Pharmacogenetics to Avoid Loss of Hearing (PALOH) trial: a protocol for a prospective observational implementation trial.

INTRODUCTION: In conjunction with a beta-lactam, aminoglycosides are the first-choice antibiotic for empirical treatment of sepsis in the neonatal period. The m.1555A>G variant predisposes to ototoxicity after aminoglycoside administration and has a prevalence of 1 in 500. Current genetic testing can take over 24 hours, an unacceptable delay in the acute setting. This prospective-observational trial will implement a rapid point of care test (POCT), facilitating tailored antibiotic prescribing to avoid hearing loss. METHODS AND ANALYSIS: The genedrive POCT can detect the m.1555A>G variant in 2

Establishment and assessment of the hepatic venous pressure gradient using biofluid mechanics (HVPGBFM): protocol for a prospective, randomised, non-controlled, multicentre study.

INTRODUCTION: Portal hypertension (PH) is a severe disease with a poor outcome. Hepatic venous pressure gradient (HVPG), the current gold standard to detect PH, is available only in few hospitals due to its invasiveness and technical difficulty. This study aimed to establish and assess a novel model to calculate HVPG based on biofluid mechanics. METHODS AND ANALYSIS: This is a prospective, randomised, non-controlled, multicentre trial. A total of 248 patients will be recruited in this study, and each patient will undergo CT, blood tests, Doppler ultrasound and HVPG measurement. The study cons

Adaptation and feasibility assessment of a school-based Suicide Prevention Intervention for Adolescents (SPREAD Study) in Nigeria: protocol for a cluster randomised controlled feasibility trial.

INTRODUCTION: Adolescent suicide constitutes a critical public health challenge in low-income and middle-income countries (LMICs), where structural deficits in mental health infrastructure compound an already substantial burden. In Nigeria, where suicidal ideation among secondary school students has been documented at a lifetime prevalence of 22.9%, curative clinic-based responses are neither scalable nor sustainable. School-based universal interventions offer a pragmatic upstream alternative. The Youth Aware of Mental Health (YAM) programme has demonstrated robust efficacy in European context

Evaluation of an innovative family-centred care and prevention intervention for children with overweight and obesity: a mixed-methods study protocol of the randomised controlled fruehstArt study in Germany.

INTRODUCTION: Childhood overweight and obesity pose a growing public health problem with increasing prevalence both in Europe and globally. Reasons can be found in behavioural factors such as a sedentary lifestyle, eating habits or low exercise levels and to a lesser extent in a genetic predisposition or a metabolic disorder. Preventing children with obesity and overweight to grow into obese teenagers is therefore of high importance. However, there are currently no established care and prevention programmes in Germany for the early reduction of overweight and prevention of obesity in children

Early-life infectious and nutritional exposures and cardiovascular risk in early adulthood in Uganda: protocol for a new round of data collection in the Entebbe Mother and Baby Study birth cohort at 21 years (EMaBS@21).

INTRODUCTION: Non-communicable diseases, particularly cardiovascular diseases (CVDs), have become major contributors to morbidity and mortality in sub-Saharan Africa (SSA) and are projected to surpass infectious diseases as the leading cause of death among adults by 2030. Although CVDs have traditionally been associated with older age and obesity, adverse cardiovascular phenotypes are increasingly being observed in younger and leaner individuals in SSA. This pattern suggests that pathways to CVD risk in SSA may differ from those described in high-income countries. Early-life infectious exposur

Constraint-Induced Movement Therapy With or Without Neuromuscular Electrical Stimulation Versus Conventional Therapy for Upper Extremity Function in Chronic Stroke Survivors: Protocol for a Cluster Randomized Controlled Trial.

BACKGROUND: Motor impairment is a common consequence of stroke, significantly impacting patients' daily functioning and quality of life. More than 80% of stroke survivors experience upper extremity (UE) motor impairments, which severely impact their ability to carry out daily living activities. Combining motor rehabilitation techniques, such as constraint-induced movement therapy (CIMT), with sensorimotor stimulation methods, such as neuromuscular electrical stimulation (NMES), may be beneficial in enhancing UE functional recovery after a stroke. This combined approach could activate a broader

Randomised, double-blind, parallel group, placebo-controlled, trial of Bactek for the prevention of lower respiratory tract infections in preterm infants in the UK: BALLOON study - study protocol.

INTRODUCTION: A significant proportion of infants born at ≤29+6 weeks' gestation develop lung disease during the neonatal period, thus putting them at risk of developing prematurity-associated lung disease in childhood and adulthood. After discharge from the neonatal unit, pre-existing lung disease in preterm-born infants is exacerbated by (often frequent) respiratory viral infections requiring greater health utilisation, including hospital admissions, than their term-born equivalents. Opportunities to prevent viral infections in infancy are largely limited to anti-respiratory syncytial virus

Ethics, transplantation, and the changing role of anatomists.

Anatomists are regarded as custodians of cadaveric material donated to science. Almost every facet of medical science has experienced explosive advances. This has impacted directly on anatomists and their role. Increasingly, anatomists are raising concerns with regard to the treatment of human tissue (Jones,2002, Clin. Anat. 15:436-440). The Korperwelten (Bodyworlds) of Gunther von Hagens et al. (1987, Anat. Embryol. 175:411-421) has evoked considerable debate about the treatment of human cadavers. Thus far clinical anatomists have had little role to play in policy formulation, legislation, an

National qualitative study in France to explore parents' lived experience at the birth of a child with a large or giant congenital melanocytic nevus.

INTRODUCTION: Congenital melanocytic nevi (CMN) are congenital malformations that cannot be detected by prenatal ultrasounds and may have a significant physical and psychological impact on affected individuals, particularly when they are large. Congenital anomalies are known to impair parental quality of life, which may in turn affect the child's well-being and development. However, little is known about the feelings and lived experience of parents on the birth of a CMN-affected child. The aim of this study is to describe the lived experience of parents of a child with a large or giant CMN, and

"… Infections are not confined in labs…": Community engagement for Controlled Human Infection Studies: Opinions of researchers, bioethicists and research regulators in Uganda.

Controlled human infection studies play a critical role in advancing our knowledge of infectious diseases and developing therapeutics. However, these studies involve knowingly exposing research participants to pathogens, raising significant ethical, social, environmental and logistical concerns. Controlled human infection studies are being conducted in settings where individuals are economically disadvantaged and may have limited access to healthcare, education, and basic social amenities, making community engagement a central element in ensuring the ethical conduct of controlled human infecti

Online Remote Behavioural Intervention for Tics (ORBIT-UK): protocol of a single cohort usability study.

INTRODUCTION: Tourette syndrome is a common, disabling childhood-onset condition. Exposure and response prevention (ERP) is an effective treatment for tics, yet access remains limited due to a shortage of trained therapists and uneven geographical distribution of services. The ORBIT trial demonstrated that internet-delivered ERP is both clinically and cost-effective, but was developed on a university research platform, not suitable for widescale roll-out. To enable adoption by the National Health Service (NHS) in England, ORBIT has been redeveloped on an NHS compliant platform. This study will
