The adolescent with pelvic inflammatory disease: assessment and management.

More than 1 million women in the United States are reported to have pelvic inflammatory disease (PID). Of these women, 70% are under 25 years of age. Many cases go unreported and often are unrecognized, especially in 11- to 15-year-old adolescents. The presentation of PID can be a diagnostic challenge for the clinician. Many clinicians assert that a diagnosis of PID in an adolescent is an indication for hospitalization, simply because of the high rate of noncompliance with a two-drug regimen in this age group. However, many health care plans no longer support hospitalization for uncomplicated

Barriers to Implementing SMART for Asthma in Pediatric Primary Care.

OBJECTIVES: To characterize awareness, use, and perceived barriers to implementing single maintenance and reliever therapy (SMART) in primary care for children aged 5 years or older, hypothesizing high clinician awareness but limited use due to multilevel clinician-, system-, and caregiver barriers. METHODS: We conducted a sequential explanatory mixed-methods study among pediatric primary care clinicians within a practice-based research network. Clinicians completed a survey assessing SMART awareness, prescribing practices, and perceived barriers, guided by the Consolidated Framework for Impl

Systematic review protocol: The effects of theory-based interventions for self-help practices in the management of mild to moderate depression.

BACKGROUND: This protocol outlines a planned systematic review to assess the effects of theory-based interventions for self-help practices in managing mild to moderate depression. Depression is a leading global cause of disability and contributes significantly to the worldwide burden. Self-help practices present promising avenues for care. When grounded in psychological theory, these interventions may offer a structured and effective approach to symptom management. However, their effectiveness varies and often lack a theoretical foundation. The summarized evidence on theory-based interventions

Impact of regional multi-disciplinary team on the management of complex urogynaecology conditions.

INTRODUCTION AND HYPOTHESIS: Following the publication of the National Institute for Health and Care Excellence guidelines on the management of pelvic floor dysfunction, articles speculating on the benefits and costs of local and regional multi-disciplinary teams (MDTs) have been in circulation. To date, there has been no formal assessment of the impact of a regional MDT on the management of women with complex urogynaecological conditions. METHODS: Throughout the existence of the West of Scotland (WoS) Regional Urogynaecology MDT, from May 2010 to December 2015, 60 patients with complex Urogy

Modifiable Risk Factors for Alzheimer Disease and Related Dementias Among Adults Aged ≥45 Years - United States, 2019.

Alzheimer disease,* the most common cause of dementia, affects an estimated 6.5 million persons aged ≥65 years in the United States (1). A growing body of evidence has identified potential modifiable risk factors for Alzheimer disease and related dementias (ADRD) (1-3). In 2021, the National Plan to Address Alzheimer's Disease (National Plan) introduced a new goal to "accelerate action to promote healthy aging and reduce risk factors for Alzheimer's disease and related dementias" to help delay onset or slow the progression of ADRD (3). To assess the status of eight potential modifiable risk fa

Preparedness of Primary Health Care Clinic (PHCC) Providers for the Provision of Oral Pre-exposure Prophylaxis (PrEP) to Adolescent Girls and Young Women (AGYW): Experiences from KwaZulu-Natal, South Africa.

About two-thirds of all HIV infections in 2024 were in sub-Saharan Africa, where adolescent girls and young women (AGYW) bear a disproportionate burden. Daily oral pre-exposure prophylaxis (PrEP) offers an HIV prevention option for AGYW at high risk who are unable to negotiate safer sex with male partners. Primary health care clinics (PHCCs) staffed by nurses are an important access point for health services, including antiretroviral treatment, family planning, and antenatal care. Effective delivery of new and existing services depends on the preparedness and acceptability of interventions amo

Translation, cross-cultural adaptation and psychometric validation of the Brazilian version of the dialysis patient-perceived exercise benefits and barriers scale.

BACKGROUND: Chronic kidney disease (CKD) in dialysis patients compromises musculoskeletal health and reduces physical activity levels. The Dialysis Patient-Perceived Exercise Benefits and Barriers Scale (DPEBBS) was specifically developed to assess dialysis patients' perceptions of exercise. This study aimed to translate, cross-culturally adapt, and evaluate the psychometric properties of the Brazilian version of the DPEBBS (EPAD). METHODS: A cross-sectional study was conducted following the COSMIN guidelines. Psychometric properties assessed included reliability, internal consistency, and co

A Practical Guideline for the Implementation of Shared Decision-making in Complex Ventral Incisional Hernia Repair.

BACKGROUND: Although obtaining preoperative procedural consent is required to meet legal and ethical obligations, consent is often relegated to a unidirectional conversation between surgeons and patients. In contrast, shared decision-making (SDM) is a collaborative dialog that elicits patient preferences. Despite emerging interest in SDM, there is a paucity of literature on its application to ventral incisional hernia repair (VIHR). The various surgical techniques and mesh types available, the potential impact on functional outcomes and quality of life, the largely elective nature of the opera

Communication to promote and support physical distancing for COVID-19 prevention and control: a rapid review.

BACKGROUND: This review is an update of a rapid review undertaken in 2020 to identify relevant, feasible and effective communication approaches to promote acceptance, uptake and adherence to physical distancing measures for COVID-19 prevention and control. The rapid review was published when little was known about transmission, treatment or future vaccination, and when physical distancing measures (isolation, quarantine, contact tracing, crowd avoidance, work and school measures) were the cornerstone of public health responses globally. This updated review includes more recent evidence to exte

Pain education for patients with non-specific low back pain in Nepal: protocol of a feasibility randomised clinical trial (PEN-LBP Trial).

INTRODUCTION: Low back pain (LBP) is the leading cause of years lived with disability in Nepal and elsewhere. Management of LBP that is evidence-based, easily accessible, cost-effective and culturally appropriate is desirable. The primary aim of this feasibility study is to determine if it is feasible to conduct a full randomised clinical trial evaluating the effectiveness of pain education as an intervention for individuals with LBP in Nepal, relative to guideline-based physiotherapy treatment. The findings of the study will inform the planning of a full clinical trial and if any modification

Effect of guided counseling on nutritional status of pregnant women in West Gojjam zone, Ethiopia: a cluster-randomized controlled trial.

BACKGROUND: Undernutrition during pregnancy affects birth outcomes adversely. In Ethiopia, despite nutrition counseling on the maternal diet being given by the health workers during pregnancy, maternal undernutrition is still high in the country. Hence, this study aimed to assess the effect of guided counseling using the health belief model and the theory of planned behavior on the nutritional status of pregnant women. METHODS: A two-arm parallel cluster randomized controlled community trial was conducted in West Gojjam Zone, Ethiopia, from May 1, 2018, to April 30, 2019. The nutritional stat

Computerized predictive models in hospital settings for detecting severe maternal complications: a systematic review protocol.

CONTEXT: Maternal mortality remains a public health problem, especially in low- and middle-income countries. Complications such as hypertension, gestational diabetes, pre-eclampsia, postpartum hemorrhage, and infections are among the main causes of preventable deaths. In this scenario, computerized predictive models have emerged as tools for the early identification of risks and the prevention of serious complications associated with maternal death in hospital care. Therefore, this systematic review is designed to assess the available evidence on the effectiveness of these models when applied
