Memantine (a N-methyl-D-aspartate receptor antagonist) in the treatment of neuropathic pain after amputation or surgery: a randomized, double-blinded, cross-over study.

UNLABELLED: Evidence has accumulated that the N:-methyl-D-aspartate receptor system plays a role in continuous and particularly, in stimulus-evoked pain after nerve injury. We examined, in a randomized, double-blinded, cross-over fashion, the analgesic effect of memantine (a N:-methyl-D-aspartate receptor antagonist) in a group of patients with chronic pain after surgery. We randomized 19 patients to receive either memantine or placebo in the first 5-wk treatment period. A washout period of 4 wks was followed by another 5-wk treatment period with the opposite drug. The dosage of drug was incre

Extracellular cold-inducible RNA-binding protein drives NLRP3-mediated pyroptosis in acute ischemic stroke.

Acute ischemic stroke (AIS) triggers rapid sterile inflammation that worsens secondary brain injury. Damage-associated molecular patterns (DAMPs) from stressed/necrotic cells activate inflammasomes and pyroptosis. Cold-inducible RNA-binding protein (CIRP), a stress-responsive DAMP linked to maladaptive inflammation, has unclear clinical relevance to ischemic stroke and mechanisms in post-ischemic neuroinflammation. We measured serum CIRP, neuronal injury markers (neuron-specific enolase [NSE] and S100 calcium-binding protein β [S100β]), and pyroptosis mediators (NOD-like receptor family pyrin

The Changing Landscape of Pediatric Burns in the United Kingdom: A 20-Year Epidemiological Study.

Burns are a leading cause of childhood injury. Analysis of pediatric burn epidemiology is essential for targeting educational and public health strategies, but there is limited available UK data, particularly traversing the COVID-19 pandemic. We hypothesize that the last 20 years have seen significant changes in injury patterns and service provision, with a marked effect from national lockdown measures. This study aims to describe the epidemiology of pediatric burns referrals and admissions within a pediatric burns unit in the United Kingdom over 20 years (2005-2024). A retrospective analysis

Tumor-phrenic nerve spatial relationship and intraoperative traction-induced injury during mediastinal tumor resection: implications for real-time protective strategies.

BACKGROUND: Phrenic nerve injury during mediastinal tumor resection can lead to significant postoperative diaphragmatic dysfunction. Current intraoperative protection techniques are imprecise and lack real-time feedback. We aimed to develop and validate a quantifiable, multimodal neuroprotective strategy. METHODS: A total of 370 patients undergoing minimally invasive mediastinal tumor resection were enrolled in a two-phase cohort. Tumor-nerve spatial parameters, traction force, and intraoperative temperature were measured. The intervention group received automated cold saline irrigation and p

Cardioprotective effect of Interleukin-11 against warm ischemia-reperfusion injury in a rat heart donor model.

Shortage of donor organs for heart transplantation is a worldwide problem. Donation after circulatory death (DCD) has been proposed to expand the donor pool. However, in contrast to the donation after brain death that undergoes immediate cold preservation, warm ischemia and subsequent reperfusion injury are inevitable in DCD. It has been reported that interleukin-11 (IL-11) mitigates ischemia-reperfusion injury in rodent models of myocardial infarction and donation after brain death heart transplantation. We hypothesized that IL-11 also offers benefit to warm ischemia in an experimental model

Neuroinflammation drives sex-dependent effects on pain and negative affect in a murine model of repeated mild traumatic brain injury.

The Center for Disease Control and Prevention estimates that 75% of reported cases of traumatic brain injury (TBI) are mild, where chronic pain and depression are 2 of the most common symptoms. In this study, we used a murine model of repeated mild TBI to characterize the associated pain hypersensitivity and affective-like behavior and to what extent microglial reactivity contributes to these behavioral phenotypes. Male and female C57BL/6J mice underwent sham or repeated mild traumatic brain injury (rmTBI) and were tested for up to 9 weeks postinjury, where an anti-inflammatory/neuroprotective

Enhanced Mitochondrial DNA Repair Resuscitates Transplantable Lungs Donated After Circulatory Death.

BACKGROUND: Transplantation of lungs procured after donation after circulatory death (DCD) is challenging because postmortem metabolic degradation may engender susceptibility to ischemia-reperfusion (IR) injury. Because oxidative mitochondrial DNA (mtDNA) damage has been linked to endothelial barrier disruption in other models of IR injury, here we used a fusion protein construct targeting the DNA repair 8-oxoguanine DNA glycosylase-1 (OGG1) to mitochondria (mtOGG1) to determine if enhanced repair of mtDNA damage attenuates endothelial barrier dysfunction after IR injury in a rat model of lung

Long-Term Outcomes of Digital Nerve Repair Accompanied by Digital Artery Injury in Flexor Zone 2.

