disease with propensity to disseminate widely, mimic other conditions, and cause pathology at locations distant in place and time is readily treatable if recognized at an early stage. The concentration of military bases in endemic areas and the mobility of military personnel suggest a heightened potential for case presentations elsewhere and a need for elevated diagnostic suspicion on the part of military physicians

Perplexing pericarditis caused by coccidioidomycosis.

In the patient I have described, evaluation of chronic progressive pulmonary coccidioidomycosis presenting itself as cryptogenic pericarditis revealed an unsuspected large endobronchial coccidioidoma. At follow-up after 2 years of fluconazole therapy, the patient was asymptomatic, with a dramatic decrease in infiltrate but no change in CF titers. In endemic areas, coccidioidomycosis should be included in the differential diagnosis of idiopathic pericarditis, especially in high-risk patient populations. Additionally, airway obstruction or compromise might be an early finding in chronic coccidia

Treatment of coccidioidomycosis osteomyelitis with itraconazole in a horse. A brief report.

Itraconazole, a tricyclic azole effective against a number of deep mycotic diseases, was used to treat a Quarter Horse filly with coccidioidomycosis. The horse was almost normal after 90 days of treatment. Five months after discontinuing itraconazole treatment, the filly had severe neck pain and neurologic signs from recurrence of coccidioidomycosis and was treated with itraconazole for an additional 6 months. Her clinical condition improved to almost normal and the filly has remained normal for 2 years. There was no evidence of drug toxicity.

Antigenemia detected in human coccidioidomycosis.

To advance the serological diagnosis of coccidioidomycosis, a radioimmunoassay was developed to detect an antigen of Coccidioides immitis and evaluated with serum samples from patients and controls. The assay antigen was fractionated from a culture filtrate of C. immitis cells by Sephacryl S300 gel chromatography. The assay used preliminary acid-heat extraction to dissociate serum-bound antigen and staphylococcal protein A as an absorbent. Antigenemia was detected in five (56%) of nine patients with coccidioidomycosis. Three of four negative tests occurred with sera from patients who had alrea…

High-dose therapy with fluconazole > or = 800 mg day-1.

Fluconazole dosages greater than 800 mg day-1 have been reported in about 900 patients treated for candidemia, oropharyngeal candidiasis and cryptococcal meningitis in HIV-infected patients, and for initial therapy of endemic mycoses. In patients with life-threatening infections caused by Candida spp., Cryptococcus neoformans and Coccidioides immitis, results of a limited number of dose-finding trials with non-neutropenic and HIV-infected patients show dose-dependent responses. These study results indicate that higher daily doses of fluconazole than are currently approved for these indications…

Prevalence of chorioretinal scars associated with coccidioidomycosis.

A retrospective study was conducted to determine the prevalence and spectrum of chorioretinal involvement among individuals with previously documented systemic coccidioidomycosis. Fifty-four subjects were given a modified ocular examination, and their medical records were abstracted for clinical classification of the disease and its correlation with ocular findings. Five of 54 subjects with documented past infection with Coccidioides immitis had characteristic inactive peripheral chorioretinal scars. No relationship between the presence of scars and the extent of disease was evident; typical s…

Treatment of fungal meningitis with miconazole.

Twelve patients with fungal meningitis (ten cases were due to Coccidioides immitis, two were from Cryptococcus neoformans) were treated with brief courses of intravenous (IV) miconazole. Eleven patients, including patients with severe, chronic disease, had been treated unsuccessfully with amphotericin B. Four patients also received miconazole injected directly into the CSF. The drug was well tolerated by any route, with mild reversible side effects. After IV administration the miconazole concentration in the CSF rarely exceeded the minimal inhibitory concentration (MIC) of the infecting organi…
