Electrophysiologic properties of atrial muscle in paroxysmal atrial fibrillation.

The electrophysiologic properties of atrial muscle were studied by programmed atrial stimulation in 42 patients with paroxysmal atrial fibrillation (PAF) and in 53 control patients without PAF. Single premature atrial stimulation was given at the right atrial appendage following 8 basic stimuli with a basic cycle length of 500 ms. Repetitive atrial firing (RAF) was defined as the occurrence of 2 or more successive premature atrial activations following single premature atrial stimulation. Fragmented atrial activity (FAA) was defined as an increase by more than 75% of the duration of the atrial

Contribution of atrioventricular synchrony to left ventricular systolic function in a closed-chest canine model of complete heart block: implications for single-chamber rate-variable cardiac pacing.

This study assessed the impact of atrioventricular (AV) synchrony on characteristics of left ventricular (LV) systolic function during ventricular pacing over a wide heart rate range in a conscious closed-chest canine model of complete AV block. Ten healthy adult dogs underwent thoracotomy during which complete AV block was created by formaldehyde injection, and paired ultrasonic sonomicrometers were positioned on the LV anterior-posterior minor axis. Following recovery from surgery, peak and end-diastolic LV transmural pressure, maximum dP/dt, stroke work, end-diastolic minor axis dimension,

[Electrocardiographic diagnosis in atrioventricular discordance].

The ECG of 37 patients with atrioventricular discordance were studied and divided in two main groups: I) Situs solitus and II) situs inversus, either a) without associated defects or b) with associated defects. It was concluded: 1) The first vector of ventricular activation, oriented in the same sense of that of atrial activation is a fundamental diagnostic sign; 2) The preponderance of anatomical right ventricle, leads to the diagnosis of unassociated defects; 3) The recognition of biventricular hypertrophy obliges to the diagnosis of associated defects; 4) The AQRS deviation and the shorteni

Clinical use of I.C.I. 50172 as an antidysrhythmic agent in heart failure.

I.C.I. 50172 was used to slow the ventricular rate when conventional treatment had failed in 19 patients. All were either in congestive cardiac failure or in the immediate postoperative period following valve replacement. The ventricular rate was controlled in seven out of eight patients with atrial fibrillation, in six out of nine patients with supraventricular tachycardia, and in two patients with sinus rhythm. Important side-effects were not seen.

Why and how to support screening strategies to prevent sudden death in athletes.

Sudden death in athletes occurs because of the existence of hidden cardiovascular disorders which, during effort, may jeopardize the electrical stability of the heart, triggering ventricular tachycardia and/or fibrillation. Apart from rare conditions of ion channel diseases in the setting of a structurally normal heart, in which the disorder may be easily diagnosed on basal or stress test ECG, cardiac abnormalities at risk of causing sudden death may affect the aorta (Marfan syndrome), the coronary arteries (congenital coronary artery anomalies, premature coronary atherosclerosis), the myocard

Heart rate variability derangements in dogs with Chagas disease: a potential indicator of autonomic and cardiac disruption.

OBJECTIVE: To assess heart rate variability (HRV) as a marker of autonomic nervous system disruption and its role in disease progression in dogs with Chagas disease (CD), and to evaluate arrhythmias and conduction abnormalities in symptomatic and asymptomatic groups. METHODS: A prospective observational study was conducted on dogs treated at a small animal hospital in central Texas from August to December 2023. Ambulatory 24-hour Holter monitoring was conducted to assess HRV metrics (proportion of pairs of successive NN intervals differing by > 50 milliseconds and root mean square of the succ

Heart Disease in Children: Cardiac Dysrhythmias.

Cardiac dysrhythmias in children can be due to a structural abnormality or an intrinsic defect in the electrical conduction system of the heart. In a child with a structurally normal heart, the mechanisms for dysrhythmias are the same as in adults, although the incidence and prevalence are different. Supraventricular tachycardias (SVTs) originate above the ventricles. In children, the two peak ages of onset for SVT are from before birth through the first year of life, and from ages 6 to 8 years. In most cases, there is spontaneous clinical resolution within the first year. When treatment is ne

Genetic insights into cardiac conduction disorders from genome-wide association studies.

BACKGROUND: Substantial data support a heritable basis for cardiac conduction disorders (CCDs), but the genetic determinants and molecular mechanisms of these arrhythmias are poorly understood, therefore, we sought to identify genetic loci associated with CCDs. METHODS: We performed meta-analyses of genome-wide association studies to identify genetic loci for atrioventricular block (AVB), left bundle branch block (LBBB), and right bundle branch block (RBBB) from public data from the UK Biobank and FinnGen consortium. We assessed evidence supporting the potential causal effects of candidate ge

Left bundle branch area pacing (LBBAP) Auto Threshold algorithms Evaluation for Conduction System Pacing: The LATECS pilot Trial.

