Surgical treatment of intra-articular fractures of the trapezium.

Eleven patients with intra-articular fractures of the trapezium were evaluated after surgical treatment with a mean follow-up time of 47 months (range, 25-80 mo). There were 6 vertical split and 5 comminuted fractures. All fractures involved high-energy trauma in men. Five resulted from motorcycle accidents. An associated Bennett's fracture occurred in 4 patients. Three fractures went undiagnosed at the time of initial evaluation. At late follow-up evaluation, 8 of 11 patients experienced some pain, mainly at the trapeziometacarpal articulation. There was no statistical difference in thumb mot

Non-vascularized Fibular Autograft and Locking Plate: A Solution to the Resistant Nonunion of a Tibial Metaphyseal Fracture.

The union of tibial fractures often raises concerns. In one such case, a 62-year-old female patient presented in our Emergency Room (ER) with a comminuted tibial metaphyseal fracture resulting from a traffic accident. The patient underwent three surgical procedures in the following two years. The first intervention was open reduction internal fixation (ORIF) with a plate and screws. The second intervention, which took place three months after the first surgery, addressed the union delay through implant removal and closed reduction and internal fixation (CRIF) with an antegrade intramedullary t

Risk Factors and Interventional Predictors for Postoperative Malunion and Nonunion in Adult Mandibular Fractures: A Scoping Review.

Mandibular fractures are one of the most commonly treated injuries in oral and maxillofacial surgery. While malocclusion has been well studied as a postoperative complication, malunion and nonunion remain comparatively unreported despite being significant complications associated with the healing of facial fractures. This scoping review aims to evaluate risk factors, predictors, surgical variables, and management strategies associated with postoperative malunion or nonunion in mandibular fractures. PubMed, Embase, Web of Science, and Google Scholar were searched for English-language studies pu

The effect of osteochondral fragment loss on maximal tibiotalar articular stress in posterior malleolus fractures: A finite element study.

INTRODUCTION: Posterior malleolus fractures are frequently associated with varying degrees of comminution. This comminution often leads to osteochondral fragment loss from the posterior articular surface of the tibial plafond. The purpose of this study is to use finite element modeling to determine whether osteochondral defects at the posterior malleolus fracture interface significantly influence tibiotalar contact stress. METHODS: 3D models of 10 randomly selected patients were created of the tibia and talus from CT scans. A layer of cartilage was added to simulate contact at the tibiotalar

Extra-Articular Distal Tibia Non-Union Treated With Ipsilateral Fibular Strut Graft with Implants In Situ - A Case Report.

INTRODUCTION: Distal tibial comminuted fractures are usually a result of high-energy trauma. The subcutaneous location of the bone pre-disposes to open fractures and decreased vascular supply. All these local and biological factors contribute to increased risk of non-union. Mechanical factors such as stability of the fixation and method of fixation also add to it. Hence, it is prudent to evaluate all these factors in a case of distal tibial non-union and choose optimal method of management of distal tibia non-union. This case presentation is portrayal of one such scenario. CASE REPORT: A 65-y

Type 1 Monteggia Equivalent Fracture with Ipsilateral Distal Radius Fracture: A Case Report.

In this case report, we present a 13-years-old patient who sustained a Monteggia equivalent fracture along with an ipsilateral distal radius fracture following a fall on the elbow. Comminuted ulnar fracture was treated with open reduction and internal fixation with a bridging plate. After restoring the ulnar length, the radial neck fracture was successfully reduced. The distal radius fracture was managed conservatively. Our literature review shows that, the patient is one of the comparatively older patients treated with open reduction and internal fixation in this area and that a successful ou

Treatment of unilateral cervical facet fractures without evidence of dislocation or subluxation: a narrative review and proposed treatment algorithm.

Isolated cervical spine facet fractures are often overlooked. The primary imaging modality for diagnosing these injuries is a computed tomography scan. Treatment of unilateral cervical facet fractures without evidence of dislocation or subluxation remains controversial. The available evidence regarding treatment options for these fractures is of low quality. Risk factors associated with the failure of nonoperative treatment are: comminution of the articular mass or facet joint, acute radiculopathy, high body mass index, listhesis exceeding 2 mm, fragmental diastasis, acute disc injury, and bil

Treatment of Capitellum and Trochlear Fracture using Headless Screw with Concomitant Olecranon Fracture Through Posterior Approach: A Case Report.

Articular fracture of distal humerus account for 2% of all adult elbow fracture. The coronal shear fracture combined with olecranon fracture require stable anatomical reduction and stabilisation for early rehabilitation and mobility. Numerous reports have described various approaches in tackling this type of fracture and method of fracture fixation. Here, we shared a case of a lady with traumatic coronal shear capitulum and trochlear comminuted fracture fixed using posterior elbow approach with fracture site open window technique in managing distal humerus articular fracture with ipsilateral o

Intercondylar fractures of the distal humerus in adults.

Intercondylar fractures of the distal humerus in adults are difficult fractures to treat because of their rarity and associated significant comminution. The results of managing these fractures non-operatively are limited by failure to get anatomical reduction and early mobilization, which often results in painful stiff elbow and/or pseudarthrosis. Hence an operative management with anatomical reduction of the fragments becomes the treatment of choice for these fractures. The experience of managing 20 such fractures by open reduction and internal fixation with early mobilization is presented. N

Closed nailing of tibial shaft fractures.

A total of 98 tibial shaft fractures were internally fixed by closed intramedullary nailing. Twenty-seven of the fractures were open and 71 were closed. In 62 patients (67 per cent) the fracture was the consequence of road traffic accidents; 32 patients (35 per cent) had other skeletal and soft tissue injuries. All the fractures united with a good range of knee and ankle movements; no disuse atrophy was encountered. Most patients returned to work early. In 14 patients comminution of the fracture resulted in up to 2 cm of shortening. Eight tibial fractures had delayed union. It is concluded tha

Humeral capitellum fracture combined with humeral lateral column injury: A novel classification system and treatment algorithm.

BACKGROUND: Shear humeral capitellum fracture (CF) treatment can be complicated by comminution of the distal lateral humeral column (LHC). Although treatment with a 3.5 mm posterolateral distal humerus plate with support (PDHPWS) has been proposed, its indications have not yet been outlined. The purpose of this study was to describe a classification system for this fracture pattern and provide a therapeutic algorithm to avoid complications associated with PDHPWS fixation. METHODS: Thirty-four patients who underwent surgical treatment for CF with LHC comminution were enrolled. The humeral capi

Volar long locking compression plate fixation for distal radius fractures with metaphyseal and diaphyseal extension.

Comminuted distal radial fractures with metaphyseal and diaphyseal extension are uncommon and remain a challenge to treat. The purpose of this study was to assess the radiographic and functional outcomes of treatment with the volar long locking compression plate (LCP) system for distal radius fractures with metaphyseal and diaphyseal extension. This retrospective study was performed on 22 consecutive patients who were treated with open reduction and internal fixation with the application of a 2.4-mm-extra-long LCP volar distal radius plate. Patients were evaluated based on clinical signs and r
