Eight-year institutional review of carotid artery stenting.

OBJECTIVES: Vascular surgeons have increasingly become proficient in carotid artery stenting (CAS) as an alternative treatment modality for cervical carotid artery occlusive disease. We analyzed our early and late outcomes of CAS over the last 8 years. METHODS: We report a single-center retrospective review of 388 carotid bifurcation lesions treated with CAS using cerebral embolic protection from May 2001 to July 2009. Data analysis includes demographics, procedural records, duplex exams, arteriograms, and two-view plain radiographs over a mean follow-up time of 23.0 months (interquartile ran

Intimal fibroelastic thickening--its etiological significance in juvenile cerebral infarction. Case report.

Recurrent occlusive stroke of the vertebrobasilar system is reported in a 16-years-old man. Multiple arterial occlusions, segmental stenosis, or irregular dilatation were found in the angiography involving both the intra- and extracranial vertebro-basilar arteries. Surgical specimen of the extracranial vertebral artery with localized irregular lumen disclosed fibroelastic thickening of the intima with mural thrombus, which were thought to be an embolic source. Etiological significance of this intimal lesion in juvenile cerebrosvascular accident is discussed.

Infarct patterns, collaterals and likely causative mechanisms of stroke in symptomatic intracranial atherosclerosis.

BACKGROUND: There are limited data on the specific mechanisms of stroke in patients with intracranial atherosclerotic stenosis (ICAS). We undertook this study to describe infarct patterns and likely mechanisms of stroke in a large cohort of patients with ICAS, and to evaluate the relationship of these infarct patterns to angiographic features (collaterals, stenosis location and stenosis severity). METHODS: We evaluated infarct patterns in the territory of a stenotic intracranial artery on neuroimaging performed at baseline and during follow-up if a recurrent stroke occurred in patients enroll

Neurologic Complications of Cardiac Disease and Cardiac Procedures.

OBJECTIVE: This article reviews the neurologic complications of common cardiac conditions and cardiac procedures. LATEST DEVELOPMENTS: During the past two decades, there has been an expansion of pharmacologic and procedural strategies to reduce stroke risk in patients with atrial fibrillation (eg, direct oral anticoagulants, left atrial appendage occlusion, and catheter ablation), patent foramen ovale (ie, percutaneous closure in selected patients), and those undergoing cardiac surgery (eg, embolic protection devices). ESSENTIAL POINTS: Because stroke is the most common neurologic complicati

Real-time observation of a high-echoic mass in the left ventricle during transcatheter aortic valve implantation: a case report.

BACKGROUND: Increasing number of symptomatic patients with severe aortic stenosis is treated with transcatheter aortic valve implantation (TAVI). Stroke is one of the most serious complications of TAVI, and the majority of cerebral events in patients undergoing TAVI have an embolic origin. CASE SUMMARY: A 90-year-old female underwent trans-femoral TAVI for symptomatic severe aortic stenosis. Just before the implantation of the transcatheter heart valve (THV), transoesophageal echocardiography (TOE) showed a mobile, high-echoic mass attached to the THV, which gradually enlarged to 26 mm, then

Large vessel stroke as initial presentation of thrombotic thrombocytopenic purpura.

A 67-year-old right-handed woman presented with dysarthria, left upper extremity weakness and right-sided neglect of 3 hours duration. Imaging of the brain revealed acute right middle cerebral artery stroke; however, tissue plasminogen activator could not be administered due to severe thrombocytopenia. A peripheral smear revealed schistocytes and the patient was treated empirically for thrombotic thrombocytopenic purpura (TTP) with therapeutic plasma exchange. An extensive workup revealed no embolic source or other cause for stroke, and a diagnosis of large vessel infarct secondary to TTP was

Neurologic manifestations of Chagas disease.

Chagas disease is endemic in Latin America and has become an emerging problem in developed countries because of international migrations. The protozoan Trypanosoma cruzi is the etiologic agent and the disease is divided into two phases. The acute phase is mostly asymptomatic or presents with unspecific symptoms. Rarely, a severe and often fatal form occurs in immunosuppressed patients or infants, characterized by meningoencephalitis (sometimes including brain tumor-like lesions) and myocarditis. The chronic phase consists of an indeterminate, asymptomatic form followed by digestive, cardiac, o

Artery-to-artery embolism with a mobile mural thrombus due to rotational vertebral artery occlusion.

