Classification of juvenile idiopathic arthritis: should family history be included in the criteria?

OBJECTIVE: (i) To determine the efficacy of the Durban classification for children with juvenile idiopathic arthritis (JIA) where < 5 joints were involved at onset (with systemic arthritis excluded) by determining the proportion of the cohort that proved to be "unclassifiable"; (ii) to define reasons for cases being "unclassifiable," particularly regarding family history; and (iii) to compare the efficacy of a proposed hierarchical system (an unofficial modification of the Durban classification) with the Durban classification, where family history details are included as descriptors, rather th

Long-term follow-up study of patients with hypospadias focusing on their sex life.

BACKGROUND: Hypospadias is a well-known congenital malformation (approximately 18.6 new cases reported per 10 000 male newborns); however, little is known about the long-term effects of corrective surgery on patients' sex life in adulthood. AIM: Our aim was to identify how surgical outcomes contribute to patients' and partners' satisfaction with the operated penis and to the ability to have a satisfying sex life and father children, and to better understand which aspects of corrective surgery are the most important. METHODS: We analyzed the clinical data of 501 adult patients who were operat

Sex-differences in the intergenerational transmission of mental disorders among schizophrenia probands: familial risk and protective factors in a population-based study.

BACKGROUND: Parental schizophrenia confers intergenerational mental health risks. The sex-specific transmission patterns remain poorly quantified. This study examined parent-reported transmission rate and familial risk factors among Chinese offspring of schizophrenia probands. METHODS: In this cross-sectional study, we enrolled 27,315 schizophrenia probands and 35,772 offspring. Psychiatric disorder diagnoses of offspring were parent-reported and subsequently verified via the medical management system. Age- and sex-standardized rate referenced China's 2020 census. Robust Poisson regression ge

Concerned significant others of people with gambling problems in Finland: a cross-sectional population study.

BACKGROUND: Problem gambling not only impacts those directly involved, but also the concerned significant others (CSOs) of problem gamblers. The aims of this study were to investigate the proportion of male and female CSOs at the population level; to investigate who the CSOs were concerned about; and to investigate sociodemographic factors, gender differences, gambling behaviour, and health and well-being among CSOs and non-CSOs. METHODS: The data (n = 4484) were based on a cross-sectional population study. Structured telephone interviews were conducted in 2011-2012. The data were weighted ba

Splitting as a consequence of severe abuse in childhood.

Clinical data suggest that Kernberg's description of splitting as a defense mechanism is useful in conceptualizing the psychological consequences of abuse in childhood in certain patients. The splitting in these patients is similar to his description of splitting in borderline patients in that it compartmentalizes and sequesters certain overwhelming and painful ego states accompanying negative introjects of the betraying primary object and the betrayed self. These sequestered introjects, furthermore, act as automatons, generating behaviors that arbitrarily re-enact their content even though th

Parental genetically predicted liability for coronary heart disease and risk of adverse pregnancy outcomes: a cohort study.

BACKGROUND: Adverse pregnancy outcomes (APO) may unmask or exacerbate a woman's underlying risk for coronary heart disease (CHD). We estimated associations of maternal and paternal genetically predicted liability for CHD with lifelong risk of APOs. We hypothesized that associations would be found for women, but not their male partners (negative controls). METHODS: We studied up to 83,969‬ women (and up to 55,568‬ male partners) from the Norwegian Mother, Father and Child Cohort Study or the Trøndelag Health Study with genotyping data and lifetime history of any APO in their pregnancies (1967-

Perspectives on male partner notification and treatment for syphilis among antenatal women and their partners in Kampala and Wakiso districts, Uganda.

BACKGROUND: Syphilis screening can be successfully integrated into antenatal clinics, and potentially avert significant morbidity and mortality to unborn infants. A minority of male partners report for testing and treatment, increasing the likelihood of reinfection. We conducted a qualitative study to understand factors influencing male partners to seek treatment after syphilis notification by their pregnant partners. METHODS: A purposeful sample of 54 adults who participated in the STOP (Syphilis Treatment of Partners) study was stratified by gender (24 women, 30 male partners) and enrolled

Report from the National Transplantation Pregnancy Registry (NTPR): outcomes of pregnancy after transplantation.