Objectives  Better healing results of any tissue or area is closely linked with a well-blood supply in reconstructive surgery. Peripheric nerve healing is closely related to blood supply as well. We aimed to assess whether there was any difference between digital nerve healing with and without extrinsic blood supply. Methods  We assessed 48 patients with unilateral digital nerve injury at zone 2. Twenty-four of them had unrepairable arterial injury and other 24 had no arterial injury. The 24 patients in the "unrepaired artery group" (UA) and 24 patients in the "intact artery group" (IA) were c

The approach to frostbite in Turkey: A retrospective study.

Increased participation in outdoor activities and an epidemic of homelessness have caused the incidence of cold injuries in the civilian population of Turkey to rise dramatically during the past 20 years. Knowledge of treatment is crucial for emergency physicians in rural and urban areas. Recent developments have significantly advanced the understanding of the pathophysiology of hypothermic and frostbite injuries. The authors undertook a retrospective review of frostbite cases in the East Anatolia region of Turkey. The mean altitude of East Anatolia is 1600 m, and temperatures may be -35 degre

Causes of lamb morbidity and mortality in the Ethiopian highlands.

A study was conducted to investigate causes of lamb morbidity and mortality on farms and on-station at Debre Berhan during 1989 and 1990. It showed pneumonia (bacterial and/or verminous), starvation-mismothering exposure (SME) complex, gastrointestinal parasites, enteritis, abomasal impaction and physical injuries to be important health constraints on productivity. Neonatal mortalities were 51.5% and 46.3% on farms and on-station respectively and occurred owing to management problems such as SME, abomasal impaction and physical injuries. On the farms the lamb birth weight was 2.56 +/- 0.25 kg

Childhood burns in Zaria, Nigeria.

From 1971 to 1980, 429 children with burn injuries were admitted to the Ahmadu Bello University Hospital, Zaria. These were major burns in 275 patients, moderate in 82 and minor in 72. Fourteen of the patients were neonates, 102 infants, 228 were 5 years old or younger and 85 were older. Socioeconomic factors contributing to the injuries included the use of firewood for cooking at ground-level and for warming the house and body during the cold season; loose indigenous garments; thatch-roofed huts and the post-partum rituals of mud-bed heating and hot baths. Flame burns exceeded scalds with a s

Management of Burn Wound in Diabetic Old Patient with Panchagavya Formulations - A Case Report.

Burn injuries are often prevalent and among the most catastrophic injuries encountered in casualty or surgery departments, involving skin and other tissues. These injuries typically result from heat sources such as solids, liquids, or fire, while scalds arise from hot liquids, electricity, chemicals, cold, friction, or radiation. Despite varying causes, the male-female ratio of burn cases remains consistent. In India, the annual incidence of burns is estimated at six to seven million. Topical antimicrobial agents are crucial for treatment. This case study focuses on an 83-year-old woman with a

Evaluation of electrophotobiomodulation as a recent tool in the armamentarium of treatment of fingertip injuries.

BACKGROUND: Traditional reconstructive options of fingertip injuries are technically difficult, usually need donor site skin grafting, leave visible scars, and need a protracted period of finger immobilization resulting in joint stiffness. Electro-photobiomodulation (EPBM), is the process of combining intense pulsed light and radiofrequency to modify tissues to help the body heal itself, lower inflammation, and promote wound healing. PATIENTS AND METHODS: This study included 60 patients presented with fingertip injuries. Patients were randomly divided into two groups. Group 1 includes patient

Kidney clamp, perfuse, re-implant: a useful technique for graft salvage after vascular complications during kidney transplantation.

Although intra-operative vascular complications during renal transplantation are rare, injuries associated with prolonged ischemia may lead to graft threatening early and late complications. This series describes a novel technique for intra-operative repair of vascular complications in five patients over a three-yr period. The method consists of rapid graft nephrectomy and re-preservation of the graft with cold University of Wisconsin solution, which allows for controlled/precise back table repair of the vascular injury without incurring prolonged warm ischemia time. In three cases, the donor

DHAM-BMSC matrix promotes axonal regeneration and functional recovery after spinal cord injury in adult rats.

Axonal regeneration and functional recovery after complete spinal cord injury (SCI) are limited in adult mammals. The purpose of this study was to investigate whether an innovative composite matrix made from a denuded human amniotic membrane (DHAM) seeded with bone marrow stromal cells (BMSCs) could promote axonal regeneration after SCI. We transplanted the composite matrix between the stumps of severed spinal cords immediately following SCI. Twelve weeks after surgery, the functional recovery of the hindlimbs of operated rats was evaluated by the Basso-Beattie-Bresnahan (BBB) locomotor rating
