Fracture-dislocations of the tarsometatarsal joint, a combined anatomical and computed tomographic study.

Based on anatomical and computed tomographic data as well as experience with the treatment of 30 patients with fracture-dislocation of the tarsometatarsal (Lisfranc) joint, a pathophysiological model is described in which the shape of the foot and ligamentous configuration in combination with applied forces are of pivotal importance. CT imaging helps to elucidate the extent of the lesions, easily overlooked in straight radiographs. In the transverse plane we discern three grades of dislocation. Grade 1, virtually no displacement; grade 2, dislocation of half of the shaft; grade 3, total displa

A comparative study of the treatment of unilateral fractured and dislocated mandibular condyles in the rabbit.

This study evaluated the results of three methods of managing simulated unilateral dislocated fractured mandibular condyles in the rabbit. The first method consisted of surgical reduction of the fractured condyle to its anatomic position and its subsequent fixation with a titanium plate. The second method differed from the first by completely detaching the condyle, then replacing it as free graft. The third method left the fractured condyle in its dislocated position outside the glenoid fossa. Eighteen rabbits were used, 6 animals for each method. Half the animals in each group were killed at

Fracture of the upper end of the ulna associated with dislocation of the head of the radius in children.

Dislocation of the head of the radius associated with fracture of the upper end of the ulna is a common elbow injury in children. The literature on diagnosis, however, is relatively limited. The injury can be misdiagnosed and consequently mismanaged. In the present series of 46 children, the age range was from 2.5 to 12 years. The dislocation of the radial head was lateral in 50% of the cases, posterolateral in 28.2%, anterolateral in 17.4%, and anterior in 4.3%. Thorough clinical and radiological examinations are essential. Simple bending of the upper end of the ulna may help to avoid missing

Improved results in the treatment of civilian vascular injuries associated with fractures and dislocations.

Patients with vascular injuries associated with fractures or dislocations of the extremities were managed according to a standard protocol, which included the liberal use of preoperative arteriography, early fasciotomy when indicated, individualization of timing and type of orthopedic procedures, arterial reconstruction primarily with interposition reversed saphenous vein grafts, repair of major venous injuries, routine completion arteriography, and regular postoperative monitoring of Doppler-derived ankle/brachial indices. Adherence to these principles led to a limb salvage rate of 97% in 38

Stabilization of thoracic and thoracolumbar fracture-dislocations with Harrington rods and sublaminar wires.

Seventeen unstable thoracic and thoracolumbar fracture-dislocations have been treated by Harrington double-distraction rod technique supplemented by doubled 18-gauge stainless-steel wires applied to two intact laminae above and below the injury. Fourteen of the 17 cases were grossly comminuted and translated beyond 50% of the width of the spinal column. No external bracing was used in the 13 patients with neurologic deficits. At the time of follow-up examinations (greater than 22 months in all patients) there was no loss of reduction or fixation in any patient. Solid fusion was achieved in all

An analysis of the late effects of traumatic posterior dislocation of the hip without fractures.

The long-term results of 74 cases of simple traumatic dislocation of the hip are reported and the effects of the cause of dislocation and of the occupation and age of the patient on the prognosis are assessed. The average follow-up was 14.65 years. Contrary to the widely held view that there are no long-term complications of this injury, we found that, overall, 24 per cent of the dislocated hips went on to develop osteoarthritis. The incidence was highest in manual workers with 37.5 per cent of miners injured in car accidents developing osteoarthritis compared with only 20 per cent of the sede

[Experiences with the Balser plate in dislocations of the acromioclavicular joint and lateral fractures of the clavicle].

Surgery of acromioclavicular luxations and lateral fractures of the clavicula with the Balser plate is described as a reliable low-complication method on the basis of the authors' own experiences in 13 patients. This treatment method offers significant advantages over the other conventional methods of surgery. Early mobilization of the patients is possible if exercise stability is present. Costly and complicated immobilization can be omitted, and so can reconstruction of the coracoclavicular ligaments or syndesmoplasty. The period of disability is shortened.

Posterior fracture-dislocation of the distal part of the fibula. Mechanism and staging of injury.

The Bosworth fracture, a fixed posterior fracture-dislocation of the distal part of the fibula due to external rotation of the supinated foot, is a rare injury. In this report we review the literature, present two new cases, and describe our cadaver studies, showing that the initial stages of the injury are identical to those of a Maisonneuve fracture. The final stages that are unique to this fracture-dislocation are the posterior dislocation and fracture of the fibula and the fracture of the medial malleolus or rupture of the deltoid ligament. The objectives of treatment are reduction of the

[Surgical management treatment of dislocation fractures of the hip. Classification--indications--results].

30 fracture-dislocations of the hip were operated in the years 1967-1973. Indication, operative approach, X-ray diagnostic and complications are discussed. Postoperatively continuous traction is applied for some weeks, 26 patients were followed up with 11 very good, 10 good, 3 fair and 2 poor results. It is pointed out, that open reduction with interfragmentar compression - osteosynthesis is a real progress in the treatment of irreducable fracture-dislocations of the hip.

An Exceptionally Uncommon Hip Injury: Femoral Neck Fracture Dislocation with Abductor Avulsion: A Case Report.

CASE: This report presents an extremely rare case of a femoral neck fracture dislocation with associated complete hip abductor avulsion. The patient was treated with open reduction of the dislocation, open reduction internal fixation (ORIF) of the femoral neck, and repair of the hip abductors. The patient initially healed but developed avascular necrosis (AVN) requiring conversion to total hip arthroplasty at 3 years. A Harris Hip Score was 92 eleven years after injury. CONCLUSION: We recommend prompt, anatomic ORIF of the femoral neck fracture dislocation, direct repair of the hip abductor c…

Thoracic Fracture-Dislocation with Complete Vertebral Body Translation and Spinal Cord Transection: A Case Report.

CASE: A 46-year-old man suffered thoracic spine fracture dislocation in a motorcycle accident, presenting with complete lower extremity paralysis. Imaging revealed a T8 vertebral dislocation and complete spinal cord transection. Urgent surgery approximately 36 hours postinjury included posterior fixation, bone grafting, duraplasty, and spinal cord stump-plasty. Postoperatively, his neurological status remained unchanged. Minimal cerebrospinal fluid leakage resolved; no other major complications developed. He is stable and mobile through a wheelchair. CONCLUSION: Prompt surgical stabilization …

Dorsal Perilunate Dislocation Coupled With a Distal Radius Fracture: A Report of a Rare Case.

High-energy traumatic injuries to the wrist have been reported in the literature, which may result in isolated perilunate dislocations or distal radius fractures (DRFs). Dorsal perilunate dislocations are reported to be more common compared to volar dislocations. The concurrent presentation of a dorsal perilunate dislocation with a DRF has been rarely reported. Objective post-operative outcomes of this injury pattern, such as active motion, sensation, and function, are important with follow-up because of the complexity of this condition. The purpose of this report is to present the clinical pr
