Drug-induced ocular inflammatory diseases.

Ocular inflammation can arise in the form of conjunctivitis, uveitis, episcleritis and scleritis. The uveitides can be subdivided by anatomical location into anterior and posterior uveitis or categorized by location of inflammation, e.g., iritis, pars planitis or iridocyclitis. Multiple drugs have been associated with ocular inflammation and much has been written on the subject. Discussed here is a sampling of drugs representing classes of medication associated with ocular inflammation. However, this is not a comprehensive list and interested readers are encouraged to access the National Regis

Molecular mechanisms of action of bisphosphonates: current status.

PURPOSE: Bisphosphonates are currently the most important class of antiresorptive agents used in the treatment of metabolic bone diseases, including tumor-associated osteolysis and hypercalcemia. These compounds have high affinity for calcium ions and therefore target bone mineral, where they are internalized by bone-resorbing osteoclasts and inhibit osteoclast function. EXPERIMENTAL DESIGN: This article reviews the pharmacology of bisphosphonates and the relationship between chemical structure and antiresorptive potency. We also describe new insights into their intracellular molecular mechan

Bisphosphonates and oral cavity avascular bone necrosis: a review of twelve cases.

BACKGROUND: Intravenous bisphosphonates are the current standard of care for the treatment of hypercalcemia of malignancy and for the prevention of skeletal complications associated with bone metastases. Recently, retrospective case studies have reported an association between long-term bisphosphonate therapy and osteonecrosis of the jaws. PATIENTS AND METHODS: The data for twelve patients, referred to either an oral and maxillofacial surgeon or to an oral medicine specialist for the management of clinically apparent chronic oral osteonecrosis of unknown etiology, were reviewed. All had recei

Pharmacologic modulation of periprosthetic osteolysis.

Wear and periprosthetic osteolysis of total joint replacements continue to be the most important problems in arthroplasty surgery. Despite the introduction of improved technologies including alternative bearing surfaces for TJRs, wear is inevitable because of relative movement at different interfaces and processes such as electrolysis and material degradation. Worn, clinically failing implants need to be followed closely and revised when appropriate. However, early wear and minor osteolysis do not result necessarily in progressive failure of the prosthesis. Indeed such cases may be followed up

Recommendations for monitoring antiresorptive therapies in postmenopausal osteoporosis.

Antiresorptive agents are effective in preventing and treating postmenopausal osteoporosis, provided they are taken as directed. Regular physical examinations including height measurements may fail to ensure optimal compliance. Bone mineral density (BMD) measurement is indispensable for determining whether treatment is warranted. The measurement can be repeated after 2 years at least, provided quality-control procedures are adequate. BMD changes over time should be compared to the least significant change calculated from the in vivo BMD reproducibility at the measurement center. However, BMD c

Managing the care of patients with bisphosphonate-associated osteonecrosis: an American Academy of Oral Medicine position paper.

BACKGROUND: This position paper addresses the prevention of bisphosphonate-associated osteonecrosis (BON) and the management of care of patients with cancer and/or osteoporosis who are receiving bisphosphonates and who have BON or are at risk of developing it. METHODS: The authors reviewed the literature available on this newly described oral complication. Information of interest included bisphosphonates, the medications associated with this oral complication; the patient population at risk of developing BON and the diseases being treated with this class of medications; the clinical presentat

Moving into the future: treatment of bone metastases and beyond.

In various common cancers, the skeleton is a preferred site of metastasis. It is also threatened by bone loss resulting from anti-cancer therapy. Like bone metastases in advanced cancer, cancer treatment induced bone loss (CTIBL) substantially increases fracture risk and dramatically decreases quality of life and patient autonomy. Both chemotherapy and endocrine deprivation therapy (EDT) can significantly decrease bone mineral density (BMD). This is aggravated by the often long duration of EDT, particularly in the adjuvant setting. Cumulative bone loss can put patients at risk of osteopenia an

Treatment of bone disease in cystic fibrosis.

