Fracture of the femur with accompanying arterial injury represents approximately 1% of all femoral fractures. Controversy exists regarding the choice of fixation and the sequence of fixation and vascular repair. We report on the treatment of six patients with seven distal femoral fractures and angiographically documented arterial injuries treated over a 20-month period. The treatment protocol consisted of angiography followed by provisional external fixation and early primary exchange to an intramedullary nail. Five of the seven fractures were open. Three fractures were caused by blunt trauma,

OBJECTIVE: To assess the risk of soft-tissue injury during percutaneous placement of external fixation pins in the proximal radius.
DESIGN: An anatomic study with embalmed cadaver limbs.
SETTING: Hand and upper limb centre at a university-affiliated hospital.
INTERVENTIONS: Two 4-mm Hoffman half pins were percutaneously placed along the dorsoradial ridge of the radius, four finger breadths proximal to the radial styloid process.
MAIN OUTCOME MEASURES: Injuries to soft tissues including tendons, nerves and vessels were noted.
RESULTS: Nerve or tendon injuries occurred in 7 of 26 forearms.

The article describes the application to a minimal open screw-osteosynthesis combined with a fixateur externe following dislocated articular multifragment fracture of the radius. No extensive uncovering of the fragments was performed. The results seen at follow-up examination in 12 patients are presented.

Repair of a comminuted, spiral oblique, proximal diaphyseal femoral fracture in a 7-day-old calf was achieved by use of an intramedullary pin, cerclage wires, and external fixator. Six stainless steel wires were used for full cerclage to secure a long butterfly fragment and multiple incomplete and complete nondisplaced fragments to the femur in order to create 2 principal fragments. Axial alignment and resistance to bending was provided by a round, double-pointed, end-threaded intramedullary pin (6.35 mm in diameter), which was inserted in a retrograde fashion. A type-1, double-connecting-bar

From January 1987 to May 1989, a total of 38 adult cases were treated with 15 femoral and 25 tibial shaft shifting procedures from external fixation to reamed intramedullary nailing and followed up for at least 1 year (average, 23 months) at the authors' institution. The indications for the shifting procedures included routine sequential femoral external fixation and failed tibial external fixation treatment such as loss of reduction, atrophic healing process and inability of patients to tolerate casting. The delay period for the shifting procedure was 5 days in the femur and 48 days in the ti

Ten cases seen over the last eight years were reviewed by clinical and radiological re-examination one year after injury. The results encourage us to continue treating young patients with critical soft-tissue injury, compartmental syndrome, open fracture and multiple injury by this technique.

Thirty-eight patients in whom a fracture of the posterior malleolus was shown to comprise 25 per cent or more of the articular surface on the lateral radiograph were followed for an average of forty-four months (range, twenty-four to ninety-nine months). All injuries were judged to be a result of external rotation or abduction of the talus. Fifteen patients had fixation of the posterior malleolus, and twenty-three did not. Open reduction and internal fixation was carried out on all associated fractures of the medial and lateral malleoli. Satisfactory reduction of the posterior malleolus was of

Interfragmentary lag screw internal fixation was combined with external fixation to treat five difficult oblique lower extremity fractures. Uncomplicated primary bone healing was achieved in all five cases in spite of crush injury, devascularization, or local infection. This method represents a treatment option for the difficult oblique fracture and reconfirms the role of fixation in fracture biology.

External compressive fixators were used for fixation after a femoral trochanteric osteotomy for severe slipped capital femoral epiphysis in two large teen-aged boys. The method was found to simplify both the operative fixation and the postoperative management. However, the healing time seemed to be prolonged.

Distal radius fractures are common in elderly patients and often lead to functional impairment. This study compared closed reduction with external fixation (EF) and open reduction with locking plate fixation (LP) in elderly patients. A retrospective analysis was conducted on 80 elderly patients with unilateral distal radius fractures treated in our hospital from 2021 to 2024. Patients were divided into a closed reduction and EF group (n = 40) and an open reduction and locking plate internal fixation group (n = 40), according to the surgical procedure. General demographic and fracture-related d

Purpose: The main focus of this study is to investigate the clinical efficacy of external fixation treatment using removed locking plates in patients with failed debridement and antimicrobial therapy of infection after internal fixation of fractures. Patients and Methods: From January 2019 to January 2023, our medical institution treated 13 patients who had failed debridement and antimicrobial therapy for infection after internal fixation of fractures. All patients had their internal fixation devices removed, underwent thorough debridement, and then the removed locking plates were repurposed f

BACKGROUND: Peripheral nerve block is preferable for lower extremity surgery because it sufficiently blocks pain pathways at different levels providing excellent anaesthesia at the site of surgery. We designed this study to compare the efficacy and safety of SOFT block (sciatic-obturator-femoral technique) compared with spinal anaesthesia in patients undergoing surgery for fixation of open tibial fractures using Ilizarov external fixator.
METHODS: One hundred and seven patients ASA I, II scheduled for fixation of open tibial fractures using Ilizarov external fixator. The patients were randoml

OBJECTIVES: To compare the rates of deep infection, flap failure, and nonunion after temporary internal fixation (TIF) with temporary external fixation (ex-fix) in the management of Gustilo-Anderson type IIIB open tibia fractures.
DESIGN: Retrospective comparative cohort study.
SETTING: Level 1 trauma center.
PATIENTS/PARTICIPANTS: Sixty-four consecutive patients with a Gustilo-Anderson type IIIB open diaphyseal tibia fracture (OTA/AO 42) were treated between May 2014 and May 2016. Forty-seven patients (TIF = 24; ex-fix = 23) met the inclusion criteria and underwent 2-stage reconstruction.

UNLABELLED: Early open reduction and internal fixation (ORIF) with plates and screws for plafond injuries caused by skiing initially reported by Ruedi and Allgower proved inadequate for the treatment of high-energy motor vehicle accident type injuries. The purpose of our study was to review our treatment protocol using a spanning external fixator placed semi-emergently medially across the joint and a later staged ORIF of just the articular surface to achieve and maintain anatomic reduction.
METHODS: We preformed a retrospective study of 35 patients with 37 highly comminuted severe (OTA 43-B3

BACKGROUND: Fracture related infections (FRI) are more commonly associated with open fractures. Currently available diagnostic criteria for FRI can diagnose them once infection is clinically apparent. Novel biomarkers procalcitonin and interleukin-6 have established a role in periprosthetic joint infections. We conducted this study to determine their role in predicting FRI in patients with open fractures undergoing staged, early definitive fixation conversion.
METHODS: this was prospective, cohort study included patients undergoing definitive fixation of long bones within 2 weeks of primary e

BACKGROUND: It is unclear whether postoperative pain differs by treatment type for patients with severe open tibial fractures.
METHODS: We performed a secondary analysis of data from the FIXIT study. Adults with severe open tibial fractures were randomized to undergo definitive modern external ring fixation (n = 122) or internal fixation (n = 132). Primary outcomes were pain intensity and interference at 6 and 12 months, measured by the Brief Pain Inventory. Secondary outcomes were Numeric Pain Rating Scale (NPRS) scores and the incidence of moderate to severe pain. Post hoc subanalysis compa
