Effect of 1 month of zopiclone on obstructive sleep apnoea severity and symptoms: a randomised controlled trial.

Hypnotic use in obstructive sleep apnoea (OSA) is contraindicated due to safety concerns. Recent studies indicate that single-night hypnotic use worsens hypoxaemia in some and reduces OSA severity in others depending on differences in pathophysiology. However, longer clinical trial data are lacking. This study aimed to determine the effects of 1 month of zopiclone on OSA severity, sleepiness and alertness in patients with low-moderate respiratory arousal thresholds without major overnight hypoxaemia.69 participants completed a physiology screening night with an epiglottic catheter to quantify

Role of common hypnotics on the phenotypic causes of obstructive sleep apnoea: paradoxical effects of zolpidem.

Hypnotics are contraindicated in obstructive sleep apnoea (OSA) because of concerns of pharyngeal muscle relaxation and delayed arousal worsening hypoxaemia. However, human data are lacking. This study aimed to determine the effects of three common hypnotics on the respiratory arousal threshold, genioglossus muscle responsiveness and upper airway collapsibility during sleep.21 individuals with and without OSA (18-65 years) completed 84 detailed sleep studies after receiving temazepam (10 mg), zolpidem (10 mg), zopiclone (7.5 mg) and placebo on four occasions in a randomised, double-blind, plac

The Effect of the Cervical Orthosis on Swallowing Physiology and Cervical Spine Motion During Swallowing.

Cervical orthosis is used to immobilize the neck in various disorders such as trauma and post-operation. However, it is still uncertain how cervical orthosis restricts the degree of movement of the cervical spine during swallowing and how they affect swallowing physiology. The purpose of this study was to evaluate these issues using the Philadelphia(®) Collar. We conducted videofluorography of swallowing in 39 healthy subjects (23 men, 16 women; mean age of 34.3 years) with and without cervical orthosis. To compare the two conditions regarding the cervical spine motion, we determined the angul

Day surgery for vocal fold lesions using a double-bent 60-mm Cathelin needle.

Day surgery for vocal cord lesions overcomes the disadvantages of laryngomicrosurgery under general anesthesia. We present our experience with treatment of vocal fold lesions using a long double-bend Cathelin needle that can access all parts of the vocal cords. A 23G, 60-mm-long Cathelin needle was bent twice by 45(o) at a distance of 1 and 2 cm from the tip, and was attached to a syringe. Under topical anesthesia and nasal endoscopy of the laryngopharynx, the needle was inserted percutaneously perpendicular to the skin, the direction of insertion being altered when the bends in the needle rea

Unilateral superior laryngeal nerve lesion in an animal model of dysphagia and its effect on sucking and swallowing.

We tested two hypotheses relating to the sensory deficit that follows a unilateral superior laryngeal nerve (SLN) lesion in an infant animal model. We hypothesized that it would result in (1) a higher incidence of aspiration and (2) temporal changes in sucking and swallowing. We ligated the right-side SLN in six 2-3-week-old female pigs. Using videofluoroscopy, we recorded swallows in the same pre- and post-lesion infant pigs. We analyzed the incidence of aspiration and the duration and latency of suck and swallow cycles. After unilateral SLN lesioning, the incidence of silent aspiration durin

High-dose radiotherapy alone for patients with T4-stage laryngeal cancer.

BACKGROUND AND PURPOSE: The purpose of this retrospective study was to report on the efficacy of radiotherapy alone in patients with T4-stage laryngeal cancer and to establish the prognostic value of (a) the size and location of the extralaryngeal tumor extensions and (b) of emergency tracheostomy. PATIENTS AND METHODS: A group of 114 patients were treated with definitive radiotherapy between 1990 and 1996. The piriform recess was involved in 37 cases (33 %), the base of the tongue and glosso-epiglottic vallecula in 34 cases (30 %), and the hypopharyngeal wall in 10 cases (9 %). In 16 cases (

The historical Latin and etymology of selected anatomical terms of the larynx.

The etymological evolution of the anatomical terms larynx, cricoid, glottis, epiglottis, and thyroid (cartilage) dates to antiquity. Human dissection replaced animal in the 16th and 17th centuries and terms evolved. This evolution was recorded in the literature largely in Latin. We translated key studies of laryngeal anatomy from the 16th century to better understand this evolution. We present the Latin with our translations, and historical commentary as essential to this understanding. Vesalius favored the Latin scutiform (shield) for the thyroid cartilage, but recognized peltalis (shield). T

An evaluation of poor LMA CTrach views with a fibreoptic laryngoscope and the effectiveness of corrective measures.

BACKGROUND: The LMA CTrach++ system has integrated fibreoptics to enable viewing of the glottis to aid tracheal intubation via a laryngeal mask conduit. Earlier evaluations of this system had shown high success rates of insertion, ventilation and tracheal intubation, but frequent difficulty or failure to view the glottis. METHOD: We evaluated CTrach views in 100 patients. In 69 patients with partial or no views of the glottis, we evaluated the causes with the aid of an Olympus LF-DP fibreoptic laryngoscope passed through the CTrach. In those with poor views, we tested the effectiveness of sim

Sternothyrohyoideus myectomy or staphylectomy for treatment of intermittent dorsal displacement of the soft palate in racehorses: 209 cases (1986-1991).