PURPOSE OF REVIEW: As individuals with cystic fibrosis (CF) have experienced marked improvements in longevity over the last three decades, bone disease has emerged as a new problem. Bone disease in CF has not been previously reviewed in this journal. Therefore, this review will give a brief overview of bone disease in CF and then concentrate on treatment options. RECENT FINDINGS: In some series, as many as three fourths of adults with CF have low bone density. Decreased absorption of fat-soluble vitamins due to pancreatic insufficiency, altered sex hormone production, chronic inflammation, ph

Ibandronate: new options in the treatment of osteoporosis.

Bisphosphonates are currently considered the treatment of choice for corticosteroid-induced male osteoporosis and represent one of the options for the prevention and treatment of postmenopausal osteoporosis. The treatment of osteoporosis with antiresorbing agents requires long-lasting or even lifelong therapies. Oral bisphosphonates are reasonably well tolerated, but a sizable proportion of patients report esophageal symptoms which are occasionally severe. In addition, their intestinal absorption is only 0.5-1% and the presence of any residual food in the stomach completely blocks absorption.

Bisphosphonates in oncology: physiopathologic bases and clinical activity.

Osteoclastic activation is the ultimate way of bone resorption in neoplasia, induced by the combined effects of tumor-secreted humoral factors (especially parathyroid hormone-related peptides) and osteoclastic-osteoblastic interaction. Bisphosphonates inhibit the osteoclast activity and reduce bone resorption and are a valuable supportive measure for bone disease of neoplasms. Experimental models also suggest an activity of bisphosphonates against cancer cells. Controlled studies, especially in advanced breast cancer and multiple myeloma, indicate different effectiveness against the distinct s

Bisphosphonates to prevent osteoporosis in men receiving androgen deprivation therapy for prostate cancer.

Osteoporosis is an important complication of androgen deprivation therapy for prostate cancer. Androgen deprivation therapy either by bilateral orchiectomies or treatment with a gonadotropin-releasing hormone agonist decreases bone mineral density (BMD) and increases the risk of fracture. Dietary factors and lifestyle may contribute to bone loss. There are limited prospective data about treatment or prevention of osteoporosis in men with prostate cancer and many recommendations are based on studies of postmenopausal osteoporosis. Lifestyle modification including smoking cessation, moderation o

Idiopathic hyperphosphatasia.

Idiopathic hyperphosphatasia is a rare autosomal recessive bone disorder, characterized by excessive bone resorption and bone formation. The radiographic appearances include widening of the diaphyses, vertebral osteoporosis, acetabular protrusion, and thickening of the skull vault. There is considerable variability in phenotype, with some cases diagnosed in infancy and others in later childhood. Most cases appear to arise from inactivating mutations in the gene encoding osteoprotegerin, a product of osteoblasts that is critically involved in osteoclastogenesis. Treatment with inhibitors of bon

Bisphosphonates: from the laboratory to the clinic and back again.

Bisphosphonates (BPs) used as inhibitors of bone resorption all contain two phosphonate groups attached to a single carbon atom, forming a "P-C-P" structure. The bisphosphonates are therefore stable analogues of naturally occuring pyrophosphate-containing compounds, which now helps to explain their intracellular as well as their extracellular modes of action. Bisphosphonates adsorb to bone mineral and inhibit bone resorption. The mode of action of bisphosphonates was originally ascribed to physico-chemical effects on hydroxyapatite crystals, but it has gradually become clear that cellular effe

Paget's disease of bone: current concepts in pathogenesis and treatment.

Recent advances have been made in understanding the aetiology, diagnosis and treatment of Paget's disease of bone (osteitis deformans). The role of viruses in the pathogenesis of the disease has been extensively investigated, although no causative link has yet been demonstrated between infective agents and Paget's disease. Recent advances in the diagnosis of the disease have been made with increasing usage of radionucleotide imaging to determine the extent and distribution of the lesions. Treatment options have been extended with the emergence of the biphosphonates as potential main line thera
