Global, regional, and national burden of acute hepatitis (1990-2021): a systematic analysis of the global burden of disease in 2021.

BACKGROUND: Acute viral hepatitis (AVH) remains a global health concern, with significant variations in incidence, mortality, and DALYs across different regions, age groups, genders, and socioeconomic levels. This study explores AVH trends using data from the Global Burden of Disease (GBD) database (1990-2021). METHODS: We analyzed age-standardized incidence, mortality, and DALY rates across 204 countries and 27 super-regions. Trends were quantified using the estimated annual percentage change (EAPC) and decomposition analysis to assess the contributions of population growth, aging, and epide

Periodontitis and atherosclerotic cardiovascular disease.

Atherosclerotic cardiovascular disease (ASCVD) is a major global health concern linked to significant morbidity and mortality. Recent research has explored the relationship between ASCVD and periodontitis, a prevalent inflammatory oral condition. Epidemiological studies have suggested a strong association between periodontitis and ASCVD, even proposing that periodontal disease could be a modifiable risk factor for cardiovascular conditions. This review critically analyzes the current evidence for a potential causal role for periodontitis in ASCVD. While randomized controlled trials have demons

The role of older antibiotics in the treatment of skin and soft tissue infections: current perspectives.

PURPOSE OF REVIEW: The aim is to discuss the evidence and recent literature on the role of older antibiotics in the treatment of skin and soft tissue infections (SSTIs). RECENT FINDINGS: The choice of therapy for SSTIs is complicated in view of the rising antimicrobial resistance (AMR) and the availability of new antibiotics. SSTIs are predominantly caused by Staphylococcus aureus and beta-hemolytic streptococci, but other organisms can be involved in patients with comorbidities or post trauma. Treatment options are dictated by the accessibility and cost of newer antibiotics in resource-const

The current status of risk-stratified breast screening.

Apart from high-risk scenarios such as the presence of highly penetrant genetic mutations, breast screening typically comprises mammography or tomosynthesis strategies defined by age. However, age-based screening ignores the range of breast cancer risks that individual women may possess and is antithetical to the ambitions of personalised early detection. Whilst screening mammography reduces breast cancer mortality, this is at the risk of potentially significant harms including overdiagnosis with overtreatment, and psychological morbidity associated with false positives. In risk-stratified scr

Repurposing of drugs approved for cardiovascular diseases: Opportunity or mirage?

Drug repurposing is a promising way in drug discovery to identify new therapeutic uses -different from the original medical indication- for existing drugs. It has many advantages over traditional approaches to de novo drug discovery, since it can significantly reduce healthcare costs and development timeline. In this review, we discuss the possible repurposing of drugs approved for cardiovascular diseases, such as β-blockers, angiotensin converting enzyme inhibitors (ACE-Is), angiotensin II receptor blockers (ARBs), statins, aspirin, cardiac glycosides and low-molecular-weight heparins (LMWHs)

Evaluating the link between Paraoxonase-1 levels and Alzheimer's disease development.

At present, the etiopathogenesis of Alzheimer's disease (AD), the most common form of dementia, remains far to be fully deciphered. In the recent years, also the centrality of amyloid-β peptide in the pathogenesis of the neurodegenerative disease has been questioned and other hypotheses have been advanced. Notably, a common denominator of many of these theoretical models is represented by oxidative stress, which is widely proposed to play a role in the disease initiation and/or progression. Paraoxonase 1 (PON1) is a high-density lipoprotein (HDL)-associated enzyme that endows its carrier with

Reviewing the role of human papillomavirus in oral cancer using the Bradford Hill criteria of causation.

Human papillomavirus is a well-established risk factor for oropharyngeal cancer, although its role in oral cancer is still debated. Inconclusive evidence of its role in oral cancer is due to conflicting data arising from methodological differences, mostly due to the use of diagnostic tests with varying sensitivity and specificity. In addition, there is a lack of experimental data linking HPV to oral cancer. Recent epidemiological studies provide data on HPV prevalence in oral squamous cell carcinoma and other potentially malignant oral disorders. Further, molecular data from in vivo and in vit

Spatio-temporal variations of typhoid and paratyphoid fevers in Zhejiang Province, China from 2005 to 2015.

Typhoid and paratyphoid are two common enteric infectious diseases with serious gastrointestinal symptoms. Data was collected of the registered cases in Zhejiang Province from 2005 to 2015. The epidemiological characteristics were investigated and high-risk regions were detected with descriptive epidemiological methods and in-depth spatio-temporal statistics. A sharp decline in the incidences of both diseases was observed. The seasonal patterns were identified with typhoid and paratyphoid, one in summer from May to September was observed from 2005 to 2010 and the other lesser one in spring fro

The complex relationship between CD4 count, HIV viral load, trimethoprim-sulfamethoxazole prophylaxis, and skin-and-soft-tissue infection risk in patients with HIV: insights from a causal diagram and simulation study.

Skin and soft tissue infection (SSTIs) due to Staphylococcus aureus, particularly community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA), are common in human immunodeficiency virus (HIV)-infected populations in the United States. Studies have differed as to the importance of epidemiological and immunological factors in this relationship, and have employed conflicting strategies for variable selection in multivariate analyses. Developments in causal inference methods in epidemiology have emerged in the last decade to clarify relationships between variables and identify appro

What causes autism? Exploring the environmental contribution.

