Abuse-deterrent opioids: an update on current approaches and considerations.

OBJECTIVE: Abuse and misuse of prescription opioids is a significant public health concern. This review examines the strategies used to confer abuse-deterrent properties on opioid abuse-deterrent formulations (ADFs), the characteristics and supporting data for each of the available ADFs, and the role of opioid ADFs as part of a comprehensive opioid risk management plan. METHODS: A PubMed search was performed for articles published within the last 10 years using the search terms "abuse deterrent opioids". Articles were limited to clinical studies and reviews focusing on United States (US) Food

Preparedness activities and research needs in addressing emerging infectious animal and zoonotic diseases.

Emerging infectious animal and zoonotic diseases can inflict significant losses on animal production and public health, and threaten the safety and security of the food system. Threat analysis (forecasting), which monitors the measurable risk indicators of disease emergence, should be in place before the emergence of any threat. Animal and public health authorities develop and regularly re-evaluate disease preparedness, response and recovery plans, based on the 'One Health' principle. These plans should include surveillance, biosecurity measures, communication channels and training for personn

Radiation event medical management (REMM): website guidance for health care providers.

Planning for and exercising the medical response to potential chemical, biological, radiological, nuclear, and explosive (CBRNE) terrorist events are new responsibilities for most health care providers. Among potential CBRNE events, radiological and/or nuclear (rad/nuc) events are thought to have received the least attention from health care providers and planners. To assist clinicians, the U.S. Department of Health and Human Services (HHS) has created a new, innovative tool kit, the Radiation Event Medical Management (REMM) web portal (http://remm.nlm.gov). Goals of REMM include providing (1)

Alcohol Use Disorder Medication Coverage and Utilization Management in Medicaid Managed Care Plans.

IMPORTANCE: Evidence-based, patient-centered treatment for alcohol use disorder (AUD) can include pharmacotherapy with naltrexone, acamprosate, or disulfiram; however, these medications are rarely used. Medicaid managed care plans (MCPs) manage health services for nearly 80% of Medicaid enrollees and are the largest payer for addiction treatment services. Little is known about Medicaid MCP policies for AUD medications. OBJECTIVES: To describe Medicaid MCPs' coverage and management of acamprosate, naltrexone, and disulfiram for AUD and examine associations of plan characteristics and state pol

IDEAL: Maintaining PHC-focused training in a MBChB programme through a COVID-induced innovation.

Responding to the need for authentic clinical training for students in the context of coronavirus disease 2019 (COVID-19), the Stellenbosch University Faculty of Medicine and Health Sciences developed an innovative 12-week longitudinal, integrated rotation for pre-final-year medical students, the Integrated Distributed Engagement to Advance Learning (IDEAL) rotation. This saw 252 students being placed across 30 primary and secondary healthcare facilities in the Western and Northern Cape provinces. With a focus on service learning, the rotation was built on experiences and research of members o

Timing of surgical intervention for developmental dysplasia of the hip: a randomised controlled trial (Hip 'Op).

BACKGROUND: Developmental dysplasia of the hip (DDH) is a very common congenital disorder, and late-presenting cases often require surgical treatment. Surgical reduction of the hip may be complicated by avascular necrosis (AVN), which occurs as a result of interruption to the femoral head blood supply during treatment and can result in long-term problems. Some surgeons delay surgical treatment until the ossific nucleus (ON) has developed, whereas others believe that the earlier the reduction is performed, the better the result. Currently there is no definitive evidence to support either strate

Planning for Long-Term Recovery: The Impact of COVID-19 on Educators Working With Children From Seasonal and Migrant Farmworker Families.

BACKGROUND: Education faced challenges during the COVID-19 pandemic. Although the United States has declared an end to the public health emergency, schools continue to be impacted. While considerable research has focused on the impact of the pandemic on students, fewer studies have focused on educators, and even less on educators who specialize in working with marginalized student populations. METHODS: This study measured the impact of the pandemic impact on educators working with students from seasonal and migrant farmworker families to understand what supports educators need as schools plan

Exploring access to health and social supports for intimate partner violence (IPV) survivors during stressful life events (SLEs)-A scoping review.

BACKGROUND: Survivors of intimate partner violence (IPV) often face increased incidents of violence during stressful life events (SLEs) such as economic recessions, environmental disasters, and pandemics. These events can diminish the effectiveness of both formal (e.g., health, social, justice, labor, community) and informal (e.g., friends, family, neighbors) support systems. Additionally, SLEs exacerbate existing health and social inequities, making it necessary to understand the accessibility of support services during these times. This scoping review investigates access to services by indiv

Redefining the Restorative Interface in Implant Dentistry: Intermediate Multi-Unit Abutments for Enhanced Peri-Implant Tissue Stability.

