What harm does a second delivery to the pelvic floor?

OBJECTIVE: To compare the pelvic floor function of primiparous women to women after a second delivery regarding symptoms of urinary and anal incontinence, anal sphincter ruptures and bladder-neck mobility. METHODS: A questionnaire evaluating symptoms of urinary and anal incontinence was used in nulliparous women before and 27 months after childbirth. Furthermore these symptoms were correlated with functional changes of the pelvic floor based on a careful gynecologic examination as well as perineal and endoanal ultrasound. RESULTS: 112 nulliparous women were included, 49 women returned for fo

Physical therapies for prevention of urinary and faecal incontinence in adults.

BACKGROUND: Physical therapies, particularly pelvic floor muscle training, have been recommended for prevention of incontinence associated with childbearing, and prostate surgery. OBJECTIVES: To assess the effectiveness of physical therapies in preventing incontinence in adults. SEARCH STRATEGY: The Cochrane Incontinence Group trials register was searched to September 2001. Trials were also sought from the Reference Lists of relevant articles and from experts in the field. SELECTION CRITERIA: Randomised and quasi-randomised trials in adults without incontinence symptoms that compared a phys

The elderly at home: indices of disability.

The physical status of all people aged 75 and over living in and around Melton Mowbray was assessed by the responses to a series of questions on the activities of daily living that the respondents could perform. Three methods of producing an index of disability from these separate questions are investigated: principal component analysis, Guttman scaling, and a variation of Guttman scaling known as severity grading. All the methods produced very similar rankings of persons, confirming the suggestion of Bebbington that the choice of scaling method is of little consequence. Two scales emerged: on

Postcesarean pelvic floor dysfunction contributes to undisclosed psychosocial morbidity.

OBJECTIVE: To estimate the prevalence and severity of postcesarean pelvic dysfunction. STUDY DESIGN: Using biopsychosocial interviewing at home, 184 postcesarean primiparas were compared to 100 vaginally delivered women regarding symptoms of stress incontinence, anal incontinence and dyspareunia. Delivery details were confirmed from medical records. RESULTS: Comparison of postcesarean vs. vaginally delivered women revealed stress incontinence in 33% vs. 54% and dyspareunia in 27% vs. 46%, both differences reaching statistical significance, unlike anal incontinence, which was manifest in 51%

Bioabsorbable synthetic plug in the treatment of anal fistulas.

BACKGROUND: Management of anal fistulas is challenging, because surgeons must aim to obtain complete healing while sparing the sphincter and avoiding fecal incontinence. The optimal treatment method remains unknown. OBJECTIVE: This study assessed the safety and effectiveness of the use of a new synthetic fistula plug made of bioabsorbable polymers in the treatment of cryptoglandular anal fistulas. DESIGN: This study is a retrospective review of a database of patient records. SETTING: : Patients were treated at a general hospital in Italy. PATIENTS: Forty-eight patients (39 men and 9 women;…

Long-term outcome of sphincteroplasty with separate suturing of the internal and the external anal sphincter.

BACKGROUND: Sphincteroplasty is one of the treatment options for anal incontinence following obstetric injury. The aim of the study was to evaluate the long-term effect of sphincteroplasty with separate suturing of the internal and the external anal sphincter on anal continence. METHODS: A retrospective study was conducted on women who had sphincteroplasty for treatment of anal incontinence following obstetric injury. Women operated between January 1, 2011 and December 31, 2014 at Sykehuset Innlandet Hospital Trust Hamar, were invited to answer a questionnaire and participate in a clinical ex

Guidance for the provision of absorbent products for adult incontinence.

The Association for Continence Professionals first published their guidance for the provision of absorbent products for adult incontinence in 2017. This consensus document is targeted towards commissioning leads, NHS Trust Boards, Bladder and Bowel leads, among others, and has been updated over the years to ensure that all adults who suffer with continence issues undergo a comprehensive assessment and have access to an equitable service. This article provides an overview of the latest guidelines which were published in February 2023.

[Incontinence aids].

Incontinence aids are mechanical devices that help reduce, capture, absorb or channel urinary and stool loss. They can exert a significant positive impact on the quality of life and the social integration of those affected, as well as provide relief to their care-givers. Incontinence aids are used as a bridge until conservative and operative treatments bring continence, as a complement to other therapies or as a long-term course of treatment. The selection of appropriate incontinence aids, such as vaginal pessaries, penis clamps, and tampons, urinals, urine and faecal collectors, absorbing aid

Long-term functional, manometric, and endosonographic evaluation of patients operated upon with the Duhamel technique.

Long-term functional results, anal endosonography (AES), and anal canal manometry were recorded in 48 patients aged 10 to 24 years (median 18) operated upon with the Duhamel technique for Hirschsprung's disease; 60.4% had perfect fecal control, 31.3% occasional staining and/or gas incontinence, and 8.3% constant fecal soiling, and 10.4% complained of constipation. Compared to normals, the patients had significantly reduced anal canal resting and squeeze pressures. AES visualized scar tissue in both the internal and external anal sphincter.

Anorectal motility after surgery for spina bifida.

Anorectal motility was studied in 93 children (aged 15 days to 16 1/2 years) who had undergone surgery for myelocele or meningomyelocele, and 80 controls, matched for age and sex. Mean resting pressure in the anal canal of patients was markedly decreased, particularly in the presence of incontinence, and unstable. The recording of a rectoanal inhibitory reflex both in the upper anal canal and at the anal margin was associated with the presence of fecal incontinence and motor disorders of the lower extremities. Patients could be divided into four groups according to manometric pattern: 1) those…

Annual long-term functional outcomes after transanal repair for symptomatic rectocele.

PURPOSE: This study was performed to assess the long-term annual functional outcomes and quality of life (QOL) after transanal rectocele repair. METHODS: We evaluated retrospectively collected data from patients who underwent transanal repair for symptomatic rectocele between February 2012 and December 2018. The Constipation Scoring System (CSS), the Fecal Incontinence Severity Index (FISI), and several QOL questionnaires (e.g., the Patient Assessment of Constipation-QOL [PAC-QOL], Fecal Incontinence QOL, and the 36-Item Short Form Survey [SF-36]) were administered before surgery and annually…
