We have evaluated the clinical outcomes of simple excision, ulnar lengthening and the Sauvé-Kapandji procedure in the treatment of deformities of the forearm in patients with multiple hereditary osteochondromas. The medical records of 29 patients (33 forearms) were reviewed; 22 patients (22 forearms) underwent simple excision (four with ulnar lengthening) and seven the Sauvé-Kapandji procedure. The treatment of a complex forearm injury inflicted by a wartime mine explosion is presented in this study. Apart from the soft tissue damage, a 4-part fracture of the radius and loss of 19 cm of the ulnar diaphysis were present along with lesions of the median and ulnar nerves. We report on a convenient intraoperative distraction technique, called the modified Dynamic Wire-framed Traction (mDWT) method, that assists in fracture reduction and/or is used as an external fixator in patients with unstable distal radius fractures and in other hand and forearm fractures. Tendon transfers are frequently needed to improve hand function in obstetric brachial plexus injuries. The reconstruction cannot always be achieved using local donor transfers in the forearm as these are not always available. The use of non-replantable amputated parts for reconstruction of the stump is a well-established technique. The use of a free fillet flap of the hand and forearm for elbow preservation in massive trauma of the upper extremity is reported in two cases. These free flaps allowed for covering and preserving a functional elbow and a more useful stump.

A short series 8 patients with complications and failures of forearm shafts fractures treated with screws and plates was analysed. There were 2 pseudoarthroses of ulna, delayed union of radial shafts in 4 cases in which 2 had plates loosening. The additional 2 patients had the refractures of radius shortly after metal removal. Intramuscular transplantation of autologous parathyroid tissue in the forearm has been the traditional method of transplantation at the time of total parathyroidectomy. We studied effects of test H-reflex size on reciprocal Ia inhibition in forearm muscles. In both healthy control subjects and hemiplegic patients, the amount of Ia inhibition decreased as the test H-reflex size increased. It is possible that forearm reciprocal Ia inhibition in hemiplegics reported previously might be underestimated due to larger test H-reflexes used in the hemiplegics than in the controls.
