Limb Salvage After Grenade-Related Brachial Artery Injury: A Case Report.

Since war injuries are not predictable, each patient presents as a new and unique case. Before deciding on amputation for upper extremity injuries, it is necessary to evaluate all limb-sparing vascular interventions that will keep the amputation level as distal as possible, in order to improve the patient's quality of life and enable the use of more functional prostheses. While amputation may be a faster and more life-saving option for a surgeon, limb-sparing and mobile joint-preserving surgical procedures should be the preferred choice, provided the patient's overall condition permits. This c

Radial head fractures with interposed capitellar cartilage fragment-hindrance to bone healing-a case series.

INTRODUCTION: Radial head fractures account for the majority of bony elbow injuries. The individual treatment options have been described in detail. In some cases, however, an unusual concomitant injury occurs, which can significantly impede primary osteosynthesis and healing. This concomitant injury can be an interposing cartilaginous capitellar fragment. METHODS: This retrospective study describes four cases of trapped cartilage fragments of the capitellum that compromised primary osteosynthesis or primary conservative healing of a radial head fracture. Radiological imaging, function and pa…

An unusual mechanism of sideswipe injury in passengers traveling in a train and review of literature.

INTRODUCTION: Sideswipe injuries are most commonly described in passengers traveling in a car or bus, but here we report an unusual mode of sideswipe injuries, in passengers traveling in a train sitting on window seats with elbow protruding outside the window. CASE SERIES: Four patients reported to our Orthopaedic emergency 2 hours after a railway track accident with more or less similar pattern of injury, an open proximal forearm monteggia fracture-dislocation with bone and soft tissue loss along with closed fracture humerus, and with or without radial nerve palsy and intact distal pulses. W

Articular fractures of the distal part of the humerus.

BACKGROUND: The purpose of this retrospective study was to identify the patterns of distal humeral articular fractures and to analyze the results of open reduction and internal fixation of these injuries. METHODS: The cases of twenty-one patients with an articular fracture of the distal part of the humerus were reviewed at an average of forty months after the injury. Five components of the injury were identified: (1) the capitellum and the lateral aspect of the trochlea, (2) the lateral epicondyle, (3) the posterior aspect of the lateral column, (4) the posterior aspect of the trochlea, and (

Management of Missed Type-2 Monteggia Fracture Equivalent in a 9-Year-Old Child: A Case Report.

CASE: A 9-year-old girl presented with a painful stiff elbow consequent to an injury she had sustained 4 months ago. The radiographs were suggestive of a type 2 Monteggia equivalent injury. She underwent an open reduction of the radial neck fracture and proximal ulna extension osteotomy to restore radiocapitellar alignment. At one-year follow-up, the child had a painless elbow with a good functional outcome. CONCLUSION: Monteggia lesions can be frequently missed and result in significant morbidity. Obtaining appropriate radiographs for a comprehensive understanding of the injury and proper su

Fractures of the radial head and neck in children.

In thirty-three children with fractures of the radial head or neck, and with a minimum follow-up of two years, fractures of the radial neck were most common. The proximal radial epiphysis was usually closed by the age of fifteen years. The older children in the series had the worst prognosis, as did children with other associated upper-extremity injuries, usually on the medial aspect of the elbow. The results were best if treatment was initiated early, and closed reduction usually gave the most satisfactory results. Roentgenograms of the involved elbow were often abnormal even with a good clin

Bilateral and simultaneous rupture of the triceps tendons in chronic renal failure and secondary hyperparathyroidism.

BACKGROUND: We report a case of a 27-year-old (male) patient with bilateral and simultaneous rupture of the triceps tendons. He was suffering from chronic renal failure secondary to acute glomerulonephritis and for 4 years he underwent maintenance hemodialysis three times a week. This injury is extremely rare, and only one case has been reported in the international literature. Based on previous cases of tendon ruptures in patients with chronic renal failure, we believe that secondary hyperparathyroidism is the primary causative factor in his case. CASE REPORT: A primary repair was performed

[Monteggia injuries in childhood].

The Monteggia-fracture dislocation is a characteristic combined injury of the forearm with fracture of the ulna and dislocation of the head of the radius. Monteggia- and equivalent injuries are rare. In the present study, we attempt to clarify the concept of therapy and observe the influence of this injury on the growth of the forearm bones. Between 1977 and 1996, 27 patients (three to thirteen years) with a Monteggia or a Monteggia-equivalent injury were treated. We present clinical and radiological long-term results of 20 patients, two to 21 years following the injury. Group I includes 12 pa

Functional Outcome following Phrenic Nerve Transfer in Brachial Plexus Injury.

BACKGROUND: Brachial plexus injuries are debilitating injuries resulting in paralyzed shoulder to global paralysis of the upper extremity. Treatment strategies have evolved over the years with nerve transfer forming the mainstay of surgical management. Phrenic nerve provides certain advantages as donor over other options but has been less preferred due to fear of pulmonary complications. In this study, we assess the functional outcomes of phrenic nerve transfer in brachial plexus injuries. MATERIALS AND METHODS: A retrospective study was performed on 18 patients operated between 2012 and 2017

Hand-Arm Vibration Assessment and Changes in the Thermal Map of the Skin in Tennis Athletes during the Service.

