Designing a Territorial Composite Vulnerability Index to guide public health action in Cali, Colombia.

BACKGROUND: Social determinants of health are central to explaining health inequalities. In fragmented urban contexts such as Cali, Colombia, territorially focused public health action requires analytical tools to identify priority areas for intervention. OBJECTIVE: To design and apply a Territorial Composite Vulnerability Index for the city of Santiago de Cali to support the micro-planning of public health interventions. METHODS: We conducted an ecological, citywide study with a geospatial, multivariate approach. Seven indicators representing social determinants were selected: multidimensio

The effects of polio eradication efforts on health systems: a cross-country analysis using the Develop-Distort Dilemma.

Vertical disease control programmes have enormous potential to benefit or weaken health systems, and it is critical to understand how programmes' design and implementation impact the health systems and communities in which they operate. We use the Develop-Distort Dilemma (DDD) framework to understand how the Global Polio Eradication Initiative (GPEI) distorted or developed local health systems. We include document review and 176 interviews with respondents at the global level and across seven focus countries (Afghanistan, Bangladesh, Democratic Republic of Congo, Ethiopia, India, Indonesia and

The Impact of the COVID-19 Pandemic on Community Health Centers in Canada and the United States: A Scoping Review.

Community health centers (CHCs) play a critical role in addressing the health needs of diverse and underserved communities. However, there remains a significant gap in the literature examining their resilience, particularly in crises such as COVID-19. This scoping review analyzed 68 articles to identify which performance indicators have been used to assess the impact of the pandemic on CHCs, and to understand what these indicators reveal about the CHCs' performance and resilience. The findings revealed that most performance indicators focused on access, availability, and quality of care, highl

Strategies to reduce Gram-negative infections: a community perspective.

Infections caused by Gram-negative bacteria continue to be on the rise, despite efforts by the Government and health service to curb their numbers. Most of these infections arise in the community. The case for targeting community-onset healthcare-associated infections is stark and requires a shift in focus from traditionally providing increased efforts in the hospital setting to a diversion of attention to the community. This article describes the challenges faced with increasing Gram-negative bloodstream infections and explores measures being taken to reduce transmission. As recent guidance h

The Contributions of Onchocerciasis Control and Elimination Programs toward the Achievement of the Millennium Development Goals.

In 2000, 189 member states of the United Nations (UN) developed a plan for peace and development, which resulted in eight actionable goals known as the Millennium Development Goals (MDGs). Since their inception, the MDGs have been considered the international standard for measuring development progress and have provided a blueprint for global health policy and programming. However, emphasis upon the achievement of priority benchmarks around the "big three" diseases--namely HIV, tuberculosis (TB), and malaria--has influenced global health entities to disproportionately allocate resources. Preparing for Inclusion of Agricultural Workers in Climate-Related and Other Public Health Emergencies: A Quantitative Content Analysis of County Emergency Preparedness Plans, Eastern North Carolina, 2022-2023.

OBJECTIVE: Agricultural workers such as migrant and seasonal farmworkers are a population uniquely vulnerable to the negative effects of climate-related disasters. The primary aim of this study was to assess how counties in eastern North Carolina (NC) incorporated best practices related to agricultural workers in emergency preparedness planning. METHODS: The authors conducted a quantitative content analysis of 47 emergency preparedness plans from 41 eastern NC counties between November 2022 and March 2023. The research team developed a codebook based on recommended best practices from the Cen

Can family planning outreach bridge the urban-rural divide in Zambia?

BACKGROUND: Zambia experienced declining aggregate fertility and increasing aggregate contraceptive use from 1990 to 2000. Yet, in rural Zambia, progress in family planning has lagged far behind the advances made in Zambia's urban areas. The contraceptive prevalence rate in Lusaka and other urban areas outstripped the rate in rural Zambia by nearly 25 percentage points (41.2 percent versus 16.6 percent) in 2001. The total fertility rate varied between urban and rural areas by 2.5 children (4.3 versus 6.9 children). This paper considers the urban-rural differentials in Zambia and assesses famil

A Community Partnership to House and Care for Complex Patients with Unstable Housing.

Rising homelessness, especially among older adults, has significant ramifications for our health care system. People experiencing homelessness tend to experience worse health and poorer access to needed health care than people with stable housing. Commonwealth Care Alliance (CCA), a not-for-profit payer and provider that offers health plans to people dually eligible for Medicare and Medicaid, sought to address homelessness among its beneficiaries through a partnership with a local community-based housing organization, Hearth. This partnership led to many CCA members gaining access to permanent

"People are afraid of getting injections": feasibility of community-placed atropine auto-injectors in treating organophosphate poisoning in Sri Lanka.

INTRODUCTION: Organophosphorus insecticide poisoning remains a major problem in rural low- and middle-income communities. Atropine auto-injectors are used for organophosphorus nerve agent poisoning, but not organophosphorus insecticide poisoning. We explore the social and practical feasibility of introducing atropine auto-injectors in Sri Lankan villages to initiate first-line treatment for acute organophosphorus insecticide poisoning. We investigate how auto-injectors might be best integrated into the social and spatial context of rural communities with high levels of organophosphorus insecti

Community preferred drug delivery approaches for pilot roll-out of a potential novel paediatric schistosomiasis treatment option in two endemic counties of Kenya: A mixed methods study.

