PURPOSE OF REVIEW: Allergic skin diseases arise from complex interactions between epithelial barrier dysfunction and immune dysregulation. This review examines how structural and functional defects in the epidermal barrier predispose to conditions such as atopic dermatitis, allergic contact dermatitis, and chronic spontaneous urticaria, and explores how mechanistic insights into these abnormalities guide therapeutic selection. RECENT FINDINGS: Advances in molecular and genetic research have clarified the roles of filaggrin deficiency, lipid disorganization, altered skin pH, tight junction impa

CONTEXT: Zinc is essential for cutaneous barrier integrity and immune regulation. Whether zinc status is associated with severity and whether zinc supplementation provides clinical benefit in inflammatory dermatoses remains uncertain.
OBJECTIVE: To synthesize evidence on zinc status and validated severity indices in psoriasis, atopic dermatitis (AD), and seborrheic dermatitis, and to evaluate whether zinc supplementation improves clinical outcomes.
DATA SOURCES: PubMed, ScienceDirect, SpringerLink, and Google Scholar (January 2000-December 2025) were used. A hand‑search of references was con

BACKGROUND: Patients with skin of color (SOC) experience increased rates of atopic dermatitis (AD) than the general population and suffer from more severe disease. In addition, patients with SOC experience higher sensitization rates to particular allergens and are patch tested less frequently. Because of these factors, there is a need to assess whether allergic contact dermatitis (ACD) contributes to the higher disease severity in SOC patients with AD.
OBJECTIVE: The goal of this review is to systematically evaluate the potential association between ACD and AD in the SOC population.
METHODS:

BACKGROUND: Atopic dermatitis (AD) and loss-of-function mutations in the filaggrin gene (FLG) are both associated with chronic irritant contact dermatitis (ICD). As FLG mutations also are a major risk factor for AD, it is not clear whether FLG mutations are an independent risk factor for ICD or whether the risk is mediated by AD.
OBJECTIVES: To investigate the relative contribution and interaction of FLG mutations and AD in German patients with occupational ICD and controls (vocational school apprentices).
METHODS: A total of 634 patients and 393 controls were genotyped for R501X, 2282del4,

Atopic dermatitis (AD) affects up to 20% of children worldwide and is an increasing public health problem, particularly in developed countries. Although AD in infants and young children can resolve, there is a well-recognized increased risk of sequential progression from AD to other atopic diseases, including food allergy (FA), allergic rhinitis, allergic asthma, and allergic rhinoconjunctivitis, a process referred to as the atopic march. The mechanisms underlying the development of AD and subsequent progression to other atopic comorbidities, particularly FA, are incompletely understood and th

Atopic dermatitis (AD) is a multi-system inflammatory skin disorder with early onset in the skin. It is well known that Black and Hispanic children in the United States experience specific barriers in regards to accessing care for AD, including greater severity on presentation and more need for care including increased usage of emergency services. Understanding these social determinants is vital if social change is to be made and if policies are to be constructed to create enduring reductions in disparity in a meaningful way that can potentially level disease severity and access to care for al

INTRODUCTION: Atopic dermatitis (AD) is a recurring and non-contiguous dermatological condition that impacts a huge global population. Pruritus, inflammation, numerous eczematous lesions, and an unpredictable progression are the hallmark characteristics of this dermatological complication. Despite its multifactorial and incompletely comprehended pathophysiology, AD appears from a combined cause of environmental and genetic factors that lead to epidermal barrier integrity, oxidative stress, and immune system imbalance at both the skin and systemic levels.
AREAS COVERED: The lipid nanocarrier (

BACKGROUND: Escalating global air pollution poses severe threats to public health. Beyond respiratory and cardiovascular damage, air pollutants adversely affect cutaneous health. Atopic dermatitis (AD), a common chronic inflammatory skin disease, has been closely linked to environmental pollutant exposure in recent research, arousing widespread academic concern.
OBJECTIVE: This narrative review aimed to comprehensively summarize current evidence on the correlation between air pollution and AD, and further elaborate the epidemiological features, potential pathogenic mechanisms, as well as targ

The understanding of atopic dermatitis (AD) pathogenesis is evolving beyond the barrier-immune dichotomy, with neuropsychological factors gaining prominence as a central component. This review systematically decodes the multi-level crosslink within the "brain-skin axis" in AD, encompassing recent advances in peripheral sensory neuron sensitization, central neural remodeling, and neuro-immune interactions. We elaborate on the roles of neuropeptides, cytokines, the autonomic nervous system, and the hypothalamic-pituitary-adrenal axis in the itch-scratch cycle, inflammation amplification, and moo

Key therapeutic options for dermatologic care frequently extend beyond prescription medications to include evidence-based, over-the-counter (OTC) topicals that meaningfully influence disease pathophysiology. We introduce the concept of active dermatologic care (ADC), which encompasses nonprescription topical products with biologically active compounds that support and modulate key skin disease processes and can be routinely incorporated into comprehensive dermatologic care plans. We expand upon the recent concept of categorizing OTC active ingredients for acne based on their mechanisms of acti

Psoriasis is a chronic inflammatory skin disease, and previous studies among Western populations have suggested an increased risk of cardiovascular events in patients with psoriasis. However, evidence from Asian populations remains limited. We evaluated the risk of major adverse cardiovascular events (MACE) in patients with psoriasis compared with patients who have atopic dermatitis (AD) using a large-scale administrative claims database from Shizuoka, Japan. We conducted a cohort study using the Shizuoka Kokuho Database, including patients aged ≥ 40 years who were newly diagnosed with psorias

Atopic dermatitis is a compelling challenge in daily practice. Type 2 inflammation is the most common endotype in children and adolescents with AD. As a result, anti-inflammatory drugs, mainly corticosteroids (CS) and immunomodulatory agents, represent the first-line treatment to dampen type 2 inflammation. However, biologics, particularly dupilumab, dramatically changed the natural history of AD patients. A steering committee composed by expert pediatricians and dermatologists promoted a multidisciplinary Delphi Consensus to improve the knowledge about this topic. Experts in this field partic

BACKGROUND: Perinatal phthalate exposure has been suggested as a potential contributor to atopic dermatitis (AD).
OBJECTIVE: To investigate whether neonatal urinary phthalate levels predict the development of AD and to explore mechanisms involving immune activation and skin barrier dysfunction.
METHODS: Urinary phthalate metabolites collected within 48 hours after birth were analyzed by liquid chromatography-tandem mass spectrometry. AD at 12 months of age was identified based on physician diagnosis and treatment history. Logistic regression analysis was used to evaluate the associations bet

INTRODUCTION: Atopic dermatitis (AD) affects 20% of children, with persistent itching and repeated scratching that may interfere with daily activity and impair quality of life. Given the chronicity of the disease and its significant impact on quality of life, many parents and patients are interested in identifying alternative and "natural" therapies for the prevention and treatment of AD, such as nutritional supplements and dietary modifications. This article provides a narrative overview of dietary and nutritional interventions for the prevention and treatment of AD.

OBJECTIVE: Current studies on immune dysregulation in autism spectrum disorder (ASD) have not adequately accounted for the influence of common comorbidities like atopic dermatitis (AD). To address this gap, the objective of this study was to characterize the peripheral immune profile in children with both ASD and AD, and to assess whether AD comorbidity modulates the link between immune dysregulation and ASD symptoms.
METHODS: This single-center cross-sectional study recruited children with ASD who attended the outpatient clinic of Peking Union Medical Hospital (PUMCH) from April 2019 to Sept