During recent years the number of tennis athletes has increased significantly. When playing tennis, the human body is exposed to many situations which can lead to human injuries, such as the so-called tennis elbow (lateral epicondylitis). In this work a biomechanical analysis of tennis athletes, particularly during the service, was performed, considering three different types of over-grip and the presence of one anti-vibrator device. One part of the study evaluates the exposure to hand-arm vibration of the athlete, based on the European Directive 2002/44/EC concerning the minimum health and sa

Clinical rating systems in elbow research-a systematic review exploring trends and distributions of use.

BACKGROUND: Clinical rating systems are used as outcome measures in clinical trials and attempt to gauge the patient's view of his or her own health. The choice of clinical rating system should be supported by its performance against established quality standards. METHODS: A search strategy was developed to identify all studies that reported the use of clinical rating systems in the elbow literature. The strategy was run from inception in Medline Embase and CINHAL. Data extraction identified the date of publication, country of data collection, pathology assessed, and the outcome measure used.

A muscle sparing posterior approach to the elbow joint for total elbow arthroplasty.

We describe a muscle sparing approach in which the triceps is elevated without injuring the muscle or disturbing its insertion. The entire extensor mechanism is preserved in continuity, thus preventing any extensor weakness. This can be used preferentially in cases of non-union intraarticular distal humerus fractures planned for Total Elbow Arthroplasty.

Natural course in tennis elbow--lateral epicondylitis after all?

Tennis elbow is a common and difficult-to-treat condition largely because of lack of evidence. The natural history is unknown, but the condition is described as self-limiting. The objective of this case report is to describe the natural course of two control participants (pain free), who later developed tennis elbow, patient history, clinical findings, and ultrasound and colour Doppler examination before, during and after a period of tennis elbow.

Permanent hand motor deficit and failure of elbow flexion recovery following Oberlin nerve transfer. A retrospective case series.

INTRODUCTION: The Oberlin nerve transfer is widely used to restore elbow flexion after brachial plexus injury. However, donor-site morbidity, particularly permanent motor deficit, has been rarely reported. This retrospective case series describes permanent donor-site morbidity following the Oberlin procedure and explores its association with poor elbow flexion outcomes. METHODS: Nine patients (mean age, 38 years) with upper-type brachial plexus paralysis, including seven C5-C8 injuries, were referred to a tertiary centre after undergoing a primary Oberlin transfer at external institutions. The Effectiveness of Saline Load Test in Detecting Simulated Traumatic Elbow Arthrotomies: A Cadaveric Investigation.

Background The saline load test has not been well explored in the elbow. We aimed to determine 1) the saline infusion volume needed for 90%, 95%, and 99% sensitivity in detecting elbow arthrotomy; and 2) factors associated with higher volume at detection using sixteen forequarter upper extremity amputation cadavers. Methods Sixteen fresh-frozen forequarter upper extremity amputations were procured, and demographic data, including age, body mass index (BMI), and laterality, were recorded. The olecranon process, radial head, and the lateral epicondyle were palpated, and elbow arthrotomy was cons

Risk factors for cartilage damage and osteoarthritis of the elbow joint: case-control study and systematic literature review.

INTRODUCTION: This case-control study compares patients with healthy elbows to a group of symptomatic patients with cartilage damage/osteoarthritis. MATERIALS AND METHODS: The control group (n = 126) was recruited during routine medical examinations of patients (general medical offices). Included in the case group were a total of 92 patients who were undergoing arthroscopy as a result of chronic elbow discomfort. All patients were questioned with regard to occupational stress and athletic stress. RESULTS: A significantly increased risk of cartilage damage/osteoarthritis was found with subjec

Injury tolerance of the wrist and distal forearm to impact loading onto outstretched hands.

BACKGROUND: The wrist/forearm complex is one of the most commonly fractured body regions, yet the impact tolerance of the wrist is poorly understood. This study sought to quantify the injury tolerance of the adult male forearm-wrist complex under loading simulating axial impact to an outstretched hand. METHODS: Fifteen isolated cadaveric forearm/wrist specimens were tested. Loading was applied via an instrumented drop tower device designed to impact the palmar surface of the hand with the wrist extended to approximately 90 degrees. Impact severity was modulated by adjusting the boundary condi

Radial head fractures: loss of cortical contact is associated with concomitant fracture or dislocation.

HYPOTHESIS: Among radial head fractures displaced greater than 2 mm (Broberg and Morrey modified Mason type 2), separation (complete loss of cortical contact) of at least 1 radial head fracture fragment is associated with a complex injury pattern, meaning that there are other concomitant elbow fractures or ligament injuries. MATERIALS AND METHODS: We identified 291 consecutive skeletally mature patients with 296 radial head fractures treated during a 6-year period. Of these, 121 consecutive fractures of part of the radial head displaced greater than 2 mm (type 2) were classified according to

Simultaneous dislocation of the radial head and distal radio-ulnar joint. A case report.

Isolated dislocation of the distal radio-ulnar joint and isolated dislocation of the radial head in adults are not common injuries. A simultaneous dislocation of the radial head and distal radio-ulnar joint with no other injury seems to be extremely rare since only one report was found in the English literature. A similar case, but with some differences in presentation and treatment is reported.

The term "floating" used in traumatic orthopedics.

BACKGROUND: The term "floating" is used in orthopedic literature to describe certain patterns of skeletal injuries that share one common character which is disruption and discontinuity of bones above and below a joint. The first time used in orthopedic literature being in late 1970 to describe a type of elbow injury. Later the word was used increasingly and applied to a variety of injuries affecting the knee, shoulder, hip, forearm, hand, and ankle. Currently, there are about 12 different skeletal injuries described as floating. OBJECTIVES: The aim of this article was to define the term "floa
