Unveiling the enigma: A rare case of eosinophilic granuloma.

Eosinophilic granuloma is a benign and mildest subtype of Langerhans cell histiocytosis. It has a predilection for males with age less than ten years. It presents with localized swelling in the skull, long bones, or rarely mandible. We report the case of a 17-year-old male who presented with swelling in the frontal area of his head which was tender. On radiographic imaging, ovoid lytic lesions with beveled edges were noticed. On computed tomography multiple osteolytic lesions in the bilateral frontal region focally eroding both the inner and outer cranial tables were seen. Upon confirmation of

Anterior skull base surgery for benign and malignant disease.

Teamwork between the head and neck surgeon and the neurosurgeon utilizing the craniofacial resection technique greatly extends the frontiers of surgery for tumors of the anterior half of the skull base. Tumors for which this technique may be used include benign and malignant tumors arising in the frontal bone or frontal sinus, nasal vault, ethmoid, maxilla, sphenoid or orbit. Selected nasopharyngeal lesions such as angiofibromas with anterior or middle fossa extensions and sphenoid or clival chordomas may also be approached in this manner. This report evaluates the surgical techniques currentl…

Progressive frontal intraosseous lipoma: Detection of the mosaic AKT1 variant discloses Proteus syndrome.

Proteus syndrome is a very rare disorder with progressive, asymmetrical, and disproportionate overgrowth of body parts with a highly variable phenotype. It is associated with mosaicism for the recurrent heterozygous somatic gain-of-function variant c.49G>A (p.Glu17Lys) in the protein kinase AKT1. We report on a girl with a progressive intraosseous lipoma of the frontal bone and additional, nonspecific features including mild developmental delay, strabism, and a limbal dermoid of the left eye. She did not fulfill the criteria for a clinical diagnosis of Proteus syndrome. However, mutation analy…

Cemento-ossifying fibroma involving paranasal sinuses and skull base.

INTRODUCTION: Ossifying fibroma is a rare benign fibro-osseous lesion that usually affects mandible and maxillary bone. Their localisation t … fying fibroma. Two years later a subdural empyema with frontal suppuration necessited to remove the frontal bone flap, which was re-constructed 12 months later using a synthetic material. CONCLUSION: Ossifying fibroma is a rare, benign tumour witch may recur if incomplete resection is performed. A long follow up with CT scan and MRI is required.

Pott's Puffy Tumor Presenting as Pyogenic Meningitis in an Infant.

Introduction. Pott's puffy tumor is characterized by the osteomyelitis of the frontal bone with underlying subperiosteal abscess, mostly occurring secondary to recurrent sinusitis or head trauma. Though it is a rare clinical entity in this antibiotic era, its occurrence mostly in the adolescent age group has now shown increased reporting lately in all age groups. Case Description. We describe here a case of a 4½-month-old female baby who presented to our hospital's Emergency Room with clinical features of pyogenic meningitis following aspiration of a midline frontal swelling. The infant presen…

Neuroimaging findings of extensive sphenoethmoidal dysplasia in NF1.

Whereas isolated sphenoid wing dysplasia (SWD) is a well-known clinical feature in neurofibromatosis 1 (NF1), extensive cranial defects involving multiple bones have been rarely reported in this disorder. In this report, we describe the clinical course of a 20-year-old male with NF1 and an extensive cranial bone dysplasia. The large sphenoethmoidal defect was associated with transethmoidal and orbital cephalocele as well as inferolateral herniation of the frontal lobe. In spite of the large defect, the individual did not have any symptoms or complications resulting from the osteopathy. We revi

Dose-Effect Analysis of Early Changes in Orbital Bone Morphology After Radiation Therapy for Rhabdomyosarcoma.

PURPOSE: In survivors of orbital embryonal rhabdomyosarcoma (ERMS), late effects include facial deformation and asymmetry. We sought to quantify orbital asymmetry in ERMS survivors and characterize the dose effect of radiation to the orbital bones. METHODS AND MATERIALS: We evaluated the most recent follow-up magnetic resonance imaging (MRI) in 17 children (≤21 years old) with stage 1 group III orbital ERMS treated with proton therapy between 2007 and 2018. For all patients, the orbital socket volumes were calculated and compared with the contralateral, unirradiated orbital socket. Patient ag

Frontal anaplastic oligodendroglioma showing multi-organ metastases after a long clinical course. Case report.