The efficacy of sternothyrohyoideus myectomy (SM) and staphylectomy as treatments for intermittent dorsal displacement of the soft palate was evaluated in 209 racehorses (126 Thoroughbreds, 83 Standardbreds). The 2 most common complaints were respiratory tract noise (75% of horses) and exercise intolerance (51% of horses). Stopping, defined as the inability to finish the race at racing speed, was reported in 40% of the horses. Endoscopy at rest revealed evidence of abnormal epiglottic anatomy (hypoplasia or flaccidity) in 51% of the horses and abnormal soft palate function in 36% of the horses

[Anesthesia and laryngeal muscle, especially intrinsic laryngeal muscles].

The intrinsic laryngeal muscles are highly specialized for phonation, respiration and sphincter activity. The muscles are disposed between three unpaired cartilages (the thyroid, cricothyroid and epiglottic) and the paired arytenoid cartilages. These laryngeal muscles, which are striated in character, are bilaterally disposed and can be grouped as adductors and abductors of the glottis and tensors of the vocal cords. The motor neurons for the intrinsic laryngeal muscles are located in the caudal half of the nucleus ambiguous. The intrinsic laryngeal muscles receive motor nerve supply from the

Occult node metastases in head and neck squamous carcinoma.

The present study examined the Liverpool database in an attempt to determine what proportion of N0 necks for various head and neck primary sites harbored subclinical squamous cell carcinoma and whether empiric treatment of occult disease improved survival over and above a "wait-and-watch policy" (treatment when metastasis becomes manifest). One hundred seventeen neck dissections were carried out for N0 necks, with 32% of specimens found to contain squamous cell carcinoma. The risk of carcinoma was highest in the hypopharynx, with 50% of specimens associated with a pyriform fossa primary cancer

Interstitial 192Ir flexible catheter radiofrequency hyperthermia treatments of head and neck and recurrent pelvic carcinomas.

Since September 1983, five patients with head and neck cancers and five patients with pelvic or perineal recurrences of colorectal neoplasms received 192Ir interstitial implants through flexible afterloading catheters that were modified to allow RF hyperthermia treatments of the tumor within 1 hr pre- and post-brachytherapy. Local control in the implant volume was obtained in three of the patients with head and neck cancers (base tongue--2/4; floor of mouth--1/1) with follow-up of 9 to 42 months. Two patients had local recurrences after disease-free periods of 8 and 24 months. Two of the five

Paediatric Upper Respiratory Infection in Colombia: 10-Year Nationwide Trends in Healthcare Utilization From Administrative Data (2015-2024).

BACKGROUND: Upper respiratory tract infections (URTIs) are amongst the commonest reasons for paediatric care, yet national evidence from Latin American middle-income health systems remains limited. We used the COVID-19 pandemic as a system shock to examine whether paediatric URTI care returned to its pre-pandemic trajectory or settled into a new service configuration. METHODS: We performed a nationwide population-based time-series study using Colombia's administrative healthcare registry (RIPS) linked to official population projections (DANE) for children and adolescents aged 0-17 years from

Intraday effects of outdoor air pollution on acute upper and lower respiratory infections in Australian children.

Children's respiratory health are particularly vulnerable to outdoor air pollution, but evidence is lacking on the very acute effects of air pollution on the risk of acute upper respiratory infections (AURI) and acute lower respiratory infections (ALRI) in children. This study aimed to evaluate the risk of cause-specific AURI and ALRI, in children within 24 h of exposure to air pollution. We obtained data on emergency cases, including 11,091 AURI cases (acute pharyngitis, acute tonsillitis, acute obstructive laryngitis and epiglottitis, and unspecified acute upper respiratory infections) and 1

[Does vaccination cause disease?].

Not many inventions in medical history have influenced our society as much as vaccination. The concept is old and simple. When Edward Jenner published his work on cowpox, "variolation" was quite common. In this procedure, pus of patients with mild smallpox was transferred to healthy individuals. Meanwhile smallpox has been eradicated worldwide. Diseases such as poliomyelitis, diphtheria or tetanus almost disappeared in industrialized countries. The same happened with epiglottitis and meningitis due to Haemophilus influenzae type b (Hib) after vaccination against Hib was introduced in Switzerla

Point-of-care ultrasound (POCUS) in emergency airway management: emerging evidence for assessment, prediction, and confirmation.

Effective airway management is essential in emergency medicine, where rapid patient deterioration and limited preparation time increase the risk of hypoxia, aspiration and cardiac arrest. Traditional bedside assessments and confirmation techniques such as Mallampati scoring, auscultation and capnography have notable limitations, particularly in patients with trauma, obesity, cardiac arrest or distorted anatomy. Point-of-care ultrasound (POCUS) has emerged as a reliable adjunct that provides real-time, non-invasive visualisation of airway structures, prediction of difficult laryngoscopy and rap