In tooth replacement therapy with implant-supported prostheses, optimal outcomes depend on a wide range of variables, including anatomic, phenotypic, surgical, and prosthetic factors. Adequate management of the restorative interface is particularly important for achieving predictable esthetic outcomes in the anterior maxilla and for maintaining long-term soft and hard tissue health and stability. This article describes an approach based on the use of intermediate multi-unit abutments on bone-level implants. A comprehensive workflow is presented, integrating digital planning, surgical protocols

U.S. Funding Cuts and Health System Challenges in Community-Based HIV Programmes in Rural KwaZulu-Natal, South Africa: A Qualitative Study.

Community-based organisations (CBOs) play a critical role in implementing HIV programmes, many of which have historically relied on United States (U.S.) donor funding. Recent U.S. funding cuts have disrupted community-based HIV programmes, underscoring the need to understand implementers' experiences to support sustainable service delivery. This study explored how funding instability affected community-based HIV programmes in rural KwaZulu-Natal, South Africa. A qualitative, exploratory design was employed using interviews and focus group discussions with purposively selected youth frontline w

Health surveillance in tackling COVID-19 in Brazil: a scope review.

The management of the COVID-19 pandemic in Brazil was marked by contradictory statements and controversial decisions by the head of the executive branch and the Ministry of Health, creating uncertainty about the effective implementation of federal actions to control the disease. This study aims to describe the main health surveillance strategies adopted by the federal government to combat COVID-19, with a focus on actions aimed at preventing SARS-CoV-2 infection. This is a scoping review conducted between November 2019 and January 2021, using DECS and MeSH descriptors in four databases, in add

Forecasting the Monkeypox Outbreak Using Limited Data: A Case Study of Thailand.

INTRODUCTION: On August 14, 2024, the World Health Organization (WHO) declared monkeypox (mpox) a Public Health Emergency of International Concern (PHEIC). Shortly thereafter, Thailand reported Asia's first confirmed case of the Clade Ib strain. Given Thailand's high international travel volume, anticipating short-term case trajectories is essential for situational awareness. METHODS: We forecast monthly mpox cases using the Department of Disease Control (DDC) data from July 2022 to September 2024 and evaluate Poisson and negative binomial generalized linear models (GLMs), Holt-Winters expone

What Can a Badge Do? A Pilot Study of the Vot-ER Badge in a Community Health Setting.

Civic engagement is an important example of health and health care inequity in the United States, with voting being an active form of civic engagement that allows citizens to be involved in the political processes that directly impact their health. Existing literature indicates that health care workers are willing to assist in voter registration efforts and are a trusted, nonpartisan source for patients. As such, Vot-ER was founded in 2019 to support nonpartisan voter registration efforts through health care sites, initially through emergency departments. Despite improvements in voter represen

Navigating family dynamics and ethical considerations in genetic diagnosis of pulmonary arterial hypertension: insights from in-depth semi-structured interviews.

BACKGROUND: Genetic diagnosis and precision medicine are rapidly advancing, driven by innovations in next-generation sequencing and omic methods. The UK's collaboration between national research initiatives and the National Health Service facilitates translation of research into clinical practice. This rapid transition impacts family dynamics and family planning, and raises ethical concerns, compounded by limited public and practitioner awareness of the long-term consequences of genetic diagnosis. Our objective is to explore the impact of genetic diagnosis on family dynamics and the ethical co

Factors influencing women's decisions regarding birth planning in a rural setting in Kenya and their implications for family planning programmes.

Evidence suggests that socio-cultural barriers that limit the ability of women in sub-Saharan Africa to make decisions regarding contraceptive use and childbearing contribute to the slow uptake of contraception in this region. This paper explores factors influencing women's decisions regarding contraceptive uptake, switching and discontinuation, and the implications of those decisions for family planning programmes. Data were from in-depth interviews that were conducted in 2018 with 42 women who participated in a longitudinal research project implemented in Homa Bay County of Kenya. Participan

Reproductive Life Plan Counseling and Effective Contraceptive Use among Urban Women Utilizing Title X Services.

BACKGROUND: Although the Centers for Disease Control and Prevention and the U.S. Office of Population Affairs recommend inclusion of reproductive life plan counseling (RLPC) in all well-woman health care visits, no studies have examined the effect of RLPC sessions on the decision to use effective contraception at publicly funded family planning sites. RLPC could be a particularly impactful intervention for disadvantaged social groups who are less likely to use the most effective contraceptive methods. METHODS: Using data from 771 nonpregnant, non-pregnancy-seeking women receiving gynecologica