Treating preschool age children (PSAC) for schistosomiasis has remained a challenge due to lack of a pediatric formulation. In response to this unmet need, the Paediatric Praziquantel Consortium has developed a potential novel paediatric treatment option. In advance to its roll-out to follow regulatory response, we conducted a social science study to gather information on preferred drug delivery approaches to inform implementation. A cross-sectional study was conducted in eight villages in two purposively selected Kenyan counties. A questionnaire was administered on 690 parents/guardians of PS

Coverage of insecticide-treated mosquito nets in preschool consultations and factors associated with non-use of insecticide-treated mosquito nets among children in the Democratic Republic of Congo: A base population cross-sectional study.

Insecticide-treated nets (ITNs) are among the most effective malaria prevention interventions, yet their actual use among children remains below national targets. This study assesses ITN coverage and utilization among children attending preschool consultations (PSC) and identifies factors associated with non-use. A cross-sectional study was conducted in 2025 in 156 health facilities across six provinces of the Democratic Republic of the Congo (DRC). Children aged 6-59 months accompanied by a parent or guardian were included. Data were collected during a household survey through face-to-face in

From 3T to HLAA: developing a participatory model for health literacy empowerment among older adults in Northeastern Thailand.

BACKGROUND: Thailand’s transition to a super-aged society poses significant challenges for medication safety and rational health product use among older adults, particularly in rural Northeastern communities where health literacy remains limited. This study aimed to develop and refine the HLAA (Health Literacy–Learning–Access–Advocacy) Model, grounded in the culturally embedded 3T (Think–Test–Take Action) behavioral pathway, using a participatory action research (PAR) framework. METHODS: A community-based PAR design guided by the Plan–Act–Observe–Reflect (PAOR) cycle was conducted in Schools f

A Collaborative Approach to Understanding the Intersections of Practice and Policy for Peers in the Alcohol and Other Drugs Sector.

INTRODUCTION: Peers in the alcohol and other drug sectors possess lived-living experience (LLE) crucial for shaping community care. However, genuine consumer collaboration is often confounded by stigma. This study examined peers' perceptions, exploring their experiences regarding workforce dynamics, policy implications, and impacts on health equity. In presenting the research, we sought to synthesise the research methods and illustrate the methodological innovation and knowledge production in substance use research through authentic collaboration. METHODS: We purposively sampled peer networks

Protocol for a community-engaged mixed-methods study for climate-related disaster preparedness and health care resilience in North Carolina.

BACKGROUND: Climate change-induced disasters pose an enormous risk to human health. With the risk of climate change disasters at an all-time high, it is imperative to strengthen the resilience of health and community systems. Working with community collaborators who provide services across the spectrum of the disaster cycle will make disaster preparedness plans context-specific, sustainable, and accepted by the community. Using evidence-based Implementation Science approaches can facilitate the development and evaluation of community-academic partnership initiatives to build responsive and res

Tailoring physical activity maintenance strategies for cancer survivors: A study protocol for an adaptive intervention.

BACKGROUND: Moderate-vigorous physical activity (MVPA) (i.e., aerobic and resistance exercise) is effective for improving multiple health outcomes for cancer survivors, and structured, supervised community-based exercise programs can facilitate MVPA adoption among cancer survivors. However, maintaining MVPA after these programs end can be challenging for some. The purpose of this paper is to describe the protocol of an adaptive intervention designed to enhance MVPA maintenance among cancer survivors who have recently completed a community-based exercise program. METHODS: Participants are insu

Adverse Event Leads to Quality Improvement Plan: Learning From a Classroom Latex Exposure.

A growing number of students with health conditions, many of which carry with them the potential for an emergency, increases the likelihood of adverse events occurring in the school setting. An adverse event can be either preventable or nonpreventable. This article reviews a preventable adverse event which took place in a second-grade classroom of a student with spina bifida. Children with spina bifida are at increased risk for the development of latex allergy and subsequent anaphylaxis. The seriousness of accumulated exposure to latex necessitated initiation of a quality improvement plan with

Commentary: Discharging the Homeless - A Daily Issue in Acute Care.

There are complex issues surrounding hospital discharge planning for people experiencing homelessness. The issue involves the disconnection across policy areas of housing, income supports and mental health, and later health generally. Different models for different types of communities (large urban, mid-size, small and rural areas) likely need to be developed as well as for different types of conditions and different housing histories. The quality of data needs improvement including accuracy. Housing items need to be part of admission processes so that the need for post-discharge housing can b

Failure to Appear: Mental Health Professionals' Role Amidst Pretrial Justice Reform.

Pretrial detention reform is keeping people who have been arrested on low-level charges out of jail while they await trial. This reform has implications for people with serious mental illnesses who are overrepresented in the criminal legal system and who can now stay connected to families, employment, community supports, and treatment providers while their cases are processed. However, such reforms may have uniquely negative consequences for those with serious mental illnesses. In this Open Forum, the authors argue that it is critical for mental health professionals to understand what pretrial

Walking the tightrope: developing a systems approach to balance safety and mobility for an aging society.

In an aging population, safety and mobility are often viewed as being in conflict, when in fact they are two values that must be reflected equally in transportation policy. The challenge for policy makers and for society is to emphasize and optimize both. To achieve this, a comprehensive systems approach to driver health, personal decision-making and planning, community and transportation systems construction, vehicle design, and licensing regulations is needed. Such an approach requires crafting policies based on conceptualizing the issue as a continuum of resources facilitating safety and mo