Rotational vertebral artery (VA) occlusion can cause ischemic strokes due to hemodynamic insufficiency and possibly artery-to-artery (A-to-A) embolism. The former is known as bow hunter's stroke. The latter has been proposed only from indirect evidence. We have described a 7-year-old boy with cerebral infarction associated with A-to-A embolism due to repetitive rotational VA occlusion. He had a mobile mural thrombus at the VA occlusion site on head rotation. Surgical treatment may effectively prevent recurrences.

A dynamic concept of middle cerebral artery occlusion and cerebral infarction in the acute state based on interpreting severe hyperemia as a sign of embolic migration.

The present study investigates the pathogenesis of focal cerebral hyperemia, its effect on brain tissue and discusses its pathophysiological and therapeutic importance in the light of interpreting severe hyperemia as a sign of arterial reopening probably due to embolic migration. Cerebral angiography, serial CT-scans and serial TC99 -scans were performed in a consecutive group of 73 patients with completed stroke all admitted to hospital within 3 days after stroke onset. When possible the regional cerebral blood flow (rCBF) was studied with the intracarotid Xe 133 injection method. Twenty-nine

Management of Patients at Risk of Ischemic Stroke With Left Ventricular Systolic Dysfunction in the Absence of Intracardiac Thrombus: A Scientific Statement From the American Heart Association.

Left ventricular (LV) systolic dysfunction, defined here as reduced LV ejection fraction ≤40% or left wall motion abnormality, is commonly found in patients with ischemic stroke. Although there is an increased risk of incident and recurrent embolic stroke among patients with LV dysfunction without thrombus detected, the data supporting anticoagulation in these patients are limited. In this scientific statement, we summarize the latest evidence regarding the risk of incident and recurrent stroke in patients with LV dysfunction as well as best practice recommendations regarding the management of…

Rapid Embolic Event Chain: An Unusual Presentation of Marantic Endocarditis.

Nonbacterial thrombotic endocarditis (NBTE), formerly known as marantic endocarditis, is characterized by sterile, noninfectious vegetations typically associated with hypercoagulable states, most commonly malignancies. Despite its noninfectious nature, NBTE can lead to serious complications similar to those seen in infectious endocarditis, including stroke, venous thromboembolism, splenic or hepatic infarcts, and acute intestinal ischemia. We present an unusual case involving a cascade of rapid embolic events in a patient newly diagnosed with metastatic lung cancer. The patient developed criti

Incidental Finding of Arteria Lusoria in COVID-19 Patient with Aortic Thrombus Complicated by Recurrent Limb Ischemia.

Coronavirus disease causes a hypercoagulable state with endothelial inflammation. These conditions can produce embolic complications. Thrombosis can occur in different locations and may lead to ischemic stroke and ischemic limb. We report the case of a 64-year-old woman with recent severe acute respiratory syndrome coronavirus 2 pneumonia who was admitted to the hospital due to a giant floating thrombus in the ascending aorta with multiple ischemic events. Aortic computed tomography angiogram revealed an aberrant right subclavian artery associated with bicarotid trunk that might have protected

Endovascular thrombectomy and stenting in the management of carotid fibromuscular dysplasia presenting with major ischemic stroke.

Fibromuscular dysplasia (FMD) affecting the internal carotid artery (ICA) is considered a benign condition; however, retinal and cerebral ischemic events may occur. We present a patient with carotid FMD presenting with a major ischemic stroke due to major embolic occlusion of the ICA in conjunction with a hemodynamic component caused by narrowing and beading of the lumen associated with FMD. The patient was successfully treated with intracranial stent-assisted thrombectomy followed by cervical ICA stenting that aimed to reconstruct and angioplasty the FMD-affected arterial segment. Recently, s

Embolic events caused by aortic thrombi: an underestimated entity?

Stroke and other thromboembolic events are mainly caused by emboli from heart, aorta and other arteries. In this paper we describe a group of 5 middle-aged patients suffering from emboli caused by large thrombi in the aorta. Since the development of giant thrombi under high flow conditions in the aorta is a pathophysiological process which is not well understood, a model of flow distribution by numerically simulating the Navier-Stokes equation for an incompressible fluid was generated. This model simulated how such thrombi may develop in the aorta. We hypothesize that large thrombi issuing fro
