Implementing a Validated Multi-Symptom Assessment Tool During Telephone Triage to Reduce Oncology Patient Emergency Room Visits.

Cancer treatments induce multiple unwanted side effects that often go unrelieved, resulting in emergency room (ER) visits. Oncology clinics have established triage clinics (TCs) for symptom management, thereby improving access to care and decreasing ER utilization. In addition, evidence proves that validated patient-reported outcome (PRO) tools support improved symptom management and decreased ER visits. This quality improvement project aimed to determine if or to what degree implementing the MD Anderson Symptom Inventory (MDASI) tool decreases emergency room visits, with or without hospitaliz

TAVR and cancer: machine learning-augmented propensity score mortality and cost analysis in over 30 million patients.

IntroductionCardiovascular disease (CVD) and cancer are the top mortality causes globally, yet little is known about how the diagnosis of cancer affects treatment options in patients with hemodynamically compromising aortic stenosis (AS). Patients with cancer often are excluded from aortic valve replacement (AVR) trials including trials with transcatheter AVR (TAVR) and surgical AVR (SAVR). This study looks at how cancer may influence treatment options and assesses the outcome of patients with cancer who undergo SAVR or TAVR intervention. Additionally, we sought to quantitate and compare both

Opioid Use at End-Of-Life Among Nova Scotia Patients With Cancer.

Purpose: To determine the factors associated with opioid analgesic prescriptions as measured by community pharmacy dispensations to all Nova Scotia (NS) patients with cancer at end-of-life from 2005 to 2009. Methods: The NS Cancer Registry and the NS Prescription Monitoring Program (NSPMP) were used to link Nova Scotians who had a cancer diagnosis and received a prescription for opioids in their last year of life (n = 6,186) from 2005 to 2009. The association of factors with opioid dispensations at end-of-life were determined (e.g., patient demographics, type of prescriber, type of cancer, and

Patient relevant endpoints in oncology: current issues in the context of early benefit assessment in Germany.

UnlabelledThe German AMNOG healthcare reform includes a mandatory early-benefit-assessment (EBA) at launch. As per German social code, EBA is based on registration trials and includes evaluation of the patient-relevant effect of the new medicines compared to an appropriate comparator as defined by the Federal Joint Committee (G-BA). Current EBA decisions released have unveiled issues regarding the acceptance of some patient-relevant endpoints as G-BA and IQWiG are grading the endpoints, focusing on overall survival as the preferred endpoint in oncology.A taskforce of experienced German outcome

Fatigue as main symptom in an elderly multiple myeloma patient: a Case Report.

INTRODUCTION: The high prevalence of fatigue as an atypical clinical manifestation in general practice creates notable diagnostic challenges, particularly in geriatric patients. METHODS: We describe a case of an 80-year-old female presenting with fatigue, initially attributed to poorly controlled diabetes mellitus or upper respiratory infection, who was ultimately diagnosed with multiple myeloma after admitted to the Department of General Medicine at a general hospital. Initial diagnostic workup including physical examination revealed the presence of anterior thoracic mass with undetermined e

Enrollment of adolescents and young adults onto SWOG cancer research network clinical trials: A comparative analysis by treatment site and era.

BackgroundFew adolescents and young adults (AYAs, 15-39 years old) enroll onto cancer clinical trials, which hinders research otherwise having the potential to improve outcomes in this unique population. Prior studies have reported that AYAs are more likely to receive cancer care in community settings. The National Cancer Institute (NCI) has led efforts to increase trial enrollment through its network of NCI-designated cancer centers (NCICC) combined with community outreach through its Community Clinical Oncology Program (CCOP; replaced by the NCI Community Oncology Research Program in 2014).M

Social Isolation, Loneliness, Anxiety, and Quality of Life in a Chronic Ulcer Population With and Without Stigma: An Observational Trial.

Chronic leg ulcers impact millions worldwide, causing visible symptoms including discoloration, swelling, scarring, and requiring bulky bandaging, which often leads to stigma. Stigma may worsen other psychosocial impairments. This observational study examined stigma, social isolation, loneliness, anxiety, and quality of life (QOL) in adults aged 50+ receiving clinic-based wound care. Participants provided sociodemographic information and completed validated assessments. Among the 26 completers (13 with stigma/S+, 13 without stigma/S-), the average age was 63.2 years; 38% were female, and 58% B

Medical history predicts phenome-wide disease onset and enables the rapid response to emerging health threats.

The COVID-19 pandemic exposed a global deficiency of systematic, data-driven guidance to identify high-risk individuals. Here, we illustrate the utility of routinely recorded medical history to predict the risk for 1741 diseases across clinical specialties and support the rapid response to emerging health threats such as COVID-19. We developed a neural network to learn from health records of 502,489 UK Biobank participants. Importantly, we observed discriminative improvements over basic demographic predictors for 1546 (88.8%) endpoints. After transferring the unmodified risk models to the All

Hospital Use, Morbidity, and Cancer Risk by Age Group in Neurofibromatosis Type 1: A Nationwide Retrospective Cohort Study.

Neurofibromatosis type 1 (NF1) affects multiple organs progressively, leading to a shortened life expectancy. This study aimed to identify the complications that are prominent in each age group by comparing 28,082 patients with NF1 and 84,246 matched controls. Benign and malignant neoplasms along with neurological complications were the primary contributors to hospital use across most age groups. Respiratory and circulatory disorders were a common cause of hospital use in children and older adults, respectively. Except for soft tissue and central nervous system-originated malignancies, which a

Association between race and treatment patterns and survival outcomes in multiple myeloma: A Connect MM Registry analysis.

BackgroundStudies have reported racial disparities in access to and use of multiple myeloma (MM) treatments between African American (AA) and White patients. Although AA patients demonstrate longer disease-specific survival, this has not uniformly translated into improved survival over time. The association between race and treatment patterns and survival outcomes was analyzed using data from the Connect MM Registry.MethodsThe Connect MM Registry is a large US, multicenter, prospective observational cohort study of patients with newly diagnosed MM. Nephrology in Veterinary Medicine.

Veterinary nephrology is a specialized field of veterinary medicine providing a high level of care for animals with all types of kidney disease. Veterinarians complete extensive training to become board-certified in veterinary nephrology-urology. Companion animal nephrology is the most advanced field; however, all species are afflicted by a variety of renal disorders. Most naturally occurring animal kidney diseases have similar disorders found in people; where veterinary research is lacking, clinical management is often modified from standard of care in people. Veterinarians have become adept

Risk of cancer in patients with thalassemia and sickle cell disease: a systematic review.

BACKGROUND: Individuals with thalassemia or sickle cell disease (SCD) are at an elevated risk of developing cancer, yet the full extent of this risk remains inadequately characterised. METHODS: We conducted a systematic review using three databases (MEDLINE, EMBASE and Scopus) to compare the cancer risk in patients with thalassemia and SCD with that of the general population. Secondly, we assessed the impact of other putative risk factors in patients with thalassemia and SCD on cancer risk. RESULTS: Five studies from four countries were included, encompassing a total of 24,439 individuals ov