BACKGROUND: Automated threshold measurements (ATM) and output adaptation improved safety and follow-up of cardiac implantable devices (CIED) in the last years. These algorithms were validated for conventional cardiac pacing; however, they were not suitable for permanent His Pacing. Left bundle branch area pacing (LBBAP) is an emerging technique to obtain physiologic cardiac stimulation; we tried to assess if ATM could be applied to this setting. METHODS: Consecutive patients receiving ATM-capable CIED and LBBAP in our hospital were enrolled in this prospective, observational trial; they were

Spontaneous Sinus Rhythm Restoration in Patients With Refractory, Permanent Atrial Fibrillation Who Underwent Conduction System Pacing and Atrioventricular Junction Ablation.

Ablate and pace (A&P) with conduction system pacing (CSP) improves outcomes in patients with symptomatic permanent atrial fibrillation (AF). Data on spontaneous sinus rhythm restoration (SSRR) in this setting are lacking. This study aimed to assess the incidence and the predictors of SSRR in a population of patients with permanent AF who underwent A&P with CSP. Prospective, observational study, enrolling consecutive patients with symptomatic permanent AF (of documented duration >6 months) and uncontrolled, drug-refractory high ventricular rate, who underwent A&P with CSP. Evaluation of Criteria for Left Bundle Branch Capture.

Left bundle branch pacing (LBBP) provides electrical and mechanical synchrony at low and stable pacing output and effectively corrects distal conduction system disease. The criteria for differentiating LBBP from LV septal pacing has not been validated in large trials. There are several electrocardiography-based and intracardiac electrogram-based criteria to confirm LBB capture. In this section, the authors review these criteria and their overall accuracy.

A Continuous Pacing and Recording Technique for Differentiating Left Bundle Branch Pacing From Left Ventricular Septal Pacing: Electrophysiologic Evidence From an Intrapatient-Controlled Study.

BACKGROUND: Left bundle branch pacing (LBBP) is a promising approach for achieving near-physiologic pacing. However, differentiating LBBP from left ventricular septal endocardial pacing (LVS(e)P) remains a challenge. This study aimed to establish a simple and effective method for differentiating LBBP from LVS(e)P and to evaluate their electrophysiologic characteristics. METHODS: LBBP, using continuous uninterrupted pacing and real-time monitoring of electrocardiograms along with intracardiac electrograms, was performed in 97 consecutive patients. We evaluated the electrophysiologic characteri

Electrical Dyssynchrony in Cardiac Amyloidosis: Prevalence, Predictors, Clinical Correlates, and Outcomes.

BACKGROUND: Conduction-system involvement in cardiac amyloidosis (CA) is common. The prevalence, clinical correlates and impact on outcome related to ventricular electrical dyssynchrony in CA remain insufficiently elucidated. METHODS: Data from a prospectively maintained registry of patients with CA diagnosed in the Cleveland Clinic's amyloidosis clinic was used to determine the frequency of electrical dyssynchrony (defined as a QRS > 130 msec). The relation with the clinical profile and clinical outcome was assessed. To determine the impact of hypertrophy on QRS prolongation, a QRS-matched c

Effects of adaptive left bundle branch-optimized cardiac resynchronization therapy: a single centre experience.

BACKGROUND: Adaptive cardiac resynchronization therapy (aCRT) is associated with improved clinical outcomes. Left bundle branch area pacing (LBBAP) has shown encouraging results as an alternative option for aCRT. A technique that can be accomplished effectively using LBBAP combined with coronary venous pacing (LOT-aCRT). We aimed to assess the feasibility and outcomes of LOT-aCRT. METHODS: LOT-aCRT, capable of providing two pacing modes, LBBAP alone or LBBAP combined with LV pacing, was attempted in patients with CRT indications. Patients were divided into two groups: those with LBBAP and LV

Pacing in vasovagal syncope: Physiology, pacemaker sensors, and recent clinical trials-Precise patient selection and measurable benefit.

The role of pacing in vasovagal syncope (VVS) is considered from a physiological basis. Most VVS patients lose consciousness due to hypotension before severe bradycardia/asystole occurs. Patients who benefit from dual-chamber pacing typically are older with highly symptomatic, late-onset, frequent and severe syncope with short/no prodrome and documented severe cardioinhibition. Tilt testing is of value in patients with recurrent unexplained syncope to identify important hypotensive susceptibility stemming from reduced venous return and stroke volume (SV). A negative tilt test in vasovagal pati

Excitatory effect of bradykinin on intrinsic neurons of the rat heart.

The heart receives sympathetic and parasympathetic innervation through the intrinsic cardiac nervous system. Although bradykinin (BK) has negative inotropic and chronotropic properties of cardiac contraction, the direct effect of BK on the intrinsic neural network of the heart is still unclear. In the present study, the effect of BK on the intracardiac ganglion neurons isolated from rats was investigated using the perforated patch-clamp technique. Under current-clamp conditions, application of 0.1 μM BK depolarized the membrane, accompanied by repetitive firing of action potentials. When BK wa