Specific data on pregnancies following transplantation continue to accrue in the National Transplantation Pregnancy Registry (NTPR) in each type of organ recipient, with the following conclusions: 1. While the majority of kidney recipients appear to tolerate pregnancy well, a small percentage develops rejection, graft dysfunction and/or graft deterioration. Overall, there is a slight increase in the mean postpartum creatinine level when compared with the prepregnancy level, which has been noted in previous investigations by the NTPR. One neonatal death attributed to thrombotic cardiomyopathy w

Performance monitoring is altered in adult ADHD: a familial event-related potential investigation.

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is a common neurodevelopmental disorder that starts in childhood and frequently persists in adults. Electrophysiological studies in children with ADHD provide evidence for abnormal performance monitoring processes and familial association of these processes with ADHD. It is not yet known whether these processes show the same abnormalities and familial effects in adults. METHOD: We investigated event-related potential (ERP) indices of performance monitoring in adults with ADHD compared to age matched control participants. We subsequen

Early childhood respiratory tract infections according to parental subfertility and conception by assisted reproductive technologies.

STUDY QUESTION: Are children conceived by ART or born to subfertile parents more susceptible to upper or lower respiratory tract infections (URTI, LRTI)? SUMMARY ANSWER: ART-conceived children had a higher frequency of and risk of hospitalization for respiratory infections up to age 3, which was only partly explained by parental subfertility. WHAT IS KNOWN ALREADY: Some studies report increased risks of infections in children conceived by ART. Results for URTIs and LRTIs are inconclusive, and the contribution of underlying parental subfertility remains unclear. STUDY DESIGN, SIZE, DURATION:

Health-related quality of life and influencing factors in parents of children with congenital heart disease: a systematic review and meta-analysis.

INTRODUCTION: Parents of children with congenital heart disease (CHD) have been described as 'hidden patients,' which negatively impacts their health-related quality of life (HRQoL). The extent of HRQoL impairment among parents of children with CHD and its contributing factors remain controversial. This systematic review and meta-analysis aimed to systematically examine the HRQoL in parents of children with CHD and sought to determine the influencing factors. METHODS: A comprehensive search for articles was performed via CINAHL, Embase, PsycINFO, PubMed, Web of Science, China National Knowled

PICU diaries and parental PTSD symptoms-a pilot randomized controlled trial assessing feasibility and psychological outcomes.

INTRODUCTION: Parents of children admitted to pediatric intensive care units (PICUs) are at increased risk of post-traumatic stress disorder (PTSD), potentially affecting long-term family well-being. This analysis evaluates the feasibility of a pilot randomized controlled trial (RCT) of PICU diaries, including recruitment, retention, and data collection, while providing insights into intervention implementation. Additionally, selected exploratory findings are reported, including parental trauma-related symptoms at six months post-discharge and the potential influence of gender and resilience o

Low reliability of DNA methylation across Illumina Infinium platforms in cord blood: implications for replication studies and meta-analyses of prenatal exposures.

BACKGROUND: There is an increasing interest in the role of epigenetics in epidemiology, but the emerging research field faces several critical biological and technical challenges. In particular, recent studies have shown poor correlation of measured DNA methylation (DNAm) levels within and across Illumina Infinium platforms in various tissues. In this study, we have investigated concordance between 450 k and EPIC Infinium platforms in cord blood. We could not replicate our previous findings on the association of prenatal paracetamol exposure with cord blood DNAm, which prompted an investigatio

Protocol for a randomised controlled trial of a co-produced, complex, health promotion intervention for women with prior gestational diabetes and their families: the Face-it study.

BACKGROUND: Gestational diabetes mellitus (GDM) is associated with an increased risk of future diabetes in both mother, father and offspring. More knowledge is needed about how to effectively reduce the risk of diabetes through sustained behavioural interventions in these families. The Face-it intervention is a complex health promotion intervention embedded in multi-level supportive environments. The aim of the intervention is to reduce type 2 diabetes risk and increase quality of life among families in the first year following a GDM-affected pregnancy by promoting physical activity, healthy d