PURPOSE OF REVIEW: Autism is a biologically based disorder of brain development. Genetic factors--mutations, deletions, and copy number variants--are clearly implicated in causation of autism. However, they account for only a small fraction of cases, and do not easily explain key clinical and epidemiological features. This suggests that early environmental exposures also contribute. This review explores this hypothesis. RECENT FINDINGS: Indirect evidence for an environmental contribution to autism comes from studies demonstrating the sensitivity of the developing brain to external exposures s

Dynamic modeling of vaccinating behavior as a function of individual beliefs.

Individual perception of vaccine safety is an important factor in determining a person's adherence to a vaccination program and its consequences for disease control. This perception, or belief, about the safety of a given vaccine is not a static parameter but a variable subject to environmental influence. To complicate matters, perception of risk (or safety) does not correspond to actual risk. In this paper we propose a way to include the dynamics of such beliefs into a realistic epidemiological model, yielding a more complete depiction of the mechanisms underlying the unraveling of vaccinatio

Overview of modifiers of methylmercury neurotoxicity: chemicals, nutrients, and the social environment.

It has been known for decades that methylmercury is a potent neurotoxicant, and that the developing brain is more susceptible to impairment as a result of methylmercury exposure than is the adult. Exposure to methylmercury is exclusively through consumption of fish and marine mammals. In recent years, the potential for protection against methylmercury toxicity by nutrients present in fish, particularly omega-3 fatty acids and selenium, has been explored in both epidemiological and experimental studies. There is evidence from several studies that fish consumption per se and methylmercury body b

Intrauterine programming of the endocrine pancreas.

Epidemiological studies have revealed strong relationships between poor foetal growth and subsequent development of the metabolic syndrome. Persisting effects of early malnutrition become translated into pathology, thereby determine chronic risk for developing glucose intolerance and diabetes. These epidemiological observations identify the phenomena of foetal programming without explaining the underlying mechanisms that establish the causal link. Animal models have been established and studies have demonstrated that reduction in the availability of nutrients during foetal development programs

Work-related musculoskeletal disorders of the hand and wrist: epidemiology, pathophysiology, and sensorimotor changes.

The purpose of this commentary is to present recent epidemiological findings regarding work-related musculoskeletal disorders (WMSDs) of the hand and wrist, and to summarize experimental evidence of underlying tissue pathophysiology and sensorimotor changes in WMSDs. Sixty-five percent of the 333 800 newly reported cases of occupational illness in 2001 were attributed to repeated trauma. WMSDs of the hand and wrist are associated with the longest absences from work and are, therefore, associated with greater lost productivity and wages than those of other anatomical regions. Selected epidemiol

[Electrosmog as a health risk factor: sources of artificial electromagnetic fields, evaluation of health risk, prevention methods].

In recent years we have observed how electromagnetic (EM) radiation enters our daily life. The strength of man-made EM field is often far above the natural level and this finding has encouraged a large group of researchers to investigate its possible health effect. Non-ionizing radiation and extremely low-frequency electric and magnetic fields have been the subject of intensive theoretical and experimental works since Adey published his observations concerning non-linear and non-thermal biological effects. At the same time an epidemiological material appeared suggesting that EM field generates

A statistical method for assessing a threshold in epidemiological studies.

I describe a method for estimating and testing a threshold value in epidemiological studies. A threshold effect indicates an association between a risk factor and a defined outcome above the threshold value but none below it. An important field of application is occupational medicine where, for a lot of chemical compounds and other agents which are non-carcinogenic health hazards, so-called threshold limit values or TLVs are specified. The method is presented within the framework of the logistic regression model, which is widely used in the analysis of the relationship between some explanatory

Syphilis control. The historic context and epidemiologic basis for interrupting sexual transmission of Treponema pallidum.

Syphilis control has been the prototypic sexually transmitted disease (STD) public health program of the 20th century. However, the disease remains nearly as much an epidemiologic enigma as it did in the early 1900s. This article examines the historic and epidemiologic bases for syphilis control, using unpublished data to supplement a recent model of STD transmission. The authors recommend building on such traditional individually oriented strategies as case finding, partner notification, and presumptive treatment as a basis for future community-oriented, population-based strategies including

The need to incorporate polysubstance use in neuropsychopharmacology research: Biological lessons and new opportunities.

Critical evaluation and refinement of animal models is essential for neuroscientists to understand complex physiological and pathological processes related to psychiatric diseases. In general, preclinical studies modeling drug dependence and problematic substance use have been limited to the administration of a single substance; however, there is a growing appreciation that this approach has failed to capture the complexities of humans and has stifled translational efforts. Polysubstance use represents the overwhelmingly common patterns of alcohol and drug use in humans. For example, epidemiol

Animal pneumocystosis: a model for man.

Pneumocystis carinii is an important pulmonary pathogen responsible for morbidity and mortality in patients with AIDS. Apart from AIDS, cases of pneumocystosis have been reported in patients receiving immunosuppressive therapy associated with organ transplantation without chemoprophylaxis and in malignant blood diseases. In vitro models are only of limited interest because there is no continuous in vitro culture. The in vivo models have contributed a great deal to the understanding of human Pneumocystis carinii pneumonia. Indeed, animal models remain of prime interest for many purposes, princi