A 17-year-old woman presented with an anaplastic oligodendroglioma manifesting as generalized seizure. Neuroimaging studies revealed a right frontal tumor. Histological examinations of biopsy specimens revealed that the tumor was oligodendroglial in nature. Total resection was repeated four times, and malignant change was evident within the tissues. The final diagnosis was anaplastic oligodendroglioma. Despite irradiation, combination chemotherapy, and interstitial hyperthermia, the tumor grew rapidly but was confined to the cavity created by previous removal operations. She suffered bone pain

[A case of recurrent bacterial meningitis by delayed cerebrospinal fluid (CSF) leakage due to a head trauma].

A 42-year-old man was admitted due to recurrent bacterial meningitis, as he had been treated here for bacterial meningitis three years prior to the current event. He had a remote history of head injury that he had almost forgotten, and his laboratory data showed no immunodeficiency state. 111In-DTPA cisternography showed an abnormal radioactive accumulation in the frontal lobe adjacent to the left frontal sinus at 23 hours after intrathecal injection, and MPR CT images revealed the left frontal sinus bone fracture. These findings indicated that he had a head injury by which a delayed CSF fistu

Cranial Base Reconstruction and Secondary Frontal Advancement for Meningoencephalocele Following LeFort III Osteotomy in a Patient with Crouzon Syndrome: Case Report.

Patients with Crouzon syndrome have increased risks of cerebrospinal fluid rhinorrhea and meningoencephalocele after LeFort III osteotomy. We report a rare case of meningoencephalocele following LeFort III midface advancement in a patient with Crouzon syndrome. Over 10 years since it was incidentally found during transnasal endoscopic orbital decompression, the untreated meningoencephalocele eventually led to intermittent clear nasal discharge, frontal headache, and seizure. Computed tomography and magnetic resonance imaging demonstrated meningoencephalocele in the left frontal-ethmoid-maxilla

Multidisciplinary Treatment of a Pediatric Orbital Roof Fracture: Case Report and Literature Review.

Pediatric orbital roof fractures are a relatively rare trauma. In children, fractures of the facial skeleton can be associated with significant morbidity. Potential complications of orbital roof fracture include both neurosurgical complications such as frontal lobe injury, dural tears, or herniation, and ophthalmologic and reconstructive surgery problems such as proptosis, diplopia, and extraocular muscle entrapment. In most cases, surgical intervention is unnecessary, as these fractures are minimally displaced. When surgery is warranted, however, for displaced fractures or those associated wi

Possible Skull Base Erosion After Prolonged Frontal Sinus Stenting.

Frontal sinus stenting is widely used with the goal of maintaining nasofrontal duct patency after sinus surgery. The general recommendation is to leave stents in place for 6 months; however, prolonged stenting up to 6 years has been reported with no complication. We present the first reported case of frontal sinus posterior table and skull base erosion following prolonged frontal sinus stenting. A 57-year-old female presented with chronic sinusitis and nasal obstruction. Imaging revealed pansinusitis with retained stents in each frontal sinus that were placed 8 years prior. On the right, there

Frontal Sinus Volume and Frontal Bar Position Are Associated in Unicoronal Craniosynostosis.

BACKGROUND: Frontal sinus pneumatization plays an important role in brow protrusion, and absence of frontal sinus development may be associated with brow retrusion. Using unicoronal craniosynostosis as a model, the authors studied the relationship among frontal sinus volume, supraorbital retrusion, and brow position. METHODS: The authors conducted a retrospective review of unicoronal craniosynostosis patients with head computed tomographic scans and photographs taken from ages 5 to 18 years. Frontal sinus volume and supraorbital retrusion were calculated using three-dimensional computed tomog

Klippel-Trenaunay-Weber syndrome with hemimegalencephaly.

There are many documented cases of Klippel-Trenaunay-Weber syndrome characterized by the triad of port wine stain, varicose veins, and hypertrophy of bones and overlying tissue. A few cases have even included the additional findings of hemimegalencephaly. We report a case of Klippel-Trenaunay-Weber syndrome with hemimegalencephaly and calvarial hypertrophy, but no evidence of limb or trunk hypertrophy.

Sinus mucocele secondary to craniofacial trauma in a dog.

A sinsus mucocele formed in the frontal sinus of a dog secondary to obstruction of the nasofrontal opening. The dog was successfully treated by surgical enlargement of the nasofrontal opening, curettage of the sinus epithelium, and placement of a fat graft and drain in the sinus. Although mucocele formation is uncommon in animals, it should be considered as a differential diagnosis for masses of the sinonasal area. Characteristic radiographic and cytologic findings should alert the clinician to this diagnosis, and along with aggressive treatment, should shorten the diagnostic and treatment del
